Abstract
Amaç: Bu çalışmanın amacı, küçük damar hastalığına bağlı vasküler kognitif bozukluk gelişen olgulardaki kognitif bozuklukların nöropsikolojik testler ve bir sanal gerçeklik uygulaması olan sanal süpermarket uygulaması (SSU) ile değerlendirilmesidir.Olgular ve Yöntem: Bu çalışmaya 75 küçük damar hastası dahil edilmiştir. Ayrıntılı geleneksel nöropsikolojik testler sonucunda vasküler kognitif bozukluk saptanan 39 olgu hasta grubu olarak, nöropsikolojik testleri normal olan olgular kontrol grubu olarak belirlenmiştir. İki gruba ayrıntılı nöropsikolojik testler yanı sıra duygudurum ve işlevsellik ölçekleri ve SSU uygulanmıştır. Verilerin istatiksel analizi SPSS 22.0 programı ile yapılmıştır.Bulgular: Hasta ve kontrol grubu nöropsikolojik test skorları arasında anlamlı farklılık göstermiştir. SSU değişkenlerinden doğru miktarda ödeme ve süre ile nöropsikolojik test sonuçları gruplar arasında anlamlı farklılık göstermiştir. SSU süresi ile nöropsikolojik test sonuçları arasında çeşitli düzeylerde ilişki saptanmıştır. SSU'nun hasta ve kontrol grubu ayırmada %77.8 doğru sınıflama oranına sahip olduğu bulunmuştur. SSU süre cut of değeri 5.51 dakika olarak alındığında %80 sensitivite ve %75 spesifisite oranları ile tanı testi olarak kullanılabileceği gösterilmiştir.Sonuç: Günlük pratikte kognitif bozukluk tanısı için uzun süre alan, uygulanması için uzmana ihtiyaç duyulan nöropsikolojik testler kullanılmaktadır. SSU, küçük damar hastalarında kognitif bozukluk gelişen olguları birbirinden ayırt etmede başarılı bulunmuştur. SSU sayesinde daha kısa sürede daha fazla hastaya test uygulanarak vasküler kognitif bozukluk erken dönemde tespit edilebilecektir.Anahtar sözcükler: İnme, İnme sonuçları, Vasküler kognitif bozukluk, sanal süpermarket uygulaması, Nöropsikolojik test Background: Studies on virtual reality applications in the diagnosis, monitoring and rehabilitation of cognitive disorders are increasing.Objective: The aim of this study is to evaluate neuropsychological tests and viral cognitive impairment in patients with vascular cognitive impairment due to small vessel disease with virtual supermarket application (SSU), a virtual reality application.Subjects and Methods: 75 small vessel patients were included in this study. Thirty-nine patients who were diagnosed with vascular cognitive impairment as a result of detailed conventional neuropsychological tests were identified as a group of patients with normal neuropsychological tests. Two grubes were applied to detail neuropsychological tests as well as mood and functionality scales and SSU. Statistical analysis of the data was performed with the SPSS 22.0 program.Findings: The patient and control group showed a significant difference between the neuropsychological test scores. Neuropsychological test results showed a statistically significant difference between the groups in terms of the exact amount of SSU and duration and duration. There were several levels of relationship between SSI duration and neuropsychological test results. The patients and control group of the SSU were found to have a correct classification rate of 77.8% in discrimination. SSU duration cut-off value of 5.51 minutes has been shown to be useful as a diagnostic test with 80% sensitivity and 75% specificity.Conclusion: In practice, neuropsychological tests are used which require long-term diagnosis of cognitive impairment and require specialist expertise. SSU has been found to differentiate between cases of cognitive impairment in small vessel patients. With SSU, vascular cognitive impairment can be detected early by testing more patients in a shorter time.Key words: Stroke, Stroke results, Vascular cognitive impairment, Virtual supermarket application, Neuropsychological testIn this study, it was aimed to evaluate the cognitive impairments that occur in Amnestic Mild Cognitive Impairment (aMCI) patients by using both neuropsychological testing and Virtual Supermarket Application (VSM). Method: Thirty-seven aMCI patients (mean age: 66,46) and 57 gender, education and age matched healthy controls (mean age: 70,41) were participated in this study. A detailed neuropsychological test battery and Virtual Super Market (VSM) was applied to all participants.Results: There was a significant difference in neuropsychological test scores between aMCI with patients and healthy controls. aMCI-Multiple Domain (aMCI-MD) patients performed lower in SSU variables than aMCI-Single Domain (aMCI-SD) and healthy controls. aMCI-MD patients completed the VSM longer than aMCI-SD patients and controls. With the correlation analyses which all participants were included, several levels of correlations between neuropsychological test scores and Duration variables were detected. The VSM test was found to discriminate between the aMCI and healthy controls with a correct classification rate of 80.9 %.Conclusion: The possible diagnosis of aMCI-SD/MD can be determined by neuropsychological tests. A more practical and screening tool can be used in the VSM clinic with the correct classification rate than more the Mini Mental State Test.
