Papiller tiroid kanserinde, primer tedavi ile remisyon sağlanan hastalarda, uzun dönem takipte, nüksü belirleyici faktörlerin retrospektif değerlendirilmesi

Abstract
Tiroid kanserlerinin yaklaşık % 85-90'ını oluşturan PTK genellikle iyi prognozludur. Fakat uzun süreli takipte nüks oranları % 35'e varabilmektedir. Hastalar, tanı sırasında nüks olasılığı açısından bir risk sınıflamasına dahil olmakta ve bu sınıflamaya göre primer tedavi ve takip şekli belirlenmektedir. Ancak bu sınıflamalara göre düşük riskli grupta olup takipte nüks gelişen veya yüksek riskli grupta olup uzun yıllar remisyonda izlenen hastalar vardır. Çalışmamızın amacı tiroid kanseri polikliniklerimizde papiller tiroid kanseri tanısı ile izlenmekte olup primer tedavi sonrası remisyon sağlanmış hasta grubunda nüks gelişimini, nüks şeklini ve nüksü kestirmeye yarayacak parametreleri belirlemektir.Tiroid kanseri polikliniklerimizce takip edilen toplam 1882 hastanın dosyaları retrospektif olarak incelenmiş ve primer tedavi sonrası yapılan ilk I-131 TVT ve uyarılmış Tg değerlendirmesine göre remisyonda (antiTg negatif olan hastalarda; TVT de tutulum yok ve TSH > 30 ? U/ml iken Tg değeri < 2 ng/ml olması, antiTg pozitif olan hastalarda; I-131 TVT de tutulum olmamasının yanında, boyun ultrasonografisinin normal bulunması, başlangıç tedavisi sırasında RAI verilmeyen hastalarda ise; TSH baskılı ve antiTg negatif iken, Tg düzeyinin < 1 ? U/ml bulunması ve boyun USG'nin normal olması remisyon olarak kabul edilmiştir.) ve dosya kayıtlarının yeterli olduğu belirlenen 492 hastanın verileri (yaş, cins, tümör özellikleri, uygulanan tedavi, takip süresi, primer tedavi-ilk değerlendirme sırasındaki Tg, TSH, AntiTg düzeyleri, nüks gelişimi, nüks şekli,..) kaydedilmiştir.Takip sırasında, 492 hastanın sadece 12'sinde (%2.4) nüks saptanmıştır. Bunların 7'si servikal LN metastazı, 5'i lokal nüks şeklindedir. Nüks tespit edilen ve remisyonda kalan grupların değerlendirilmesinde, ilk I-131 TVT sırasındaki uyarılmış Tg düzeyi (ablasyon sonrası Tg), tanı-primer tedavi sonrası ilk TVT sırasındaki antiTg pozitiflik oranı, tümör boyutu ve takip süresi nüks grubunda anlamlı olarak fazladır (p, sırası ile 0.013, 0.029, 0.01, 0.024 ve 0.002). Ablasyon sonrası uyarılmış Tg düzeyinin > 0.25 ng/ml bulunması da nüks ile ilişkilidir (p=0.009). ?Cox? regresyon analizi ve ?Kaplan-Meier? nükssüz sağkalım değerlendirmelerinde başlangıç LN metastazı varlığı nüks için anlamlı bir risk faktörü olarak ortaya çıkmıştır (p, sırası ile 0.011, 0.01).Sonuç olarak, PTK' nın uzun süreli takibinde nüksü öngörmede yararlı olabilecek parametreler; başlangıç LN metastazı varlığı, ablasyon sonrası Tg düzeyi, devam eden antiTg pozitifliği ve takip süresidir. The % 85-90 of thyroid cancers are papillary thyroid cancers and they usually have good prognosis. But after long follow-up the recurrence rate may be as high as %35. The primary treatment and the follow-up plan of the patients are chosen according to the risk stratification for recurrence at the time of diagnosis, Despite this risk stratification, some patients with low risk recure whereas some patients with high risk are disease free for life long. The aim of our study is to detect the recurrence rate, recurrence type and the parameters which may help to predict recurrence in papillary thyroid cancer patients who were in remission after the primary treatment and followed in our center.The files of 1882 papillary thyroid cancer patients are examined retrospectively. The data of 492 patients (age, gender, tumor size, tumor histology, invasion sites, treatment, duration of follow-up, TSH, Tg, antiTg levels at the time of primary treatment and at the time of first whole body scanning, presence of recurrence, type and time of recurrence,?.) who were in remission at the time of first whole body scan (WBS) and whose documentation were adequate are recorded. Remission criteria in antiTg negative patients were; no uptake in whole body scan and stimulated Tg level < 2ng/ml when TSH >30 ? U/ml. In the case of antiTg positivity remission was accepted as; no uptake in whole body scan and normal neck ultrasonography. For patients who were treated only by surgery without RAİ, remission was accepted as normal neck ultrasonography and undetectable Tg level under supressed TSH and negative antiTg.At the end of statistical analysis, recurrence was detected only in 12 of the 492 patients (%2.4). Seven of these were as servical lymph node metastasis and 5 of these were as local recurrence. By the comparison of patients with and without recurrence; tumor size, stimulated Tg level and antiTg positivity ratio at the time of first whole body scan and duration of follow up were significantly higher in the group with recurrence (p values were 0.024, 0.013, 0.01, 0.002 respectively). The postablative Tg level as ? 0.25 ng/ml was found to be significantly related to recurrence (p=0.009). Also by Cox regression and Kaplan-Meier survival analyses initial lymph node metastasis was the only related risk factor for recurrence (p values were 0.011, 0.01 respectively).In conclusion; the parameters which may help to predict recurrence during the long follow-up time of papillary throid cancer are; presence of initial lymph node metastasis, postablative Tg level, persistance of antiTg positivity at the time of first WBS and duration of follow-up.
