Nonalkolik Steatohepatit`de Gemfibrozil`in tedavi edici değeri (Prospektif kontrollü çalışma)

Abstract
ÖZET Nonalkolik steatohepatit(NASH) klinik pratikte sık olarak karşılaşılan bir karaciğer hastalığıdır. Hastalık daha çok orta yaş grubunda görülmekte ve çoğu zaman bazı metabolik durumlarla birliktelik göstermektedir. NASH özellikle obez, diyabetik ve hiperlipidemili hastalarda karşımıza çıkan bir hastalık tablosudur. Patogenezi tam olarak anlaşılabilmiş değildir. Bununla birlikte periferik yağ dokuları ile karaciğer arasındaki yağ döngüsünün herhangi bir aşamasındaki bozukluğun NASH' e neden olabileceği kabul edilmektedir. NASH'in tedavisi konusunda çok az bilgi bulunmaktadır. Obez hastalarda önerilen kilo kaybı ve Ursodeoxycholic acid(UDCA)'in etkili olabileceğini bildiren bir çalışma dışmda önerilen başka bir tedavi yaklaşımı yoktur. Bu çalışmada, NASH'li hastalarda lipid düşürücü bir ilaç olan gemfibroziFin tedavi edici değeri araştırılmıştır. Randomize olarak iki gruba ayrılan 46 hastanın; tedavi grubunda(n=23) 4 hafta süreyle gemfîbrozil(600 mg/gün) kullanılırken, kontrol grubu(n=23) 4 hafta süreyle ilaçsız izlenmişdir. Tedavi grubunda ortalama ALT düzeyi 72.0 ± 37.0 IU/L'den 49.7 ± 28.2 IU/L'ye (p=0.008), ortalama AST düzeyi 49.1 ± 20.4 IU/L'den 38.9 ± 20.2 IU/L'ye(p=0.043) ve ortalama GGT düzeyi 55.6 ± 34.7 IU/L'den 40.7 ± 30.4 IU/L'ye(p=0.0001) düşmüş, kontrol grubunda ise anlamlı değişiklik saptanmamıştır. ALT ve AST düzeyi bakımından yanıtlı ve yanıtsız hastaların tedavi öncesindeki trigliserid düzeyleri arasında anlamlı farklılık bulunmamıştır. Çalışma sonuçlan gemfibrozil' in NASH tedavisinde yararlı olabileceğini göstermişdir. Bu konuda yapılacak yeni çalışmalarla en uygun tedavi dozu, uzun süreli tedavinin sonuçlan ve tedavinin histopatolojik bulgulara katkısı belirlenmiş olacaktır. 24 SUMMARY Nonalcoholic Steatohepatitis (NASH) is a frequent liver disease. The disease usually occurs in the middle age group and most of the time it occurs with other metabolic disorders. NASH is a clinical picture seen mostly in obese, diabetic and hyperlipidemic patients. Eventhough it's pathogenesis is not known it's accepted that it occurs as a result of defect in the fat cycle between peripheral fatty tissue and the liver. Upto now there's very little information on treatment of NASH; and apart from weight loss for obese patients and a study which showed that Ursodeoxycholic acid may be effective. In this study the effectiveness of a lipid lowering drug gemfibrozil was tested on patients with NASH. Forty-six patients were divided randomly into a study(treated) group (n: 23) who used gemfibrozil (600 mg/d) for 4 weeks and a control group (n: 23) who were followed for 4 weeks without any treatment. In the study group, mean ALT level decreased from 72.0 ± 37.0 IU/L to 49.7 ± 28.2 IU/L(p=0.008), mean AST level from 49.1 ± 20.4 IU/L to 38.9 ± 20.2 IU/L(p=0.043) and mean GGT level from 55.6 ± 34.7 IU/L to 40.7 ± 30.4 IU/L(p=0.0001) while there was no significant change in the control group. Among the patients with responsive and unresponsive ALT and AST levels, there was no significant difference in their pretreatment TGL levels This study showed that gemfibrozil may be useful in the treatment of NASH. New studies that will be undertaken concerning this topic will have to show effective drug dosage, long term results of the treatment and histopathologic findings of the treatment. 25
