Hemodiyaliz tedavisi alan hastaların bakımdan sorumlu yakınlarının ve hemşirelerinin eğitim gereksinimleri ile ilişkili görüşlerinin q metodu ile incelenmesi

Abstract

Bu çalışma hemodiyaliz tedavisi alan hastaların, bakımlarından sorumlu yakınlarının ve hemşirelerinin eğitim gereksinimleri ile ilişkili görüşlerinin Q Metodu yoluyla incelemek amacıyla tanımlayıcı olarak yapılmıştır. Araştırma, Konya merkezde bulunan Özel Ölmez Diyaliz Merkezi ve Özel Nefrotrans Karatay Diyaliz Merkezlerinde, Aralık 2017-Mart 2018 tarihleri arasında, hemodiyaliz tedavisi alan 30 hasta, hastaların bakımından sorumlu 30 hasta yakını ve hastalara bakım ve tedavi veren 30 hemşire ile yapılmıştır. Veri toplama araçları olarak hasta, hasta yakınları ve hemşireler için tanıtıcı özellikler soru formları, hemodiyaliz eğitim konularından oluşan 22 soru kartı ve sormak istedikleri soruyu yazmak istemeleri durumunda kullanılmak üzere 2 boş soru kartı kullanılmıştır. Veriler, Q metodundan (kartların önem sırasına göre sıralanması) yararlanılarak araştırmacı tarafından yüz yüze görüşme tekniği kullanılarak toplanmıştır. Toplanan veriler yüzdelik ve frekans yöntemleri ile değerlendirilmiştir. Araştırmada hemodiyaliz tedavisi alan hastaların eğitim gereksinimleri ile ilgili hasta, bakımlarından sorumlu hasta yakınları ve hemşirelerin konulara göre bazı düşüncelerinin aynı bazı düşüncelerinin ise farklı olduğu bulunmuştur. Hasta, hastanın bakımından sorumlu yakını ve hemşirelerin eğitim konularının önem sıralamasına baktığımızda; Hastaların, ihtiyaçları olduğunda psikolojik destek alma, sosyal yaşama uyum sorunlarıyla baş etme gibi psiko-sosyal sorunları içeren eğitim konularını, Hastaların bakımlarından sorumlu yakınlarının, fistül/kateter bakımı, komayı önlemeye yönelik tedbirleri almak gibi komplikasyonları önlemeye yönelik eğitim konularını, Hemşirelerin ise böbreklerin yapısı ve fonksiyonları, hemodiyalizin mekanizması gibi teknik ve teorik bilgi içeren eğitim konularına daha çok ilgi gösterdikleri sonucuna varılmıştır. Hemşirelerin Q metodunu öğrenerek, yapacakları eğitim öncesinde hastaların ve hastaların bakımından sorumlu bakım vericilerinin önem sıralamasını belirlemesi ve eğitimin hasta ve hasta bakım vericilerinin ihtiyaçları doğrultusunda planlanarak yürütülmesi önerilmektedir. This is a descriptive study carried out to examine the Opinions of Relatives and Nurses in Relation to the Training Needs of Patients Receiving Hemodialysis Treatment by using a Q Method Analysis. The study has been carried out between December 2017 and March 2018 in Private Ölmez Dialysis Center and Private Nefrotrans Karatay Dialysis Center in Konya with 30 patients receiving hemodialysis treatment, 30 relatives in charge of their care and 30 nurses who provided hospital care and treatment. As data collection instruments, descriptive feature questionnaires for patients, relatives and nurses, and 22 question cards consisting of hemodialysis training items were used. In addition, 2 blank question cards have been used for additional questions. The researcher has collected the data by using the face-to-face interview technique using the Q method (cards sorted by importance). The collected data has been analyzed by percentile and frequency methods. In the study, it has been found that some of the thoughts of the patient's relatives and nurses were different on the educational needs of the hemodialysis treatment patients while some thoughts were the same. When we consider the order of importance of the education subjects of the patient, relatives responsible for their care and the nurses, it has been concluded that; Patients are more likely to focus on educational issues that include psychosocial issues, such as getting psychological support when needed, coping with social life adaptation problems, The relatives responsible for patient care give importance to fistula / catheter care and training subjects such as preventing complications and coma preventive measures, Nurses are more interested in the educational and theoretical knowledge such as the structure and functions of the kidney and the mechanism of hemodialysis. According to the results of the study, it is suggested the inclusion of patients, relatives and the nurses to the training plan and the use of Q method for planning the priorities of the learning needs of patients, caregivers and nurses prior to the training. It is suggested that nurses should learn the Q method. So that they can determine the order of importance of the patients and their caregivers and plan the training in accordance with the needs of patients and caregivers.Keywords: Hemodialysis: CRF, Relative of the Patient, Nurse, Q Method.
