Oral skuamöz hücreli karsinom hastalarında COX-2 polimorfizmlerinin incelenmesi

Abstract
AMAÇ: Çalışmamızda Oral kavitenin skuamoz hücreli kanserlerinde COX-2 polimorfizmini DNA izolasyon yöntemleriyle araştırarak, sonuçların klinikopatolojik ve karsinogenezdeki etkisini araştırmayı amaçladık.HASTALAR ve YÖNTEMLER: Ağustos 2007- Ağustos 2012 arasında İstanbul Tıp Fakültesi KBB ve Diş hekimliği kliniğinde; Oral SCC tanısı alan 114 hasta ve 143 normal kişiden DNA izolasyonu için kan örnekleri steril EDTA?lı tüplere alınmış ve en geç bir gün içinde çalışmak üzere oda ısısında saklanmıştır. Genomik DNA örneklerinde COX-2 765G>C ve COX-2 1195G>C lokusuna ait aleller polimeraz zincir reaksiyonu ,ile çoğaltıldı.Bu çalışmanın istatiksel analizinde SPSS 11.0 paket programı kullanılarak yapılmıştır.Genotip ve allelerin görülme sıklığının gruplar arası farklılıklarının değerlendirilmesinde Ki kare ve Fisher testi kullanılmıştır. Gruplar arası demografik verilerin karşılaştırılmasında Student?s t-testi ve Annova kullanılmıştır. Allel frekansları gen sayma metoduna göre yapılmıştır.BULGULAR: Kontrol grubunda COX-2-765 C+ (CC+CG) genotipin hasta grubuna göre yüksek olduğu ve 2.7 katlık bir koruyuculuk sağladığı gözlenmiştir.Aynı şekilde COX-2-765 CC genotipli bireylerin de kontrol grubunda hasta grubuna göre yüksek olduğu tespit edilmiştir. COX-2-1195G+ bireylerin kontrol grubunda 1.9 kat yüksek olduğu, COX-2-1195AG genotip frekansının kontrol grubunda yüksek olduğu ve 2.2 katlık hastalık için koruyuculuk sağladığı gözlenmiştir . Yine bu bölgede hasta grubunda COX-2-765 G+ (GG+CG) genotipli bireylerin kontrol grubuna göre 1.7 katlık ,COX-2-765 GG taşıma hasta grubunda 2.7 katlık ,COX-2-1195AA+ genotipin hasta grubunda 1.9 katlık hastalık gelişiminde risk oluşturduğu gözlenmiştir.SONUÇ: Kontrol ve hasta grupları arasında Oral skuamoz hücreli kanser gelişimi açısından COX-2 polimorfizmi, cinsiyet, sigara ve alkol kullanımı gruplar arasında anlamlı olarak farklı tespit edilmiştir. PURPOSE: In our study, we aimed at investigating the COX-2 polymorphism in the squamous cell cancers of the oral cavity by means of the DNA isolation method and research into the effects of result on the clinical pathology and carcinogenesis.PATIENTS and METHODS: The blood samples were collected in EDTA tubes from 114 patients diagnosed with oral SCC and 143 healthy subjects, who presented to the ENT and Dental Clinic at Cerrahpaşa School of Medicine between the months of August `07- August `12, and were stored at room temperature to be studies at least one day later. In the genomic DNA samples, the alleles pertaining to the COX-2 765G>C and COX-2 1195G>C locus were proliferated using the SPSS 11.0 package software. To assess the differences between groups in terms of the prevalence of genotypes and alleles, the Chi-square test and Fisher test were used. The Student's t-test and ANOVA testing were used in order to compare the demographic data between groups. The allele frequencies were checked according to the gene counting method.FINDINGS: It was observed that the COX-2-765 C+ (CC+CG) genotype was high as compared to the patient group and provided a 2.7-fold protection. Similarly, it was identified that the number of individuals with the genotype COX-2-765 CC was higher in the control group as compared to the patient group. It was observed that the number of COX-2-1195G+ individuals was 1.9 times higher in the control group, COX-2-1195AG genotype frequency was high in the control group and it provided a 2.2-fold protection against the disease. Similarly, it was observed that the individuals in the patient group with the genotype COX-2-765 G+ (GG+CG) raised the risk of disease development 1.7 times, the individuals in the patient group with the genotype COX-2-765 GG raised it 2.7 times and the individuals in the patient group with the genotype COX-2-1195AA+ raised it 1.9 times.CONCLUSION: The COX-2 polymorphism, sex, smoking and alcohol intake were identified to be significantly different between the control group and the patient group with respect to the development of oral squamous cell cancer.
