ST-elevasyonlu miyokart infarktüslerde QRS fragmantasyonu ile nötrofil/lenfosit ilişkisinin incelenmesi

Abstract
Amaç: Akut koroner sendrom görülme sıklığı yüksek olan olumsuz sonlanımlara neden olabilecek ölümcül bir hastalıktır. Akut koroner sendromlu hastalardan ST elevasyonlu miyokart infarktüslerde PKG ile reperfüzyon uygulanan STEMİ hastalarında basit, ucuz ve kullanılabilir tetkikler ile prognoz tayini yapabilmek için kısa ve orta süreli klinik etkisini değerlendirmek için bu çalışma planlandı. Çalışmamızdaki amaç ST elevasyonlu miyokard infarktüslerde QRS fragmantasyonu ile nötrofil/lenfosit ilişkisinin incelenmesidir. Metod: Çalışmamız 500 hastalık popülasyon ile yapıldı. Çalışma retrospektif olarak planlandı. Veriler hastane otomasyon sisteminden ve arşiv taramalarından elde edildi. STEMİ' li hastaların oluşturduğu popülasyon FQRS (+) ve FQRS (-) olarak iki gruba ayrıldı. MAKO, hastane içi ölüm, ölümcül olmayan Mİ ,stent trombozu, yavaş akım, no- reflow, hastane içi VT, hastane içi VF, kardiyojenik şok, kardiyopulmoner arrest, gibi olayların bu gruplarda hangi sıklıkta olduğu ve NLO düzeylerine gruplar arası bakıldı. Bulgular: Çalışma toplam 500 hastayla yapılmıştır. Hasta popülasyonu Fqrs olup olmamasına bağlı olarak iki gruba ayrılmıştır. Grup 1' de (FQRS -) 207 hasta mevcut iken grup 2' de (FQRS+) 293 hasta bulunmaktaydı. Grup 1'in yaş ortalaması 61,1±12,1 iken grup 2 'nin yaş ortalaması 66,7 ±10,6 olup gruplar arası yaş açısından anlamlı fark gözlendi (p<0,001). Risk skorlamalarından TİMİ ve GRACE risk skorları grup 2'de daha yüksek görüldü (p=0,002, p=0,008, sırasıyla). WBC ve NLO grup 2' de grup 1' den anlamlı şekilde daha yüksek gözlendi (p<0,001). MAKO, hastane içi ölüm, hastane içi VT oranı grup 2 'de grup 1 'den daha yüksek oranda gözlendi (p<0,001). Lojistik regresyon analizinde TİMİ skor, GRACE skor, NLR, ve HT, STEMİ' nin bağımsız yordayıcıları olarak belirlendi (rölatif risk (RR):2,885; %95 güven aralığı (GA); 1,504-2,362, p<0,001; RR:1,820; %95 GA; 1,006-2,333, p=0,004; RR:1,478; %95 GA; 0,970-1,831, p=0,045; RR:1,312; %95 GA; 0,807-1,921, p=0,050). Sonuç: Çalışmamız STEMİ' li hastalarda FQRS varlığının ve NLO' nın prognoz tayini ve hastane içi klinik gözlemde anlamlı bir role sahip olduğunu göstermiştir.Anahtar kelimeler: ST elevasyonlu miyokard infarktüsü, Fragmente QRS, nötrofil/lenfosit oranı Objective: Acute coronary syndrome is a fatal disease that can cause negative outcomes with high incidence. This study was planned to evaluate the short-term clinical effect of STEMI patients with acute coronary syndrome to determine prognosis in STEMI patients undergoing reperfusion with PKG in myocardial infarction with ST elevation. The aim of our study is to investigate the neutrophil / lymphocyte relationship with QRS fragmentation in ST elevation myocardial infarction. Method: Our study was performed with a 500 participiants. The study was planned retrospectively. The data were obtained from the hospital automation system and archive scans. The population divided into two groups as FQRS (+) and FQRS (-). The frequency of events such as MAKO, in-hospital death, non-lethal MI, stent thrombosis, slow flow, no-reflow, in-hospital VT, in-hospital VF, cardiogenic shock, cardiopulmonary arrest, and NLO levels were analyzed between the groups. Results: The study included 500 patients. The patient population was divided into two groups depending on whether it was fQRS or not. There were 207 patients in group 1 (FQRS -), and 293 patients in group 2 (FQRS +). The average age of group 1 was 61.1 ± 12.1, and the average age of group 2 was 66.7 ± 10.6. There was a significant difference was observed between the groups in terms of age (p <0.001). TIMI and GRACE risk scores were higher in group 2 than group 1(p = 0.002, p = 0.008, respectively). WBC and NLO were significantly higher in group 2 than group 1 (p <0.001). The ratio of MAKO, in-hospital death, in-hospital VT was higher in group 2 than group 1 (p <0.001). In logistic regression analysis, TIMI score, GRACE score, NLR, and HT were determined as independent predictors of STEMI (relative risk (RR): 2.885; 95% confidence interval (CI); 1.504-2.362, p<0.001; RR: 1.820; 95% CI; 1.006-2.333, p=0.004; RR: 1.478; 95% CI; 0.970-1.831, p=0.045; RR: 1.312; %95 CI; 0.807 - 1.921, p=0.050). Conclusion: Our study showed that the presence of FQRS and NLO in patients with STEMI has a significant role in prognosis and in-hospital clinical observation.
