Abstract
Bu araştırma; primipar ve multipar gebelerde doğum ve postpartum döneme ilişkinendişelerin belirlenmesi amacıyla karşılaştırmalı tanımlayıcı olarak yapılmıştır.Araştırma Sağlık Bakanlığı Erzurum Nenehatun Kadın Doğum Hastanesi'neprenatal kontrol için başvuran gebelerden gönüllü olan 300 gebe üzerinde yapılmıştır.Veriler, 15 Mayıs-30 Aralık 2011 tarihleri arasında toplanmıştır. Verilerintoplanmasında `Kişisel Bilgi Formu` ve `Doğum ve Doğum Sonrası Döneme İlişkinEndişeler Ölçeği (DDSEÖ)` kullanılmıştır. Verilerin değerlendirilmesinde; yüzdelikdağılımlar, ortalama, t testi, tek yönlü varyans, Kruskall Wallis ve Mann-Whitney Utestleri kullanılmıştır.Primipar gebelerin DDSEÖ toplam puan ortalamasının 5.80+1.41, multiparların5.22+1.29 olduğu ve aradaki farkın istatistiksel olarak anlamlı olduğu bulunmuştur(p<0.001). Primipar ve multipar gebelerin her ikisinde de ilgili ölçek alt boyutlarındanalınan en yüksek puan ortalamasının `Doğum Sırasında Sağlık PersonelininDavranışına Yönelik Endişeler` alt boyutuna, en düşük puan ortalamasının ise `DoğumSonrası Eşinden Destek Alamama Endişesi` alt boyutuna ait olduğu saptanmıştır.Primipar gebelerin eğitim düzeyi yükseldikçe DDSEÖ puan ortalamasının azaldığıve puan ortalamaları arasındaki farkın istatistiksel olarak anlamlı olduğu belirlenmiştir(p<0.05). Primipar gebelerde eşin eğitim durumu ile eş mesleğinin de endişe düzeyinietkilediği bulunmuştur. Eşinin eğitim durumu ilköğretim düzeyinde olan ve eşi serbestolarak çalışan gebelerin DDSEÖ toplam puan ortalamasının daha yüksek olduğu vepuan ortalamaları arasındaki farkın istatistiksel olarak anlamlı olduğu belirlenmiştir(p<0.05).Multipar gebelerde eşin eğitim durumunun endişe düzeyini etkilediği ve eğitimdurumuna göre toplam puan ortalamaları arasındaki farkın istatistiksel olarak anlamlıolduğu bulunmuştur (p<0.05). Eşinin eğitimi yükseköğretim düzeyinde olan multipargebelerin endişesinin diğerlerine göre daha az olduğu belirlenmiştir.Primipar ve multipar gebelerin obstetrik özelliklerinden yalnızca planlanan doğumşekline göre DDSEÖ puan ortalamaları arasındaki farkın istatistiksel olarak anlamlıolduğu bulunmuştur (p<0.05). Normal doğum planlayan gebelerde endişe düzeyinindaha yüksek olduğu belirlenmiştir.Bu araştırma sonucunda; gebelerin endişe düzeylerinin genel olarak orta derecedeolduğu ve primipar gebelerde multiparlara göre daha yüksek olduğu saptanmıştır.Anahtar Kelimeler: Gebelik, endişe, doğum, postpartum dönem. The purpose of this comparative descriptive study is to determine anxietiesrelated to delivery and postpartum period in primiparous and multiparous pregnantwomen.The study was conducted on 300 voluntary pregnant women who applied forprenatal control to Erzurum Nenehatun Maternity Hospital of Ministry of Health. Thedata was collected between May 15, 2011 and December 30, 2011. `?PersonalInformation Form?? and `?Scale of Fears of Childbirth and Postpartum Period(SFCPP)?? were used to collect data. Percentage distributions, mean, t test, one-wayvariance analysis, Kruskall Wallis test and Mann-Whitney U test were used to evaluatedata.It was determined that total mean score of SFCPP of primiparous pregnant womenwas 5.80+1.41 and multiparous was 5.22+1.29 and there was statistically significantdifference (p<0.001). The highest mean score obtained by primiparous and multiparouspregnant women from sub-scales of the related scale was of the sub-scale `anxietiestowards behaviors of healthcare personnel during delivery??; and the lowest mean scorewas of the sub-scale `anxiety of not receiving any support from her husband duringpostpartum period`.It was determined that with increase in educational level of primiparous pregnantstheir SFCPP mean score decreased; and there was statistically significant differencebetween mean scores (p<0.05). Also husband's educational status and occupation had aneffect on anxiety level of primiparous pregnant women. It was found out that pregnantwomen whose husbands were primary-school graduates and self-employed had highertotal mean score of SFCPP and there was statistically significant difference betweenmean scores (p<0.05).It was identified that the husband's educational status had an effect on anxiety levelof multiparous pregnant women and there was statistically significant differencebetween mean scores in terms of educational status (p<0.05). It was determined thatanxiety of multipar pregnant whose husband's education was at the level of highereducation lower than the others.It was determined that the difference between mean scores of SFCPP ofprimiparous and multiparous pregnant women was statistically significant in terms oftheir obstetrical properties except for only according to planned mode of delivery(p<0.05). The level of anxiety was higher in pregnant women who were planningnormal delivery.In consequence of this study, it was determined that level of anxiety in pregnantwomen was generally medium; and higher in primiparous pregnant women incomparison to others.Keywords: Pregnancy, anxiety, delivery, postpartum period.
