Abstract
ÖZET Amaç; Çocukluk çağında görülen Non-Hodgkin lenfoma'da yoğun ve kombine tedavilerin kullanılması sonucu beş yıllık sağ kalım tümör evre ve alt grubuna göre değişmekle birlikte % 80' tere ulaşmıştır. Tedavi yoğunluğu, artan kür oranlarıyla birlikte toksik yan etkilerin artışım da beraberinde getirmektedir. Bu çalışmada bölümümüzde Non- Hodgkin lenfoma tanısı alıp, NHL-BFM 90 tedavi protokolü alan hastalarda ortaya çıkan lokal ve sistemik akut toksisitenin değerlendirilmesi amaçlanmıştır. Ayrıca akut toksisiteyi izlemek amacıyla yapılan tetkik sıklığını belirlemek ve bu verilerin daha sonra tedavi edilecek olguların toksisite izîemi için plan oluştururken kullanmayı amaçladık. Hastalar - Yöntem: Bu çalışma î Ocak 1993-31 Mayıs 2001 tarihlerinde Dokuz Eylül Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Anabilim Dalı Pediatrik Onkoloji Bilim Dalı'nda Non-Hodgkin lenfoma tanısı ile NHL BFM-90 kemoterapi protokolü İle tedavi edilen hastaların kemoterapi ile ilişkili toksisite verilerinin retrospektif analizi ile yapıldı. Bulgular - Sonuçlan Toplam 19 olguya NHL BFM 90 kemoterapi protokolü uygulandığı, bunlardan yedi hastanın Non-B hücreli, dokuz hastanın B hücreli, üç hastanın ise büyük hücreli anapîastik Non-Hodgkin lenfoma tedavisi aldığı belirlendi. Hastaların ortanca yaşları 12,3 (4 -17), erkek/kız oranının 2,16 olduğu görüldü. Çalışma grubunda Evre I hasta saptanmazken, on beş hastanın (% 79) ileri evre hastalıkla başvurduğu görüldü. Çalışma grubunda fataliteye neden olan akut toksisite izlenmediği, olguların %95'inde kemoterapinin akut toksisitesine bağlı olarak tedavi gecikmesi yaşandığı; gecikmelerinin en sık saptanan nedenlerinin myelosupresyon, nötropenik ateş ve oral mukozit olduğu belirlendi. Meydana gelen toksisite atakları incelendiğinde; B hücreli Non-Hodgkin tedavi grubunda saptanan 3° - 4° toksisite ile uyumlu myelosupresyon ataklarının (% 72) Non-B hücreli Non-Hodgkin lenfoma tedavi grubunda saptanan ataklara göre (% 53.6) anlamlı olarak yüksek olduğu saptanmıştır. (p:<0.001) Çalışma grubunun % 95'inde enfeksiyöz atak meydana geldiği, atakların % 64'ünün nötropenik ateşle uyumlu olduğu, izlenen nötropenik ateş ataklarında hasta kaybedilmediği belirlenmiştir. Olguların % 79'unda oral mukoza lezyonu meydana geldiği saptandı. En sık rastlanılan nörotoksisite periferik nöropati olup, bir olguda geri dönüşümsüz 4° nörotoksisite geliştiği görülmüştür. Diğer akut toksisiteîerin tedavi gecikmesine ve direkt morbidite ve mortaliteye neden olmadığı saptanmıştır.Myelosupresyon dışındaki major organ, toksisiteîerinin büyük çoğunluğunun 1° - 2° toksisite ile uyumlu olması nedeniyle, tedavi bloğu girişi harici dönemlerde laboratuar izlem sıklığının azaltılabileceği düşünülmüştür. Sonuç olarak çalışma süresince direkt toksisiteye bağlı ölüm olmasa da, bir olgu şiddetli nörotoksisite nedeniyle kür şansım yitirmiştir. Diğer yandan myelosupresyon ve enfeksiyonlar ciddi tedavi gecikmesine neden olmaktadır. Bu amaçla rutin primer büyüme faktörleri kullanımı ve influenza aşılaması uygulaması düşünülebilir. Intensive chemotherapy protocols while increasing the cure rates, cause more severe toxic side affects. The purpose of this study is to determine local and systemic side effects of NHL BFM 90 chemotherapy protocol in children with Non-Hodgkin's lymphoma who had been diagnosed and treated in our center. We also aimed to determine the frequency of laboratory investigations and use this data while monitoring the toxicity for new cases. Patients and Methods; From January 1993 to May 1998, chemotherapy related toxicity was investigated retrospectively in children who were diagnosed as Non-Hodgkin's lymphoma and received NHL BFM 90 chemotherapy protocol in our center. Results - Conclusions: A total of 19 patients were studied. Seven had Non-B cell, nine had B cell and three had large cell anaplastic Non Hodgkin's lymphoma. The median age was 12.3 (range, 4-17), male to female ratio was 2.16:1. None of the patients had stage I disease, whereas 15 patients (79 %) presented with advanced stage disease. No toxic deaths occurred during the study period but there was a treatment delay in 95% of study group. Major causes of treatment delay were myelosupression, febrile neutropenia and oral mucositis. Grade 3-4 myelosupression was more frequent in B cell Non-Hodgkin's lymphoma than Non-B cell group. (p< 0.001) Infectious episodes were seen in 95 % of study group; 64 % of infectious episodes were febrile neutropenia. No patient died of infectious toxicity. Oral mucositis was seen 79 % of study group. Peripheral neuropathy was the most frequent neurotoxicity. One patient was excluded from protocol because of irreversible 4° neurotoxicity. Other acute toxicies did not cause direct morbidity or mortality. Except myelosupression, most of the toxicities were grade 1 - 2, so during the periods other than beginning of therapy blocks» less frequent laboratory analyses may be performed. Although no toxic deaths occurred, one patient lost her curability change because of severe irreversible acute neutoxicity. On the other hand myelosupression and infections cause serious treatment delays. For that reason, using routine primary prophylactic growth factor and vaccination against influenza may result in less severe myelosupression and less frequent infectious episodes.
