Majör depresyonda hipokretin reseptör gen polimorfizmlerinin araştırılması

Abstract
Hipokretinler (Oreksinler) hipotalamus nöronlarında sentezlenen endojen nöropeptidlerdir. Bu nöropeptidler ve reseptörleri, beslenme sisteminde, sıvı alımında, enerji homeostazında, endokrin sistemde ve Majör Depresyonda bozuk olan uyuma/uyanıklık dengesinin düzenlenmesinde çok büyük bir rol oynarlar. Son yapılan çalışmalarda oreksinlerin Majör Depresyon Bozukluğunun (MDB) nöro-biyolojisinde büyük rolü olduğunu savunulmaktadır. Çalışmamızın amacı, toplumun önemli bir kesimini etkileyen majör depresyon hastalığına yatkınlık sebebi olabilecek, HCRTR1 ve HCRTR2 genlerindeki üç tane tek nükleotid polimorfizmlerinin (SNP) sıklıklarını ve hastalıkla ilişkilerini belirlemek.Çalışmamız, HCRTR1 geni rs10914456/ rs2271933 ve HCRTR2 geni rs2653349 polimorfizmleri için MD tanısı konmuş 75 hasta ve 87 sağlıklı kontrol grubu ile yapıldı. Polimorfizimler 'Gerçek Zamanlı Polimeraz Zincir Reaksiyonu' yöntemi ile saptandı. MD şiddetini belirlemek için Hamilton veBeck Depresyon ölçeklerinden yararlandı. HCRTR1-rs10914456 genotip frekansları MDB hastalarında CC%33.3, CT %42.7 TT %24, kontrol grubunda ise CC %56.3 CT %32.2 TT %11.5 ve anlamlı olduğu bulundu (p=0.009). rs2271933 genotip frekasları MDB hastalarında CC %33.3, CT %40, TT %26.7, kontrol grubunda ise CC %56.3, CT %32.2, TT %11.5 ve anlamlı olduğu bulundu (p=0,006). Hamilton skoru cinsiyet açısından karşılaştığında anlamlı bir fark bulunmadı (p>0.05). Beck skoru cinsiyet açısından karşılaştığında, kadınlarda sınırda anlamlı olarak yüksek bulundu(p=0.059). Sonuç olarak bu çalışma Türk toplumunda MDB hastalarında HCRTR1 rs10914456, rs2271933 ve HCRTR2 rs2653349 gen polimorfizmlerinin çalışıldığı ilk araştırmadır. Bu çalışmada MDB hastalarında HCRTR1 rs10914456, rs2271933 polimorfizmleri kontrol grubuna göre anlamlı olarak farklı bulunduğundan bu polimorfizmler Türk popülasyonunda depresyon hastalarında marker olarak kullanılabilir. Hypocretins (Orexins) and their receptors are neuropeptides which are synthesized in hypothalamic neurons. They play important role in feeding, in energy homeostasis and in endocrinal system. Recent studies suggest that hypocretins plays role in improving the balance of disturbed sleep, that is commonly seen during the major depression disorder (MDD) neurobiology and disease. Aim of this study is to investigate the role of hypocretin receptor genes in patients with MDD, evaluating HCRTR1 and HCRTR2 gene variants to determine the relationship between the occurrence of the disease and clinical development.In our study were used 75 patients diagnosed with MDD and 87 healthy controls. rs10914456, rs2271933, and rs2653349 polymorphisms of HCRTR1 and HCRTR2 gene respectively were determined with method of Real-time PCR. Hamilton and Beck depression scales were used to determine the severity of MDD.The patient and control groups were compared in terms of HCRTR1 rs10914456 genotype as; in patients, CC%33.3, CT %42.7 TT %24 , in control group, CC %56.3 CT %32.2 TT %11.5 and allele frequencies did differ significantly (p=0.009). rs2271933 genotype frequencies in patients with MDD were determined as CC 33.3%, CT 40%, and TT 26.7%; and in control group CC %56.3, CT %32.2, TT %11.5 and allele frequencies did differ significantly (p=0.006). Beck scores encounters in terms of gender, in women's were significantly higher in borderline (p = 0.059).As a conclusion, HCRTR1 rs10914456, rs2271933 and HCRTR2 rs2653349 polymorphisms are studied for the first time in patients with MDD in Turkish population. According to our findings, HCRTR1 rs10914456, rs2271933 polymorphisms are found to be significant in patients with MDD. So these polymorphisms can be used as markers in depressed patients in Turkish population.
