Hipertermik intraperitoneal kemoterapinin sıçanlarda kolon anastomoz iyileşmesi üzerine etkileri

Abstract
Giriş: Peritoneal yüzey maligniteleri karın içi kanserlerin en sık ölüm sebebidir. Sitoredüktif cerrahi ile birlikte kullanılan hipertermik intraperitoneal kemoterapi bu hastalığın tedavisinde ve önlenmesinde etkin bir tedavi yöntemidir. Ancak bu tedavinin önemli morbidite ve mortalite oranları vardır. Sitoredüktif cerrahi ve hipertermik intraperitoneal kemoterapinin anastomoz iyileşmesi üzerindeki etkileri hala tartışma konusudur. Amaç: Bu çalışma hipertermik intraperitoneal kemoterapi tedavisinde kullanılan kemoterapötik ajanların sıçanlarda kolon anastomoz iyileşmesi üzerine etkilerini araştırmak için tasarlandı. Gereç ve Yöntem: Ağırlığı 250-300 g olan 60 adet Wistar Albino sıçan çalışmaya dahil edildi. Sıçanların tümüne cerrahi işlem olarak sigmoid rezeksiyon ve uçuca kolorektal anastomoz uygulandı. Grup 1' e sadece cerrahi işlem, Grup 2' ye cerrahi işlem ile birlikte hipertermik intraperitoneal yıkama, Grup 3, 4, 5 ve 6'ya sırasıyla Mitomisin C, Sisplatin, Oksaliplatin ve Doksorubisin ile hipertermik intraperitoneal kemoterapi uygulandı. Bulgular: Gruplar aralarında karşılaştırıldığında patlama basınçları arasında anlamlı fark saptanmadı(p=0.81). Hidroksiprolin değerleri karşılaştırıldığında Grup 1 ve Grup 2'de hidroksiprolin değerleri anlamlı olarak diğer gruplardan daha yüksek bulundu (p<0.001). Patolojik değerlendirmede anastomoz iyileşme parametreleri hipertermik intraperitoneal kemoterapi uygulanan gruplarda daha kötü olduğu tespit edildi. Sonuç: Biyokimyasal ve patolojik veriler hipertermik intraperitoneal kemoterapinin olumsuz yönlerini göstermektedir ancak klinikte en önemli parametre olan patlama basıncı açısından olumsuz bir etki yaratmamıştır. Background: Peritoneal surface malignancies are the most common cause of death for intraabdominal cancers. Cytoreductive surgery with concomitant Hypertermic Intraperitoneal Chemotherapy is an effective way to treat and prevent this disorder. However, this treatment has been carried out important morbidity and mortality rates. Aim: This study was designed to investigate the effects of chemotherapeutic agents that have been used in Hyperthermic Intraperitoneal Chemotherapy treatment on the healing of colon anastomose in rats. Materials and Methods: 60 Wistar Albino rats weighing 250-300 g were included to the study. Sigmoid resection and end-to-end colorectal anastomosis was done as a surgical procedure in all rats. Group 1; rats underwent only surgical procedure, Group 2; rats underwent surgical procedure with hypertermic intraperitoneal lavage. Surgical procedure with concomitant Hyperthermic Intraperitoneal Chemotherapy treatment was done in the groups 3, 4, 5, 6 with Mitomicin C, Cisplatin, Oxaliplatin ve Doxorubicin, respectively. Results: When the groups were compared each other, there was no significant difference between bursting pressure values (p=0.81). However, Group 1 and 2 showed significantly higher values for hydroxyproline compared to the other groups (p <0.001). Pathological evaluation showed worse results on anastomotic healing in hyperthermic intraperitoneal chemotherapy treatment groups. Conclusion: Although Hyperthermic intraperitoneal chemotherapy treatment on the healing of colorectal anastomosis showed negative results in terms of biochemical and pathological results, it showed no detrimental effect on anastomotic bursting pressure, which is clinically the most important factor.
