Üriner inkontinansın röprodüktif ve postmenopozal dönemde kadın cinsel fonksiyon bozuklukları üzerine etkisi

Abstract
Amaç: Homojen biçimde oluşturulan hasta gruplarında; geçerlilik ve güvenilirliği oluşturulmuş testler ile ?Kadın Cinsel Fonksiyon Bozukluğu? (KCFB) ve ?Üriner İnkontinans? (Üİ) arasındaki ilişkiyi incelemeyi amaçladık.Materyal ve Metot: Çalışmaya Ocak 2010-Mart 2010 tarihleri arasında, İstanbul Tıp Fakültesi Kadın Hastalıkları ve Doğum Ana Bilim Dalı polikliniklerine başvuran 400 kadın olgu dâhil edildi. Hastalar röproduktif ve postmenopozal dönemde olmak üzere 200'er kişilik iki temel gruba ayrıldı. Her grup içinde de 100'er kişilik üriner inkontinansı olan çalışma ve üriner inkontinansı olmayan kontrol grupları oluşturuldu. Demografik özellikler için ?Reprodüktif ve Postmenopozal Dönemdeki Kadınlarda Cinsel Fonksiyon ve Seksüel Aktivite Değerlendirme Anket Formu?, KCFB ölçmek için ?FSFI? testi kullanıldı, Üİ ile KCFB ve yaşam kalitesi ilişkisini incelemek amacıyla ?King Sağlık Anketi?, yaşam kalitesini değerlendirmek amacıyla ?Beck Depresyon Ölçeği? kullanıldı. Sonuçlar %95 güvenlik aralığında, anlamlılık p<0.05 düzeyinde değerlendirildi.Bulgular: KCFB prevalansı tüm olgularda %48,25 (n=193), röproduktif dönem üriner inkontinans grubunda %54 (n=54), postmenopozal üriner inkontinans grubunda %80 (n=80), röproduktif dönem kontrol grubunda %14 (n=14) ve postmenopozal kontrol grubunda %45 (n=45) olarak saptanmıştır. FSFI skorları; üriner inkontinans gruplarında kontrol gruplarına göre ve postmenopozal olgularda röproduktif dönemdeki olgulara göre düşüktür (p<0.05). Postmenopozal üriner inkontinans grubunun King Sağlık Anketi puanları tüm kategorilerde daha yüksek saptanmıştır. Beck skorları, postmenopozal gruplarda ve inkontinans gruplarında daha yüksektir (p<0.05). İleri yaş, menopoz durumu, düşük eğitim düzeyi, görücü usulüyle evlenme, eşle cinsel konularda konuşamama ve artan parite sayısı ile düşük FSFI skorları arasında anlamlı ilişki vardır (p<0.05).Sonuç: Üriner inkontinans varlığı, kadın cinsel fonksiyonlarını ve yaşam kalitesini olumsuz yönde etkilemektedir. Bu etki postmenopozal dönemde daha fazladır. Yaş, menopoz, düşük eğitim düzeyi; KCFB için risk oluşturmaktadır. Görücü usulüyle evlenme, eşle cinsel konularda konuşamama ve artan parite sayısı kadın cinsel fonksiyonlarını olumsuz yönde etkilemektedir. Purpose: The aim of this study, with using objective tests, is to evaluate the relationship between ?Female Sexual Dysfunction? (FSD) and ?Urinary Incontinence? (UI) in homogenously made groups.Material and Methods: We included 400 women who participated Istanbul Medicine Faculty Outpatient Clinics of Gynecology between January 2010- March 2010. Patients were divided two main groups of 200, as women in reproductive age and women in postmenopausal age. Each group was divided two groups of 100, the one of study group in which women has urinary incontinence, and the one of control group in which women was continent. We used ?Assessment Questionnaire for Sexual Function and Activity in Reproductive or Postmenopausal Women? for demographic analysis, validated ?Female Sexual Function Index? (FSFI) for FSD, ?King?s Health Questionnaire? for assessing the relationship between quality of life and UI with FSD, and ?Beck Depression Inventory? for assessing quality of life. Results were evaluated in %95 confidence interval, p value less than 0.05 was considered statistically significant.Results: Prevalence of FSD was found %48, 25 (n=193) in all patients, %54 (n=54) in study group of reproductive age, %80 (n=80) in study group of postmenopausal age, %14 (n=14) in control group of reproductive age, %45 (n=45) in control group of post-menopausal age. FSFI?s scores were lower in two study group when compared with control groups and also in post-menopause group when compared with reproductive group (p<0.05). King?s Health Questionnaire score were higher in control group of postmenopausal age when compared with all other groups. Beck score?s were higher in postmenopausal group and two of study groups (p<0.05). There was a statistically significant correlation between lower FSFI scores with advanced age, menopause, increased parity, lower educational level, arranged marriage and not able to talk abot sexual issues with her husband (p<0.05).Conclusion: UI damages female sexual function and worsens quality of life. These effects tend to be increased in postmenopausal age. Age, menopause, lower educational level, are risk factors for FSD and lower quality of life. Arranged marriage, not able to talk abot sexual issues with her husband and increased parity; badly affects female sexual dysfunctions and quality of life.
