Koroner baypas cerrahisinde sıkı ve liberal glukoz kontrolünün postoperatif kognitif fonksiyon üzerine etkisinin karşılaştırılması

Abstract
Giriş: Açık kalp cerrahisi sonrası postoperatif kognitif işlev bozukluğu (POKİB) sık görülen ve risk faktörleri büyük oranda belirlenmiş bir sorundur. Hipergliseminin fokal nörolojik hasarı arttırıcı etkisi bilinmektedir. Koroner cerrahisinde gliseminin sıkı kontrolü ile kardiyak prognozun olumlu etkilendiği bildirilmiştir; ancak nörolojik etkileri konusunda araştırmasınırlıdır. Bu çalışmanın amacı, koroner cerrahisinde sıkı glukoz kontrolü ile liberal glukoz kontrolünü,POKİB açısından karşılaştırmaktır.Gereç ve Yöntem: Etik kurul onayı alınmış olan çalışmaya elektif koroner baypas yapılacak 40 hasta katılmıştır. Birinci grupta (GI n=20) sıkı glukoz kontrolü (<120mg/dl), ikinci grupta (GII n=20) ise liberal glukoz kontrolü(<180mg/dl) perioperatif ve postoperatif 24 saat boyunca uygulandı. Anestezi ve baypas yönetimi ile cerrahi protokolü tüm hastalarda aynı şekilde yapıldı.Nörokognitif değerlendirme operasyondan en az iki gün önce (T0), operasyondan bir hafta (T1) ve 3 ay (T2) sonra yapıldı. Bu amaçla psikometrik test bataryası kullanıldı.İstatistiksel analiz,gruplar arası karşılaştırmada Mann Whitney u,grup içi tekrarlayan ölçümlerde ise ANOVA ile yapıldı.Bulgular: İki grupta demografik ve operatif (baypas ve krosklemp süreleri, en düşük ısı, anastomoz sayısı, vb) veriler benzerdi. Preoperatif kognitif değerlendirmede gruplar arası fark yoktu. Hiçbir hastada majör nörolojik komplikasyon (inme, görme ve yürüme bozukluğu, bilinç bulanıklığı, vb) görülmedi. İstatiksel anlamlılık gösterilememekle birlikte tüm hastalarda,bütün testler için T1 değerleri T0'dan daha düşüktü.Gruplar arası karşılaştırmada, GI'deki hastalar minimental test,kelime listesi oluşturma,sayı menzili,erken anlık bellek ve görsel işlevler testlerinde üçüncü ay takiplerinde anlamlı olarak daha iyi performans gösterdiler(p<0,05).Sonuç-Tartışma: Kardiyak cerrahi sonrası erken dönemde bilişsel işlevlerde bozulma beklenen bir süreçtir. Glisemi değeri ile POKİB ilişkisini değerlendirdiğimiz çalışmaya göre, geç postoperatif dönemde sıkı glukoz kontrolü kognitif işlevi daha iyi korur gözükmektedir. Introduction: Postoperative cognitive dysfunction is a well known phenomenon after cardiac surgery.The effects of glycemia on neurological outcome is quite limited except focal neurological injury for this surgical population. Tight glycemic control have been shown to improve survival and to reduce morbidity in coronary surgery. The aim of this study is to compare two different glycemia protocols,tight and liberal,on postoperative cognitive function.Methods: After approval of Ethic Committee,forty patients for elective coronary surgery were included in the study. During intraoperative period and first 24hours of the surgery, the first group (GI)(n=20) was under tight glucose control (<120mg/dl) and the second one (GII) (n=20) was under liberal glycemia control (<180mg/dl). Induction and maintenance of anaesthesia was identical for two groups. Neurocognitive function which was evaluated via a psychometric test battery, was assessed at least two days before surgery (T0), as well as at first postoperative week (T1) and at third month (T2). Data were compared with Mann Whitney u test and repeated measures of ANOVA.Results: Demographic and operative data (bypass or cross-clamp time, lowest temperature, number of coronary anastomosis, etc) were comparable between groups. No major adverse neurological outcome (stroke, change in mental status, visual loss etc) has occurred. All subjects had a trend of lower neurocognitive scores at T1 than at T0; without statistical significance. Patients of GI had significantly better scores than GII for mini mental state,word list generation,digit span,Weschler memory scale and visiospatial skills(p<0.05).Conclusion: Postoperative cognitive dysfunction is commonly seen after cardiac surgery. According to the results of this study, tight glucose control seems to be associated with preserved cognitive function after coronary surgery.
