Merkezi sinir sistemi cerrahisi esnasında intraoperatif sinir sistemi görüntüleme modalitelerinin etkin kullanımı

Abstract
Amaç: Çalışmamızın amacı fakültemiz Beyin ve Sinir Cerrahisi Anabilim Dalı'nda sinir sisteminin risk altında olduğu, farklı cerrahi operasyonlarda bilinen monitorizasyon yöntemlerini ameliyatın gerektirdiği şekilde multimodal olarak kullanmaktır. Bu yöntemlerin kullanımının devamlılığını sağlamak için cerrahi ekip ve anestezi ekibi ile birlikte gerekli teknik beceri ve klinik tecrübeyi edinmektir.Hastalar ve Yöntemler: Beyin ve Sinir Cerrahisi ameliyathanesinde operasyona alınan 15 hastada multimodal monitorizasyon gerçekleştirdik. Üç serebellopontin köşede kitle, dört omurilikte kitle, iki servikal spondiloz, iki arteriyovenöz malformasyon, iki gergin kord, bir Chiari malformasyonu, bir syringomyeli olmak üzere toplam yedi farklı operasyonda duysal uyarılmış potansiyeller (SEP), transkraniyal elektriksel uyarım-motor uyarılmış potansiyeller (TES-MEP), beyin sapı işitsel uyarılmış potansiyelleri (BAEP) ve kafa çiftlerinin sürekli ve uyarılmış elektromiyografi (EMG) ile monitorizasyonu yöntemlerinden gerek görülenler kullanılarak monitorizasyon gerçekleştirilmiştir.Bulgular: Bir hastada beyin sapına yakın çalışılan köşe tümörü operasyonundan sonra yoğun bakımda 3 gün içinde spontan düzelen geçici hemiparezi ve lateral bakış kusuru gelişti. Aynı hastada yutma güçlüğü gelişti ve nazogastrik sonda ile beslenmeye başlandı. Bir hastada operasyon esnasında MEP yanıtları kaybolmakla birlikte ameliyat sonrası nörolojik bir sekel gelişmedi. Diğer 13 hastada ameliyat esnasında monitorizasyonda kritik bir değişiklik gözlenmedi ve operasyon sonrası yeni nörolojik sekel gelişmedi.Sonuç: Beyin ve Sinir Cerrahisi birimimizde birçok yeni monitorizasyon tekniğini cerrah ve anestezi ekibi ile koordinasyonu sağlayarak başarıyla gerçekleştirdik. Özellikle yüksek riskli operasyonlarda birden fazla monitorizasyon modalitesini bir arada kullanma tecrübesini edindik. Operasyon esnasında ve sonrasında elde ettiğimiz sonuçlar monitorizasyonun, cerraha operasyonu nöral hasar oluşturmadan tamamlaması için avantajlar sağladığını desteklemektedir. Objective: This study aims to use appropriate neuromonitoring modalities in operations with high neural injury risks, in the Neurosurgery Department of Istanbul Faculty of Medicine; and also purposes to enhance our clinical experience and technical skills on neuromonitoring together with surgery and anesthesia crews.Patients and Methods: We performed multimodal neuromonitoring in 15 patients in operation room of Neurosurgery Department. The operations were on seven different pathologies including three cerebellopontin angle tumor, four spinal cord tumor, two cervical spondylosis, two arteriovenous malformation, two tethered cord, one Chiari malformation and one syringomyelia operation. Somatosensory evoked potentials (SEP), transcranial electrical stimulation-motor evoked potentials (TES-MEP), brainstem auditory evoked potentials (BAEP) and continuous-triggered electromyography (EMG) monitoring of cranial nerves were used to monitoring.Results: In one cerebellopontin angle tumor patient, transient hemiparesis and lateral gaze defect were found postoperatively but recovered in three days in intensive care unit. Dysphagia was developed in the same patient and he was fed through a nasogastric catheter. In one patient MEP responses disappeared during surgery and did not recover later. That patient did not develop any postoperative neurological deficit. We did not face any critical changes in monitoring and any new neurological deficit in other 13 patients.Conclusions: Different monitoring techniques were performed succesfully in coordination with surgery and anesthesia crews. We acquired the experience of multimodal neuromonitoring techniques especially in high risk neurosurgeries. Our data obtained during surgery and postoperative outcomes suggest that intraoperative monitoring provides benefits to the surgeon for minimal neural injury.
