Kronik tonsillitli hastalarda tonsillektominin serum immünoglobülin düzeylerine etkisi

Abstract

VI. ÖZET Çalışmamızda tonsillektominin hümöral immün sistem üzerine etkisini değerlendirmeyi amaçladık.Bu amaçla kronik tonsilMi 20 hastada tonsillektomiden önce ve postoperatif 1. ayda ölçülen IgG, IgA, IgM, C3, C4 düzeylerini hem kendi aralarında hem de kontrol grubu ile kıyasladık. Kronik tonsülitli hastalarda tüm Ig değerlerinin kontrol grubuna göre yüksek olduğunu saptadık Tonsillektomi sonrası IgG ve C3'de daha belirgin olmak üzere tüm değerlerde düşme saptadık. TonsiUektominin Hb, Hct, BK, KK seviyeleri üzerine herhangi bir etkisi olmadığını tesbit ettik. Histopatolojik incelemeler sonucu, operasyonla alınan tonsillerin hepsinde kronik değişiklikler meydana geldiğini tesbit ettik. Sonuç olarak, tonsillektomi ile, kronik fokal enfeksiyon odağı halindeki tonsiller vücuttan uzaklaştırılmış olacağından kronik tonsillit tanısı konduğunda tonsillektomi endikasyonu vardır. 28

VII. SUMMARY THE EFFECT OF TONSILLECTOMY ON SERUM IMMUNOGLOBULIN LEVELS IN PATIENTS WITH CHRONIC TONSILLITIS In our study, we aimed to assess the effect of tonsillectomy on the humoral immune system. For this purpose, we compared the levels of IgG, IgA, IgM, C3, C4 of 20 patients who had chronic tonsillitis which were determined before and one month after tonsillectomy, with both the control group and each other. We determined that all Ig levels in patients with chronic tonsillitis were higher than control group's levels. We detected a decrease in all levels (especially IgG and C3) after tonsillectomy. We observed there were not any effect of tonsillectomy on Hb, Hct, RBC, WBC levels. We showed chronic histopathological changes in all tonsil specimens. As a result, we believe that if the diagnosis is chronic tonsillitis, tonsillectomy should be performed to eradicate the tonsil which behaves as a chronic focal infection source. 29
