Abstract
Deri flepleri, çeşitli defektlerin onarımında sıklıkla kullanılmakla beraber birçok plastik cerrahi prosedüründe de önemli yere sahiptir.Sigara bağımlılığı önlenebilir flep nekrozu gelişiminin en önemli eksojen nedenidir. Nikotin, sigaranın major toksik bileşenidir ve sempatik sinir sistemi ile nitrik oksit üzerindeki etkileriyle doku perfüzyonunu bozmaktadır. Nikotin, reaktif radikallerinin oluşumunu sağlayarak da hücrede lipit, protein ve deoksiribo-nükleik-asit (DNA) üzerinde oluşturduğu hasarlarla programlı hücre ölümüne ve kanserlere neden olmaktadır.Koenzim Q10 (CoQ10), her hücrede sentezlenebilen antioksidan, antienflamatuvar bir bileşiktir. Reaktif oksijen radikalleri için çöpçü görevi yapan CoQ10un endotel disfonksiyonu üzerinde olumlu etkileri mevcuttur. Çalışmamızda CoQ10, nikotinin oluşturduğu doku dolaşımını bozup, hücre yıkımını arttıran mekanizmaları bloke edebilmek amacıyla kullanılmıştır.Çalışmamızda 32 adet erişkin, Sprague-Dawley cinsi sıçanı 8er adet denek içeren dört gruba ayırdık. İki mg/kg dozda ameliyat öncesi 28 gün ve ameliyattan sonra 7 gün boyunca günde iki kere nikotin injeksiyonu yapılan sıçanların yarısına pre-op ve post op yedişer gün, 10 mg/kg dozda, parenteral yolla CoQ10 tedavisi uyguladık. Diğer bir gruptaki sıçanlara ise aynı süre ve doz ile sadece CoQ10 uyguladık. Bir grup da kontrol grubu olarak belirlendi. Pre-op yapılan tedavileri takiben tüm sıçanlarda McFarlane flebi kaldırıldı ve post-op 7. günün sonunda tüm denekler sakrifiye edilerek alınan doku örnekleri ve serumlar flep nekrozu, histopatoloji ve oksidatif stres parametreleri açısından değerlendirildi. Çalışmanın sonunda nikotin verilen ve verilmeyen gruplarda CoQ10un flep yaşabilirliğini arttırdığı, inflamasyonu ve oksidatif strese bağlı olarak oluşan hasarı azalttığını saptadıkElde ettiğimiz veriler bizlere CoQ10un, nikotinin oluşturduğu oksidan hasarı ve flep nekrozlarını azaltmada yararlı olabileceğini düşündürtmektedir. The flaps are utilized frequently in reparation of defects, and they have a significant role in many plastic surgery procedures.Smoking addiction is the most prominent exogenous etiologic factor of avoidable flap necrosis. Nicotine is the major toxic component of cigarette. Nicotine causes ischemia by disturbing the tissue perfusion and possibly causes programmed cellular death and cancer by playing role in the formation of reactive radicals.Coenzyme Q10 (CoQ10), is an endogenous anti-oxidant and an anti-inflammatory compound which acts as a scavenger for reactive radicals and prevents the endothelial dysfunction.In our study, we classified 32 adult Sprague-Dawley type rats in four separate groups. We applied parenteral CoQ10 therapy for preoperative and postoperative 7 days in 10 mg/kg doses to half of the rats that received 2 mg/kg of nicotine injection twice a day, during 28 days preoperatively and during 7 days postoperatively. One of the groups of the remaining rats received CoQ10 for the same time and dose, and the other group was nominated as the control group. Following the preoperative treatments, McFarlane flaps were elevated, and at the seventh postoperative day, all of the subjects were sacrificed. In fact, the tissue samples and the sera obtained from these subjects were evaluated for the parameters of flap necrosis, histopathology and oxidative stress.Under the light of the data obtained, we think that CoQ10 can be beneficial in decreasing the oxidative stress and the flap necrosis caused by nicotine.
