Abstract
Önalan MA. Karotis endarterektomi operasyonu geçiren hastalardaki alt ekstremite periferik arter hastalığı sıklığı. Tıpta Uzmanlık Tezi. İstanbul, 2016.Amaç: Arteriyosklerotik damar hastalığı sistemik bir hastalıktır. Bu açıdan bakıldığında; damar yatağında birden fazla bölgeyi hastalıklı hale getirme olasılığı her zaman için bulunabilmektedir. Risk faktörlerinin azaltılmadığı bir arteriyosklerozun varlığında organ tutulumlarına bağlı klinik tablolar ortaya çıkacaktır. Bizim çalışmamızdaki amacımız karotis endarterektomi operasyonu geçiren hastalardaki alt ekstremite periferik arter hastalığı sıklığını belirlemek ve risk faktörlerini ortaya koymaktır.Gereç ve Yöntem: İstanbul Tıp Fakültesi Hastanesi'nde Kalp Damar Cerrahisi Anabilim Dalı tarafından karotis endarterektomi operasyonu geçiren %72,3'ü (n=47) erkek, %27,7'si (n=18) kadın olmak üzere toplam 65 hasta bu çalışmaya alınmıştır. Hastaların yaşları 48 ile 88 arasında değişmekte olup ortalama 66,26±8,84 yıldır. Bu hastalardaki alt ekstremite periferik arter hastalığı sıklığı araştırılmıştır. Karotis endarterektomi operasyonu geçiren hastalar öykü, fizik muayene, ayak bileği kol indeksi ve alt ekstremite dupleks ultrasonografi sonuçları bir bütün halinde değelendirilerek hastalarda alt ekstremite periferik arter hastalığı olup olmadığı araştırıldı.Bulgular: Çalışmamız sonucunda karotis endarterektomi operasyonu geçiren hastalarda 65 yaş üstü olmanın, erkek cinsiyetin, sigara içmenin, aile öyküsünün, kronik böbrek yetmezliği varlığının alt ekstremite periferik arter hastalığını istatistiksel olarak anlamlı derecede arttırdığı tespit edildi. HDL düzeyleri düşük olgularda da alt ekstremite periferik arter hastalığı sıklığında artış gözlendi. Fakat hipertasiyon ve diabetes mellitus varlığı, LDL, total kolesterol, trigliserit, CRP, protein S, protein C seviyeleri, alkol kullanımı, preoperatif karotis arter darlık oranı, preoperatif nörolojik semptom varlığı, beden kitle indeksi, geçirilmiş kardiak cerrahi öyküsü, bilteral karotis endarterektomi operasyonu öyküsünde alt ekstremite periferik arter hastalığı sıklığında açısından anlamlı farklılık saptanmadı.Sonuç: Çalışmamızda karotis endarteretomi operasyonu geçiren 65 hastadan 30'unda (%46,2) periferik arter hastalığı tespit edilmiştir. İstanbul, 2016.Objective: Arteriosclerotic vessel disease is a systemic disorder. From this point of view, there is a chance of systemic process to disease multiple areas in the vascular bed. Clinical conditions associated with organ involvement can occur in the presence of arteriosclerosis where the risk factors have not been reduced. The aim of the present study was to determine the prevalence of peripheral arterial disease in the lower extremities in patients who underwent carotid endarterectomy and establish the relevant risk factors.Materials and Methods: A total of 65 patients (72.3% were males (n=47) and 27.7% were females (n=18)), who underwent carotid endarterectomy procedure at the Department of Cardiovascular Surgery in İstanbul University Faculty of Medicine Hospital, were included in the study. The mean age was 66.26±8.84 years, ranging from 48 to 88 years. The study investigated the prevalence of peripheral arterial disease in the lower extremity of these patients. The patients that underwent carotid endarterectomy were investigated as a whole for the presence of peripheral arterial disease in the lower extremities using medical history, physical examination, ankle-brachial index and the results of lower extremity duplex ultrasonography.Result: At the end of the study, age above 65 years, male gender, smoking, family history, presence of chronic renal insufficiency were found to be statistically significant in increasing the risk of peripheral arterial disease in the lower extremities of patients that underwent carotid endarterectomy procedure. There is also increased prevalence of peripheral arterial disease in the lower extremities of patients with decreased HDL levels. However, there was no significant difference in the prevalence of peripheral arterial disease in lower extremities of patients that underwent bilateral carotid endarterectomy in terms of the presence of hypertension and diabetes mellitus, LDL, total cholesterol, triglyceride, CRP, protein S, protein C levels, alcohol consumption, preoperative carotid stenosis rate, presence of preoperative neurological symptoms, body mass index, and past history of cardiac surgery.Conclusion: Lower extremities peripheral arterial disease was found in 30 out of 65 patients who underwent carotid endarterectomy (46.2%). Age (above 65 years), male gender, smoking, family history, and presence of chronic renal insufficiency were established as significant risk factors for the co-occurrence of peripheral arterial disease in patients that underwent carotid endarterectomy.Key words: Aterosclerosis, Carotid endarterctomy, Peripheral arterial disease
