Abstract
Giriş:Parotis bezi şişlikleri, tümörler, inflamatuar olaylar veya kistler başta olmak üzere çok çeşitli nedenlerle ortaya çıkmaktadır. Klinik inceleme ve radyolojik teknikler bu lezyonların natürünü belirlemede yetersiz kalmaktadır. İnce iğne aspirasyon sitolojisi baş ve boyun bölgesindeki şişliklerin incelenmesinde, basit, hızlı ve güvenilir bir metod olarak, başlangıçta kullanılabilecek temel tanısal yöntemdir.Amaçlar:Bu çalışmanın amacı, parotis bezi şişliklerinin tanısında, İİAS ve histopatoloji sonuçlarını karşılaştırmak ve İİAS' nin sensitivite ve spesifitesini tesbit etmektir.Materyal Metod:İstanbul Üniversitesi Cerrahpaşa Tıp Fakültesi 2007-2012 yılları arasında uygulanmış 82 tükrük bezi İİAS retrospektif olarak değerlendirildi, rezeksiyon materyali de olan 50 olguda İİAS ile histopatolojik tanılar karşılaştırıldı ve aynı zamanda hastaların epidemiyolojik özellikleri incelendi.Bulgular:İİAS' nin doğru tanıyı tesbit etmede sensitivitesi %66,6 , spesifitesi %97,7 , pozitif prediktif değeri %80 ve negatif prediktif değeri %95 olarak bulundu. Yararlılık oranı ise %94 olarak tesbit edildi.Sonuçlar:İİAS parotis bezi lezyonlarının önemli bir kısmını tesbit etmede yüksek oranda sensitif ve spesifik bir tekniktir. Operasyon öncesi tanı, cerrahi girişimin gerekliliğini tesbit ve rezeksiyon öncesi yeterli cerrahi yaklaşımı planlamak için son derece faydalı bir yöntemdir.Anahtar kelimeler: Parotis bezi lezyonları, ince iğne aspirasyonu, sito-histopatolojik kolerasyon. Introduction:Parotid gland swellings can arise from a number of aetiologies including tumours, inflammatory processes or cysts. Clinical examination and imaging techniques can be inaccurate to reveal the exact nature of these lesions. Fine needle aspiration cytology (FNAC) is used as the main initial diagnostic investigation for swellings in the head and neck region as a simple, rapid and safe method.Aims:The aims of this study are to correlate FNAC diagnoses with histopathology and to establish the sensitivity and specificity of FNAC in diagnosis of parotid gland swellings.Material and Method:A total of 82 salivary gland FNAs performed at Istanbul University Cerrahpaşa Medical College between 2007 and 2012 were retrospectively reviewed and the correlation between the FNAC and the histopathologic diagnosis of the 50 cases with resection samples as well as the epidemiological information of patients were evaluated.Findings:The sensitivity of FNAC to establish the accurate diagnosis was %66,6 and the specificity was %97,7 with a positive predictive value of %80 and negative predictive value of %95. The effectiveness rate was %94.Results:FNAC is highly sensitive and specific technique for the diagnosis of parotid gland lesions. It is a valuable tool for pre-operative diagnose and to determine the need for surgical intervention and assist in planning the appropriate surgical approach prior to resection.Key Words: Parotid gland lesions, fine needle aspirations, histopathology correlation.
