Abstract
Bu çalışmada, çekal-ligasyon-delme (CLP) yöntemiyle sepsis oluşturulan sıçanlarda imipenem, metilprednizolon ve vitamin K kullanımlarının TNF-?, IL-1ß, IL-6, hemogram, koagülasyon testleri, kan gazları ve yaşam süreleri üzerine etkilerinin araştırılması amaçlanmıştır. Her grupta 12 sıçan olmak üzere 7 grup oluşturuldu. Sadece operasyon yapılanlar Sham grubunu ve sepsis oluşturulanlar ise CLP grubunu oluşturdu. Diğer gruplardaki hayvanlara CLP yöntemi ile sepsis oluşturulduktan sonra imipenem (AB grubu), metil prednizolon (MP grubu), vitamin K (VK grubu), imipenem+metil prednizolon (AB+MP grubu) ve imipenem+vitamin K (AB+VK grubu) uygulamaları yapıldı. Her gruptan 6'şar hayvan 16. saatte sakrifiye edildi ve kan örnekleri alınarak analizler gerçekleştirildi. Kalan 6'şar hayvana 3 gün süresince ilaç uygulamalarına devam edildi ve yaşam süreleri kaydedildi. Serum TNF-?, IL-1ß ve IL-6 seviyelerinde en fazla artış CLP ve AB+VK gruplarında görülürken, AB+MP grubunda önemli artış kaydedilmedi. Sepsisli tüm gruplarda lenfopeni oluştu ve en şiddetli trombositopeni CLP ve MP gruplarında belirlendi. Sitokin seviyelerindeki artış, lenfopeni, trombositopeni, koagülasyon sürelerinde uzama, asidozis ve bikarbonat açığının hayvanlarda nötrofili, hiperoksemi ve hipokapni gibi kompenzatuvar mekanizmalar ile telafi edilmeye çalışıldığı görüldü. CLP, MP ve VK gruplarında hiç hayvan hayatta kalmazken, AB'de % 66,7, AB+MP'de % 66,7 ve AB+VK'de 33,3'ü yaşadı. AB grubunda ölüm saati 20 ± 4 ve AB+MP grubunda ise 31 ± 8'di. Sonuç olarak, sepsiste inflamasyon, immunsupresyon, koagülopati ve dolaşım yetmezliği gibi ölüme neden olan önemli pato-fizyolojik olayların şekillendiği anlaşıldı. İmipenem uygulamasının hayat kurtarıcı olduğu ve vücudun inflamatuar cevabına yönelik düşük dozda metilprednizolon ilavesinin de yaşam sürelerini uzattığı belirlendi. In this study, the effects of imipenem, methylprednisolone, and vitamin K administrations have been aimed to investigate on TNF-?, IL-1ß, IL-6, hemogram, coagulation tests, blood gases and surviving time in septic rats induced by cecal-ligation-puncture (CLP) technique. 7 groups were formed including 12 rats each. The Sham group was constituted from the operated ones only and the CLP group was constituted from sepsis induced ones. To the animals in the other groups, after inducing the sepsis by CLP technique, imipenem (AB group), metil prednizolon (MP group), vitamin K (VK group), imipenem+methylprednisolone (AB+MP group) and imipenem+vitamin K (AB+VK group) have been administrated. Six animals from each group were sacrificed on the 16th hour and the analyses were accomplished taking blood samples. Drug administrations were continued for 3 days in the remaining 6 animals and surviving times were recorded. The highest increases in serum TNF-?, IL-1ß ve IL-6 levels were observed in CLP and AB+VK groups and no important increase in the AB+MP group was recorded. Lymphopenia was formed in all sepsis groups and the most severe thrombocytopenia was detected in CLP and MP groups. The increase in cytokine levels, lymphopenia, thrombocytopenia, prolongation of cogulation times, asidosis and bicarbonate excess have been trying to be compensated by the mechanisms such as neutrophilia, hyperoxemia and hypocapnia. No animals survived in CLP, MP and VK groups while 66.7 % of the animals in AB, 66.7 % in AB+MP and 33.3 % in AB+VK groups survived. The death time was 20 ± 4 in AB and 31 ± 8 in AB+MP group. In conclusion, pathophysiologic events that cause death occur in sepsis such as inflammation, immunosupression, coagulopathy and circulatory insufficiency. It has been determined that imipenem administration is life-saving and low dose methylprednisolone adjunction prolongs surviving time.
