Abstract
Amaç: Çağımızın önemli sağlık sorunlarından biri olan morbid obezitenin etiyopatogenezinde değişik faktörler rol oynasa da, vücut ağırlığını belirleyen enerji dengesinin alım lehine bozulması asıl neden olarak öne sürülmektedir. Bu dengenin sağlanmasında anahtar rolü oynayan hipotalamus ve diğer organlar arası iletişim özel peptitlerle sağlanmaktadır. Bu peptitlerin başında grelin, leptin, öreksin ve insülin gelmektedir. Bu çalışmada amaç; günümüzde morbid obezitenin tedavisinde başarılı bir yöntem olarak kabul edilen laparoskopik gastrik bant tatbikinin, iştah üzerine etkili bu peptitler üzerindeki etkilerini incelemektir. Gereç ve yöntem: Çalışmaya morbid obezite (VKİ:48,3±6,7 kg/m') tanısı ile ameliyat edilen 20 kişilik (12 kadın, 8 erkek) hasta grubu ile vücut ağırlığı normal sınırlarda olan (VKİ:22,6±2,0 kg/m2), sağlıklı 20 kişilik (10 kadın, 10 erkek) kontrol grubu dahil edilmiştir. Hasta grubunda ameliyat öncesi ve sonrası birinci ay sonunda, kontrol grubunda bir kez kan örnekleri alındı. Çalışmadaki parametreler ELİSA yöntemiyle değerlendirildi. Bulgular: Hasta ve kontrol grubu arasında anlamlı yaş ve cinsiyet farkı yoktu. (p>0,05) Hasta grubunda uygulanan ameliyat ile birinci ayın sonunda anlamlı kilo kaybı sağlandı. (p<0,05) Plazma grelin düzeyi kontrol grubuna göre morbid obezlerde düşük (p:0,033) olarak saptanırken ameliyat sonrası artmış (p:0,014) olarak saptandı. Leptin ise morbid obezlerde kontrol grubuna göre yüksek (p:0,000) olarak tespit edilirken ameliyat sonrası azalmış (p:0,01) olarak tespit edildi. Oreksin-A morbid obezlerde yüksek (p:0,000) olarak tespit edilirken ameliyat sonrası düşük (p:0,000) olarak tespit edilmiştir. İnsülin direnci ile ilgili sonuçlar, leptin ve oreksin-A ile benzerlik göstermektedir, (sırasıyla p:0,000 ve p:0,008) Tartışma: Laparoskopik gastrik bant tatbiki, morbid obezite tedavisinde bir ayda anlamlı kilo kaybı sağlar. Hastalarda VKİ'nde azalma ile birlikte iştah üzerine önemli etkilere sahip peptitlerden grelinin düzeyi artar; leptin, oreksin-A, insülin düzeyi ve insülin direnci azalır.Anahtar kelimeler: Morbid obezite, gastrik bant, grelin, leptin, oreksin-A, insülin direnci Purpose: While different factors in etyopathogenesis of morbid obesity, which is most important healty problems of recent times, plays most important role, changing energy balance in favour of intake which determines body weight is suggested as the main reason. The communication between the hypothalamus, which play important role in the maintanace of this balance, and the other organs is provided by special peptides. Grelin, leptin, öreksin ve insulin are the important ones of these peptides. The purpose of this study is to investigate the effects of laparoscopic gastric band applications, a current effective treatment modality for morbid obesity, on peptides which is effective on appetide. Materials and methods: We studied 20 patients (12 female, 8 male) who were operated for morbid obesity (BMI:48,3±6,7 kg/m2) and 20 normal weighted (BMI:22,6±2,0 kg/m2) and healty persons (10 female, 10 male) for the control group. We take blood samples one time from the control group and two times from the study group (before and one month after the operation). The parameters in the study were evaluated by ELISA method. Results: There were no statisticaly meaningful age and sex differences between the control and study groups (p>0,05). By the operation significant weight loss was achieved in the first month (p<0,05). While plasma grelin level was lower in the morbid obesity group when compared with the control group (p:0,033), it increased after the operation (p:0,014). On the other hand while leptin was higher in morbid obesity group than the control group (p:0,000), it decreased after operation (p:0,01). Öreksin- A was measured higher in morbid obesity group (p:0,000) but it was measured lower after the operation (p:0,000). The reults of insulin resistance measurements were similar to the leptin and oreksin-A (in order p:0,000 and p:0,008) Conclusion: Laparoskopic gastric band application provides significant weight loss in morbid obesity treatment in one month. While BMI of the patients decrease, the level of grelin, one of the peptides that have important effects on appetide, increases; leptin, oreksin-A, insulin levels and insulin resistance decrease.Key Words: Morbid obesity, gastric banding, grelin, leptin, oreksin-A, insulin resistance
