Üniversite hastanesinde cerrahi hastalarda belirlenen malnütrisyon prevalansının riskin değerlendirilmesi ve postoperatif morbidite ile ilişkisi

Abstract
ÖZET Dokuz Eylül Üniversitesi Tıp Fakültesi Hastanesi'nde.elektif cerrahi girişim geçirecek 18 yaş ve üzeri hastalarda gerçekleştirilen bu çalışmada, yatış sırasında malnûtrisyon prevalansınm Sübjektif Global Değerlendirme (SGD) yöntemiyle saptanması, hastaların beslenme durumları ile ASA fiziksel durum sınman arasındaki ilişkinin araştırılması, malnütrisyonun postoperatif morbidite ve mortalite ve hastanede kalış sûresine etkisinin araştırılması amaçlandı. Çalışmaya sekiz cerrahi anabilim dalından elektif cerrahi girişim geçirecek 360 olgu dahil edildi Bu olguların beslenme durumları SGD yöntemi üe saptandı. Geçirdikleri cerrahi girişimlerin tipi, postoperatif komplikasyonlar, mortalite ve hastanede kalış sureleri kaydedildi Malnûtrisyon prevalansı %36.2 olarak bulundu. Olguların % 63.8'inde malnûtrisyon bulunmadığı, %28.1'inde orta derecede ve %8.1'inde ağır malnûtrisyon olduğu belirlendi. Malignitesi olan 87 olgunun % 56.3'ûnde, malignitesi olmayan 273 olgunun %29.7'sinde malnûtrisyon saptandı (pO.0001). Malnütrisyonun orta ve büyük operasyon geçirecek olgularda daha fazla olduğu belirlendi (p<0.0001). ASA m-IV sınıflarında malnütrisyonun (%56) anlamlı derecede daha yüksek olduğu saptandı (pO.0001). Postoperatif komplikasyonlar maînütrisyonlu hastalarda daha sık yaşandı (pO.0001). Hastanede kalış süresinin malnütrisyonun derecesiyle korelasyon gösterdiği belirlendi (OR:2.15,p=0.017). Malnütrisyonun cerrahi hastalarda yaygın bir sorun olduğu, postoperatif morbidite ve mortaliteyi olumsuz yönde etkilediği ve bu nedenle beslenme durumu değerlendirilmesinin, rutin preoperatif evalüasyon kapsamına alınması gerektiği; bu amaçla SGD yönteminin kullanılabileceği sonucuna varıldı. Anahtar sözcükler : Malnûtrisyon, prevalans, sübjektif global değerlendirme postoperatif komplikasyon, mortalite, hastanede kalış sûresi The Relationship of Malnutrition Prevalence on Surgical Patients Determined in a University Hospital with Preoperative Risk Evaluation and Postoperative Morbidity SUMMARY The objective of the study was to estimate the prevalence of malnutrition with Subjective Global Assessment on admission in patients undergoing elective surgery who were older than 18 years old in Dokuz Eylül University Hospital In addition, we aimed to assess the relationship between nutritional status and ASA physical status scores and the effect of malnutrition on postoperative morbidity and mortality as well as length of hospital stay. A total 360 patients from eight surgical departments were included to the study. The nutritional status of patients was defined by using Subjective Global Assessment method. Type of surgery, postoperative complications, mortality and length of hospital stay were recorded. Malnutrition prevalence of the entire patient population was 36.2% including moderate malnutrition 28.1% and severe malnutrition 8.1%. We did not find malnutrition in 63.8% of patients. Malnutrition prevalence was 56.3% in 87 malignant patients and 29.7% in 273 non-maHgnant patients (pO.0001). Malnutrition prevalence was higher in patients who had moderate and major surgery (p<0.0001). Prevalence of malnutrition was significantly high (56%) in ASA ffl-IV patients (pO.0001). Postoperative complications occurred more frequentiy in malnourished patients (pO.0001). The length of hospital stay was significantly correlated with the severity of malnutrition (OR=2,15, p=0.017). We concluded that malnutrition is a common problem in surgical patients and it has deleterious effect on postoperative morbidity and mortality. Hence, assessment of the nutritional status should be routine part of the preoperative evaluation and Subjective Global Assessment can be used as a useful tool for tins evaluation.
