Abstract
Çalısmamızdaki amaç, Radyoterapi sırasında olusan saç dökülmelerinde, saçtabulunan eser elementlerin doza baglı olarak degisimini ve saç dökülmesinde rollerininolup olmadıgını gözlemektir.Tedaviye baslamadan önce ve tedaviye basladıktan sonra 1500 ve 3000 cGy'de21 primer veya metastatik hastanın ısınlanan bölgeden toplam 61 saç örnegi alındı.Alınan örnekler gerekli islemlerden geçirilerek Atatürk Üniversitesi Fen EdebiyatFakültesi Fizik bölümde bulunan, dalga boyu ayırımlı X- ısını Floresans Spektroskopisi(XRS) cihazında incelenerek eser elementlerin degisim miktarı, Dalga Boyu AyırımlıX-Isını Floresans Spektrometresi (WDXRF) sistemi ile tespit edildi.Çalısma sonuçları SPSS 13.0 istatistik paket programı kullanılarak Varyansanalizi ve T testi ile degerlendirilmistir.Radyoterapi öncesi, 1500 cGy ve 3000 cGy'de alınan örneklerde Sodyum (Na),Silisyum (Si), Fosfor (P), Kükürt (S), Klor (Cl), Potasyum (K), Kalsiyum (Ca), Demir(Fe), Bakır (Cu) ve Çinko (Zn) degerleri % olarak belirlendi. Yapılan tek yönlü varyansanalizinde yalnızca Zn'da RT öncesi ile 1500 cGy ve 3000 cGy sonuçları arasındaistatistiksel açıdan anlamlı artıs gözlenmistir (p=0.04). Fe degerlerinde de RT öncesinegöre 1500 cGy ve 3000 cGy'de artıs gözlenmis ancak istatistiksel fark gözlenmemistir(p=0.08). Diger eser elementlerde de anlamlı degisiklikler gözlenmemistir. Çinkodegerlerinin T test analizinde RT öncesi ile 1500 cGy ve 3000 cGy degerlerikarsılastırıldıgında RT öncesine göre 1500 cGy degerlerinde fark yokken 3000 cGy'deanlamlı derecede artıs saptanmıstır ( p=0.045 ). Fe'de ise RT öncesine göre 1500 cGyde fark yokken 3000 cGy'de istatistiksel anlamlılıga dogru bir artıs gözlenmistir (p=0.076 ).Literatürde radyoterapinin neden oldugu saç dökülmesinde eser elementlerinrolünü gösteren çalısmaya rastlanamamıstır. Çalısmamızda RT sonrası yalnızca çinkodegerlerinde RT öncesine göre anlamlı artıs saptanmıstır. Demir elementinde deistatistiksel anlamlılıga yakın artıs trendi gözlenmis olup diger eser elementlerdeanlamlı bir degisiklik gözlenmemistir. Çalısma grubumuzdaki hastalara uygulananradyoterapi fraksiyonasyonu ve dozu heterojendir. Konvansiyonel fraksiyonasyonuygulanmıs fazla sayıda hasta grubuyla yapılan çalısmalar daha sonra yapılacakçalısmalara temel olusturacaktır. Aim of this study is to investigate the variations of the trace elementconcentrations whit to different doses and whether trace elements are reasons thatcontribute to hair loss or not.61 hair samples was taken from the irradiated region of metastatic or primer 21patients before and after treatment of 1500 and 3000 cGy respectively. The collectedsamples were investigated in a wave dispersive x-ray spectrometer and the variationquantity of trace elements was established in an energy dispersive x-ray spectrometerstated in Ataturk University, Faculty of Science and Art, Department of Physics.Statistical analysis was performed with a Statistical Package for the SocialSciences for Windows (SPSS 13.0, Chicago, IL, USA). All values are expressed asmeans and standard error mean (SEM). Observations were compared with UnivariateVariance Analyze and Student?s t_test and p<0.05 was considered to indicate statisticalsignificance.The preradiotherapy, 1500 cGy and 3000 cGy levels of the Sodium (Na), Silicon(Si), Phosphorus (P), Sulfur (S), Chlorine (Cl), Potassium (K), Calcium (Ca), Iron (Fe),Copper (Cu) and Zinc (Zn) are determined as percentages. 1500cGy and 3000cGy Znlevels were significantly higher than the preradiotherapy levels with univariate varianceanalyze (p=0.04). Fe levels were higher than pre radiotherapy at 1500 cGy and 3000cGy, but statistical analysis did not show any significant difference (p=0,08). Othertrace element levels were not different between preradiotherapy, levels of 1500cGy and3000cGy. A comparison with t- test analysis of Zn levels before radiotherapy and at1500 cGy and 3000 cGy was performed. There was a significant rise in the level of Znon 3000cGy (p=0.045) but no significant change was observed on 1500cGy comparedto pre radiotherapy.On the other hand, at Fe levels were higher than pre radiotherapy; a significantincrease was observed at 3000cGy (p=0,076).The literature is so poor in works dealing with the role of trace elements in hairloss due to radiotherapy that any work on this direction has not been found yet. Thisstudy has been performed to fill this gap. In this study a significant increase in Zn wasobserved after radiotherapy compared to pre radiotherapy. An increase with statisticalsignificance was observed in Fe although there is no significant difference in other traceelements. The fractionation and dose of radiotherapy performed on our study group washeterogeneous. Further studies on the roles of trace elements in a large patients group towhom conventional fractionation radiation is performed will constitute a basis to thestudies to be performed later.
