Abstract
Bu çalışmada, birbiri ile bağlantılı olmayan fakat birlikte de görülebilen, kalp-damar hastalıkları için risk oluşturabilen, hava yollarını tıkayan ve soluma ile ilgili en yaygın hastalıklardan olan Kronik Obstrüktif Akciğer Hastalığı (Koah) ve Uyku Apnesi hastalıklarına tanı koymada kullanılabilecek non-invaziv bir tanı yöntemi geliştirilmiştir. Bu amaçla 3 eksenli MEMS (Microelectro mechanical systems) tabanlı bir ivmeölçer kullanılarak soluma sırasında diyafram üzerindeki fiziksel hareketler sonucunda meydana gelen ivme değerleri ölçülmüştür.Çalışmada ilk olarak uyku apnesi atağı başladığı anda hastayı uyaran bir yöntem sunulmuştur. Bu amaçla apne anında hastayı uyarmada kullanılmak üzere, ana cihaz ile kablosuz haberleşebilen Apne Bileklik Sistemi (ABS) geliştirilmiştir. Hasta apneye girdiği anda, ABS üzerindeki titreşim motoru harekete geçirilerek hasta uyarılmakta ve hastanın tekrar nefes alması sağlanmaktadır. Bu işlem Arduino mikrodenetleyici kontrollü kapalı çevrim kontrol sistemi ile sağlanmıştır. Uykusu ağır olan hastalar için, hasta apneden çıkamadığında (ABS'nin yetersiz kaldığı durumlar için) hasta yakınlarına haber veren bir sistem de çalışma içerisinde mevcuttur. Bu özellik sayesinde hasta yakınlarına, hastaya anında müdahale etme fırsatı sunulmaktadır. Bu durum özellikle yaşlı, engelli ve çocuk grubundaki uyku apnesi hastaları için hayati öneme sahiptir.Çalışmada ikinci olarak; Koah ön tanısı için önemli parametrelerden olan spirometrik veriler (FVC, VC ve RR), medikal spirometre cihazı ile eş zamanlı olarak ölçülmüştür. Bu işlem için ölçüm verisi üzerinde sayısal işaret işleme yapılmış ve SD karta kaydedilen veriler filtrelenerek analizleri gerçekleştirilmiştir. Koah' lı hastaların geliştirilen sistemle ölçülen FVC eğrilerine bakılarak hastalık seviyesi başarılı bir şekilde belirlenebilmektedir. Böylelikle Koah hastalığı öncesi ön tanı koyma işlemi daha rahat ve zahmetsiz bir şekilde gerçekleştirilmiş olacaktır. Bu sonuçlara istinaden, geliştirilen taşınabilir, ekonomik ve ergonomik tasarıma sahip biyomedikal cihazın, mevcut medikal spirometre cihazına bir alternatif olacağı düşünülmektedir. In this study, a non-invasive diagnosis that can be used to diagnose Chronic Obstructive Pulmonary Disease (COPD) and Sleep Apnea, which are not related to each other but can be seen together, which may pose a risk for cardiovascular diseases, obstruct airways and are the most common diseases related to breathing method has been developed. For this purpose, acceleration values were measured by using 3-axis MEMS (Microelectro mechanical systems) based accelerometer as a result of physical movements on the diaphragm during inhalation. Firstly in the study, a method of stimulating the patient was presented when the sleep apnea episode started. For this purpose, the Apnea Bracelet System (ABS), which can communicate with the main device wirelessly, has been developed to be used for the patient's apnea during apnea. When the apnea starts, the vibration motor on the ABS is activated and the patient is alerted and the patient is allowed to breathe again. This process is conducted by the Arduino microcontroller controlled closed loop control system. There is an extra system which is informing the relatives of patients incase the patients who are in deep sleep wern't aweken (incase the ABS is inadequate). In this way, patients' relatives are offered the opportunity to intervene immediately. This is especially vital for the older people, disabled and children patients with sleep apnea. Secondly in the study; Spirometric data (FVC, VC, and RR), which are important parameters for pre-diagnosis of COPD, were measured simultaneously with the medical spirometer. For this process, digital signal processing was performed on the measurement data and the data recorded on the SD card was filtered and analyzed. The disease level can be determined successfully by looking at the FVC curves measured by the developed system of the patients who are affected COPD disease Thus, pre-diagnosis of disease COPD will be performed more easily and effortlessly. In view of these results, a biomedical device developed with a portable, economic and ergonomic design is thought to be an alternative to the existing medical spirometer device.
