Abstract
Alopesi Areata, çoğunlukla saçlı deride olmak üzere vücudun kıllı bölgelerinde görülen, alevlenme ve remisyonlarla seyreden kıl kaybıdır. Hastalığın sebebi tam olarak bilinmemekle birlikte, otoimmünite, çevresel ve ruhsal faktörlerin tetikleyiciliği üzerine araştırmalar yapılmaktadır. Bu çalışmanın amacı, Alopesi Areata ile depresyon arasındaki ilişkinin erken dönem nesne ilişkileri bağlamında psikanalitik kuramdan yararlanılarak araştırılmasıdır. Araştırmaya 30 kişi katılmıştır. Araştırma grubu, Alopesi Areata tanısı almış, yaşları 20-39 arasında değişen 15 yetişkin gönüllü hastadan oluşmaktadır. Araştırmanın kontrol grubu eğitim düzeyi ve cinsiyet değişkenleri araştırma grubuna eşit olacak şekilde oluşturulmuştur. Katılımcılarla psikanalitik yönelimli bir ön görüşme yapılmış, ardından Rorschach testi ve Beck Depresyon Envanteri kullanılmıştır. Elde edilen veriler Mann Whitney U testi kullanılarak karşılaştırılmıştır. Araştırma grubu ile kontrol grubu arasında Beck Depresyon Envanteri puanları açısından anlamlı fark elde edilmiştir (p<.05). Rorschach testinin niceliksel analizinden elde edilen bulgulara göre, araştırma grubu ile kontrol grubu arasında nesne kaybı endişesi ve manik savunmalara başvurma açısından anlamlı fark elde edilmiştir (p<.05). Rorschach testinin niteliksel analizinde araştırma grubunun yanıtlarında anne imgesinin tehlikeli algılandığı, boşluklara ve ayrılığa karşı hassasiyet olduğu gözlemlenmiştir. Araştırma grubunun görüşme süresince hissettikleri dayanak ve kapsanma ihtiyacı, erken dönemde annenin kapsayıcılığı ve uyarılımları karşılama yetisi ile ilgili yetersizliklere gönderme yapmaktadır. Tüm bu veriler doğrultusunda, Alopesi Areata hastalarının nesne kaybı endişesi (depresif endişe) yaşadıkları düşünülmektedir. Alopecia Areata is a loss of hair seen with exacerbations and remissions, seen in the hairy patches of the body, and mostly on the scalp. Although the cause of the disease is not fully known, research is being conducted on the triggering of autoimmunity, environmental and psychological factors. The aim of this study is to investigate the relationship between Alopecia Areata and depression using psychoanalytic theory in the context of early object relations.30 people participated in the research. The research group consisted of 15 adult volunteer patients, aged 20-39 years, who were diagnosed as Alopecia Areata. The control group of the study was designed to be equal in terms of education level and gender. A psychoanalytic preliminary interview was conducted with the participants, followed by the Rorschach test and the Beck Depression Inventory. The obtained data were compared using the Mann Whitney U test. There was a significant difference between the research group and the control group in terms of Beck Depression Inventory scores (p <.05). Findings from the quantitative analysis of the Rorschach test showed that there was a significant difference between the research group and the control group in terms of object loss anxiety and use manic defenses (p <.05).In the qualitative analysis of the Rorschach test, the researchers' responses showed that they perceived the mother image as dangerous and had sensitivity to separation. The need for support and encouragement during the interview refers to the insufficiency of early period motherhood inclusiveness and the ability to meet the excitation. Alopecia Areata patients, are thought to have experienced object loss concern (depressive anxiety) in the direction of all these data.
