Abstract
m ÖZET Gerçek hayatta trafik ve iş kazaları, sportif faaliyetlerdeki kazalar, ani vücut hareketleri gibi birtakım olaylar sonucunda, insan vücudunda kemik kırıkları meydana gelebilmektedir. Bu kırıkların tedavi yöntemleri, kırığın şekli büyüklüğü ve vücuttaki yerine göre çeşitlilik arz eder. Bu kırık tipleri arasında en çok rastlanılanlardan birisi de femur (kalça kemiği) boyun kırıklarıdır. Femurun boyun bölgesinden kırılması sonucunda, bu bölgedeki kalça eklemi fonksiyonunu yitirmiş olur. Bu fonksiyonun tekrar kazanılması amacıyla femurun boyun ve baş kısımlarının görevini görmesi amaçlanan ve femur kanalı içine yerleştirilen metal kalça protezleri kullanılır. Protezlerin sabitlenmesi ise çoğu uygulamalarda kemik çimentoları yardımı ile yapılır. Ancak bugüne kadar yapılan bazı uygulamalarda, tedaviden belli bir süre sonra kemikte, çimentoda veya protezde ikinci bir kırılma veya gevşeme gibi istenmeyen bazı durumların ortaya çıktığı tespit edilmiştir. Bu duruma hastanın ani veya uzun süreli vücut hareketleri, kemik, çimento ve protez üçlüsünün farklı mekanik özellikleri, ameliyat sırasında iki malzeme arayüzeyinde oluşan kusurlar, vücuttaki fizyolojik bozuklar, protez şekli ve malzemesi, çimento kalınlığı gibi bir çok faktörün etki ettiği söylenebilir. Bu çalışmada femur boyun kırıklarının tedavisi için, protez, çimento ve kemikten oluşturulan üçlü yapının mekanik uyumsuzları incelenmiştir. Çalışmayı kemik çimentosu kalınlığının ve femur anteversion açısının gerilme dağılımına etkisi ve kırılma mekaniği karakteristik büyüklüklerinin bulunması şeklinde iki kategoriye ayırmak mümkündür. Çalışmanın sonuçlan malzeme arayüzeylerinin en kritik bölgeler olduğunu ve anteversiyon açısı bakımından fizyolojik bozuklukların hareket sırasında yüksek gerilmelere sebep olduğunu göstermiştir. n ABSTRACT Bone fractures in the human body may occur because of some accidents in traffic, work, sports or other events. There are different treatment methods of the bone fractures according to their shape, size and position in the body. Femur (fig. 2.3) neck fractures have an important location among the types of the bone fractures. Femur neck fractures result in the loss of function of the hip joint in the fracture region. In order to gain this joint function again, metal hip prostheses located into the femur canal are used. Fixation of these prostheses into femur canal İs done by using bone cements in many applications. However, after the surgical operations, unwanted results such as second fracture in bone, cement or prosthesis and loosing of the prosthesis, have been shown in many applications so far. It can be said that these results arise from a lot of factors such as a sudden body motion of the patient, differences among the mechanical properties of bone, cement and prosthesis, microcracks in the interfaces, physiological disorders, shape and size of the prosthesis and cement thickness acting these results. In the present study, the reasons of the mechanical mis-match which may arise in the structure comprising hip prosthesis, bone cement and the bone used for the functional treatment of femur neck fractures have been investigated. It is possible to divide this work into two categories; stress analysis as a function of the cement thickness and anteversion angle (fig. 2.4) of the femur and the determination of the characteristic quantities of fracture mechanics. The results show that the interfaces are the most critical regions, and the physiologic disorders in terms of anteversion angle cause the negative effect during movement.
