Ek tedavi olarak Levetirasetam alan dirençli epilepsi hastalarında nöbetsizliği belirleyen etkenlerin geriye dönük araştırılması

Abstract
Giriş: Pek çok epilepsi sendromunda faydalı yeni bir antiepileptik ilaç olan levetirasetam (LEV) özgül niteliklere sahiptir. İlginçtir ki, LEV ile tedavi edilen hastaların %90'ı, iki apayrı grupta sınıflandırılabilir: hastaların yaklaşık 1/3'üne denk gelen ?öncül yanıtsızlar?, ve ?öncül yanıt verenler?. Bu ikili dağılım güçlü bir şekilde, oldukça sınırlı sayıda belirleyicinin famakotepkideki değişime katkıda bulunduğunu vurgulamaktadır. Fakat henüz bu beklenmedik duyarlılığı sergileyen hastaları tahmin yolu mevcut değildir. Bu araştırmada, LEV ile yapılan ek tedaviye beklenmedik bir şekilde cevap veren ilaca dirençli hastaları belirlemeye çalıştık ve ilaca cevap vermeyen hastalarla karşılaştırdık. Daha sonra, bu hasta gruplarını farklılaştıran klinik özellikleri karakterize ederek verilecek cevabın tahminini sağlayacak göstergeleri belirlemeye çalıştık.Hastalar ve yöntem: LEV tedavisine tabi tutulmuş ve yaşları 16-78 arasında olan 110 dirençli epilepsi hastasının medikal kayıtlarını geriye dönük olarak değerlendirdik. 15 hasta yan etki (n:10), nöbetlerde artış (n:3) veya etkinliğin olmaması (n:2) nedeni ile analizlerin dışında bırakıldı. Cevabı, LEV başlanmasından sonra en az 6 ay süre ile: a) nöbetsiz, b) nöbet sıklığında %50'den fazla azalma, c) nöbet sıklığında %50'den az azalma veya nöbet kontrolünde gelişme olmaması ve d) balayı şeklinde sınıflandırdık.Sonuçlar: Önceden dirençli olan 18 hasta (%18,9) LEV ile en az 6 ay boyunca nöbetsizlik ve 28'i (%29,5) nöbet sıklığında %50'den fazla azalma elde etmişti. Epilepsi süresi, yaş, cinsiyet, bazal nöbet sıklığı, önceki ve eşzamanlı AEİ sayısı ile LEV cavabı arasında ilişki saptanmadı. Nöbetsiz hastaların %72'sinde LEV dozu 1000 mg/gün veya daha azdı. Balayı grubundaki ortalama tolerans süresi 12,0 ± 8,5 ay (min 1,5-max 30 ay) idi. Post-travmatik epilepsi teşhisi istatistiki olarak daha iyi bir sonuç verirken (p=0,039), TLE ve HS için karşıt bir yanıt gözlendi (p=0,041).Yorum: Bu araştırma LEV'a tedavi yanıtını etkileyebilecek bazı klinik ve laboratuar özellikler sunmaktadır. Introduction: Levetiracetam (LEV), a newer antiepileptic drug useful for several epileptic syndromes, has specific characteristics. It is interesting, %90 of patients treated with LEV can be classified into two distinct proups:?a priori non-responders? accouting for approximately 1/3 of patients, versus ?primary responders?. This dichotomous distribution strongly emphasize that a very limited number of determinants contribute to the variance of pharmacoresponse. But as yet there is no way of predicting these unexpectedly responsive patients. In this study, we set out to identify previously drug-resistant patients who had demonstrate unexpected responsiveness to add-on therapy with LEV, and compared these to patients who had not responded to the drug. We then attempted to identify any predictors of response by characterising the clinical features that differentiated these groups of patients.Methods: We retrospectively evaluated medical records of 110 patients aged 16-78 years with refractory epilepsy exposed to LEV. 15 patients were excluded from the statistical analyses due to adverse effects (n:10), worsening of their seizure control (n:3) or lack of efficacy (n:2). We classified response into a) seizure-free, b) greater than %50 reduction in seizure frequency and c) less than %50 reduction in seizure frequency or no impovement of seizure control for a minimum of 6 months after commencing LEV and d) honeymoon.Results: 18 (%16,4) of the previously refractory patients had achieved seizure-freedom and 28 (%25,5) of them had reduction of seizure frequency greater than %50 for at least 6 months with LEV. The average tolerance duration of honeymone group (n:22;%23,2) was 12,0 ± 8,5 months (min 1,5-max 30 ). The dose of LEV in %72 of seizure-free patients was 1000 mg/day or less. The duration of epilepsy, age, sex, baseline seizure frequency, number of previous and concomitant AEDs were not associated with response to LEV. The diagnosis of post-traumatic epilepsy was statistically associated with a better response (p=0,039) but, a contrary response was observed for TLE and HS (p=0,041).Conclusion: The present study offers some clinical and laboratory features suggesting various response to LEV. But significantly larger and homogeneously phenotyped groups of patients in a prospective design need to be investigated to draw any firm conclusions.Key words: Levetiracetam, response, seizure-free, refractory epilepsy.
