ICON indeksi, PAR indeksi ve ABO-OGS/cre sistemi kullanılarak çekimli ve çekimsiz tedavilerin sonuçlarının değerlendirilmesi

Abstract
Bu araştırmanın amacı, ICON, PAR ve ABO-OGS/CRE indekslerinin arasında korelasyon olup olmadığının incelenmesi ile birlikte bu indeksler aracılığıyla çekimli ve çekimsiz sabit ortodontik tedavilerdeki tedavi ihtiyacı, tedavi zorluğu ve tedavi sonucunun değerlendirilmesidir. Araştırma materyali; Gazi Üniversitesi Diş Hekimliği Fakültesi Ortodonti Anabilim Dalı'nda 1998-2013 yılları arasında doktora öğrencileri tarafından tedavi edilen 120 olgunun tedavi öncesi ve tedavi sonrası ortodontik modelleri ve panoramik filmlerinden oluşmaktadır. Çalışmaya dahil edilen olgular 12-25 yaşları arasında olup; iskeletsel Sınıf 1, dişsel Angle Sınıf I maloklüzyona sahip, daimi dişlenme döneminde 71 kız ve 49 erkekten oluşmaktadır. Seçilen bireyler 60'ı çekimsiz, 60'ı 4 adet 1. premolar çekimli olmak üzere iki gruba ayrılmıştır. Tüm vakaların tedavi öncesi ve tedavi sonrası materyalleri ICON indeksi, PAR indeksi ve ABO-OGS/CRE sistemi kullanılarak değerlendirilmiştir. Araştırmanın sonucunda; çekimli grubun çekimsiz gruba göre anlamlı derecede yüksek oranda tedavi ihtiyacının var olduğu, tedavi zorluğu açısından değerlendirildiğinde ise çekimli grubun %36,7'sinin 'çok zor' olduğu, %33,3'ünün 'zor' olduğu bulunmuşken çekimsiz grubun %31,7'sinin 'hafif' düzeyde tedavi zorluğu içerdiği gözlenmiştir. ICON ve PAR indeksleri kullanılarak maloklüzyonun iyileşme derecesi değerlendirildiğinde ise çekimli grupta çekimsiz gruba göre daha yüksek olduğu bulunmuştur. ICON indeksi, PAR indeksi ve ABO-OGS/CRE sistemi ile tedavi başarısı değerlendirildiğinde gruplar arasında istatistiksel olarak anlamlı bir fark bulunmamıştır. ICON ile PAR indekslerinin tedavi öncesi ve tedavi sonrası total skorları arasında ise istatistiksel olarak anlamlı bir uyum bulunmuşken, tedavi sonucu açısından her iki indeks ile ABO-OGS/CRE sistemi arasında anlamlı bir uyum gözlenmemiştir. The aim of this study was to find the correlation between ICON index, PAR index and the ABO-OGS/CRE system and to evaluate extraction and nonextraction orthodontic treatment cases according to treatment need, treatment difficulty and the treatment results by the using of these indices. The material of this study includes pretreatment and posttreatment orthodontic models and panoramic films of 120 patients treated by doctoral students between the years 1998 and 2013 at Gazi University Faculty of Dentistry Department of Orthodontics. The patients (71 females, 49 males) were between 12 to 25 years old with Skeletal class 1 and permanent dentition characterized by Angle class I malocclusion. The patients were divided into two groups; a nonextraction group (60 patients) and four first premolar extraction group (60 patients). Pretreatment and posttreatment materials were evaluated by the using of ICON index, PAR index and the ABO-OGS/CRE system. The results showed that the treatment need was significantly higher in the extraction group than the nonextraciton group. In addition we found that 36.7% of the extraction group were classified as 'very difficult' and 33.3% were classified as 'difficult' whereas 31.7% of the nonextraction group were classified as 'mild'. According to both ICON and PAR indices the extraction group showed significant improvement in malocclusion when compared to the nonextraction group. There was no significant difference in treatment success between the two groups according to all indices. Pretreatment and posttreatment total scores showed a positive correlation between ICON and PAR indices whereas no correlation was found between both indices and the ABO-OGS/CRE system according to the posttreatment results.
