Abstract
m ÖZET İyot eksikliğinin veya iyot fazlalığının mide kanserinde risk faktörü olduğu hipotez edilmiştir, iyot eksikliğinin sık görüldüğü bölgemizde mide kanseri ilk sıralarda yer almaktadır. Mide kanserli hastalarda; kan, idrar ve dokudaki iyot seviyelerinin mide kanseri ile ilişkili olabileceği düşüncesiyle bu çalışma yapıldı. Çalışmamıza Atatürk Üniversitesi Tıp Fakültesi Genel Cerrahi Kliniğinde mide kanseri tanısıyla yatan 68 hasta (41 erkek, 27 kadın) ile 25 sağlıklı gönüllü birey (10 erkek, 15 kadın) dahil edildi. Hastalar TNM (T:tümör, N: Lenf Nodu, M: metastaz) göre evrelendirildi. Hastalardan idrar, kan ve ameliyat esnasında kanserli dokudan ve midenin normal dokusundan örnekler alınarak idrarda iyot konsantrasyonu, kanda proteine bağlı iyot miktarı ve dokuda mg protein başına ng iyot miktarı tesbit edildi. Analizlerde Sandell-Kolthoff reaksiyonunu temel alan metodlar kullanıldı. Kontrol grubunun idrarda iyot konsantrasyonu (IİK) 16.3 ±2.1 jj.g/dL ve proteine bağlı iyot miktarı (PBI) 6.3+2.3 ig/dl_ olarak ölçüldü. Kontrol grubunda ve mide kanserli hastalarda da IİK ve PBI'nın yaşa ve cinsiyete bağlı olmadığı bulundu. İdrardaki iyot seviyesinin hastalığın IV. evresinde (p<0.05); kandaki proteine bağlı iyot miktarının hastalığın III ve IV. evrelerinde kontrol grubuna göre daha yüksek olduğu (p<0.001) bulundu. Kanserli dokuların iyot içeriğinin (17.8 ± 14.6 ng iyot/ mg protein) komşu sağlam dokudan (41.7 ± 34.8 ng iyot/ mg protein), daha düşük olduğu (p<0.001) tesbit edildi. Bulgularımız literatürde öne sürülen mide kanserinde iyodun bir rolü olabileceği hipotezini kuvvetlendirir. Ancak; hastalığın tanısı ve prognozunun değerlendirilmesi konusunda kesin bilgi vermez. IV SUMMARY It was hypothesized that iodine deficiency or excess is associated with the development of gastric cancer. Iodine deficiency is a common health problem and gastric cancer is one of the most common cancers in this region. In this study, we aimed to investigate whether there is any relationship between gastric cancer and iodine concentration in blood, urine and gastric tissue. A total of 68 patients (41 males and 27 females), who were diagnosed as gastric cancer in the Surgery Clinics of Medical Faculty of Atatürk University, and 25 healthy volunteers (10 males and 15 females) were included in the study. The patients were classified according to the TNM (tumor, necrosis, metastasis) staging. Urine, blood and tissue samples from cancer tissue and cancer-free tissue were taken and iodine concentration in urine, protein-binding iodine level in blood and iodine per mg protein were determined. The methods used were based on Sandell-Kolthoff reaction. In the control group, urine iodine concentration was 16.3 ± 2.1 //g/dL and blood protein-binding iodine level was 6.3 ± 2.3 //g/dL. In the patient and control groups, there were no significant differences in blood protein-binding iodine levels and urine iodine concentration in terms of age and sex. Urine iodine concentration in stage IV (p<0.05) and blood protein-binding iodine level in stages III and IV were significantly higher than those of the control group (p<0.001). Cancer tissue iodine concentration (17.8 ± 14.6 ng iodine /mg protein) was lower than that of cancer-free gastric tissue (41.7 ± 34.8 ng iodine /mg protein) (p<0.001). In conclusion, our study demonstrated that iodine may have an important role in gastric cancer as suggested in the previous studies. Nevertheless, it does not give any information about diagnosis and prognosis of disease.
