Gülhane Askeri Tıp Akademisi eğitim hastanesinde ilaç uygulamalarına ilişkin geliştirilen prosedürün ilaç uygulama hataları üzerine etkisi

Abstract

ÖZET Araştırma, ilaç uygulama süreci içinde var olan sistemdeki ilaç hatalarını tesbit etmek, ilaç uygulamalarına ilişkin geliştirilen prosedürü uygulamak ve uygulanan bu prosedürün potansiyel ilaç hatalarına etkisini saptamak amacıyla tanımlayıcı ve karşılaştırmalı olarak planlanmıştır. Araştırma, GATA Eğitim Hastanesi Fizik Tedavi ve Rehabilitasyon kliniğinde yapılmıştır. FTR kliniğinde bir yıl içinde ilaç saatlerinde gerçekleşen ilaç uygulamaları araştırmanın evrenini, 1998 Ocak ve Mart aylarında gerçekleşen toplam 80 ilaç verme saatindeki ilaç uygulamaları ise örneklemini oluşturmuştur. Geliştirilen prosedür uygulanmadan önce gözlenen ilaç saatlerindeki uygulamalar (Ocak ayı içinde 40 ilaç verme saatinde gözlenen uygulamalar) kontrol grubunu, prosedür uygulandıktan sonra gözlenen ilaç saatlerindeki uygulamalar (Şubat ayı içinde 40 ilaç verme saatinde gözlenen uygulamaları) ise deney grubunu oluşturmuştur. Araştırmada veriler geliştirilen prosedür uygulanmadan önce ve uygulandıktan sonra `İlaç Hataları Tesbit Formu` ile gözlem yapılarak toplanmıştır. Elde edilen verilerin değerlendirilmesinde yüzdelik sayılar, Ki-kare ve Fisher's Kesin Ki-kare testi ile iki ortalama arasındaki farkın önemlilik testi kullanılmıştır. Araştırmada elde edilen başlıca sonuçlar şunlardır: ® İstemin alınması, ilaçların hazırlanması ve uygulanması safhasında uygulama öncesi ve uygulama sonrası harcanan süre ortalamaları açısından istatistiksel olarak anlamlı bir fark bulunmuş, hataların büyük oranda düştüğü saptanmıştır. 9 Hata tipleri, hataya neden olan faktörler, hatanın oluştuğu safhalar ve ihtiyaç duyulan işlemler açısından uygulama öncesi ve sonrasında istatistiksel olarak anlamlı bir fark bulunmuş, hataların büyük oranda azaldığı saptanmıştır.. ilaç uygulama sürecine olumlu yönde katkı sağladığı saptanmış olan ve bu çalışma kapsamında geliştirilen prosedürün, değişen şartlara göre yeniden düzenlenerek devam ettirilmesinin faydalı olacağı belirlenmiştir.

SUMMARY The research has been planned as descriptive and comparative one, to determine the errors in the process of drug administration system; to practise the procedures that developed by the researcher related to drug administration and to determine. Effect of this procedure on patential drug mistakes. The research has performed in Physical Therapy and Rehabilitation Department of GCilhane Military Medical Academy (GMMA). All Drug Administration practices in a year, were the universe of the research. And the drug administration practices between January and March 1998 were the sample. Totally 80 drug administration practices were observed. Before the procedure has been performed, 40 drug administrating practices were observed in January and these practices were the comparison group. After the procedure has been performed. 40 drug administrating practices were observed again in February and these were the experiment group. While observing.the data of research was collected by using the `Drug Administration Error Determination Form` before using procedure and after. The data were analysed by, frequencies, Chi-Square, Fisher's Certainity of Chi-Square Tests and t-test. Conclusions are as follows; s Means of time that spent for getting order, preparing the drugs to administration and administration. Before and after the procedure are significantly different as statistically. After the procedure, the errors has decreased. ® Types of errors, factors that causes the errors, the steps that the errors occurred in and procedures are needed were significantly different before and after the procedure. It is determined that. The procedure was developed in the content of this research, has effected the drug administration process positively. It was thought that it will be usefull to perform this procedure in different condition and the procedure must be reviewed for new conditions. 70
