Abstract
ÖZET İşitme Engelli Okul Çocuklarında Genetik Epidemiyolojik Bir Çalışma Dil gelişimi öncesinde oluşan ağır işitme kaybı sıklığı gelişmiş ülkelerde 0,4- 1,1/1.000 olarak bildirilmiştir. Bu işitme kayıplarmm % 50-60'ı genetik nedenli işitme kayıplarıdır. Genetik işitme kayıplarının % 60-80'ninden otozomal resesif kalıtım sorumludur. Türkiye'de yenidoğan işitme taramalarının ilk sonuçlarına göre doğuştan işitme kaybının sıklığı 1.000 canlı doğumda 2'dir. Türkiye'de işitme kaybının genetik epidemiyolojisini değerlendirmek amacıyla 31 işime engelliler okulundan toplanan 2.169 aile verisi ile ayrılma çözümlemesi yapılmıştır. Aileler anne-babanm işitme durumuna göre gruplandınldıktan sonra sporadik olguların, resesif ve dominant kalıtılan olguların oranlan tahmin edilmiştir. Olgularımızın % 23,2'sinde genetik dışı nedenler (sporadik fenokopiler) ve % 76,8'inde genetik nedenli işitme kaybı saptanmıştır. Genetik olguların % 93,4'ünden resesif ve % 6,6'sından da dominant kalıtım sorumludur. Genetik işitme kayıplarmm ve otozomal resesif kalıtımın yüksek oranda görülmesi sonuçlarımızın batı toplumunda yapılmış çalışmalardan farklı olduğunu göstermektedir. Çalışmamızın sonuçlan Türkiye'de bundan sonra yapılacak çalışmalarda ve işitme engellilere verilecek genetik danışmada kullanılmalıdır. Anahtar kelimeler: Anket, ayrılma çözümlemesi, epidemiyoloji, genetik çözümleme, işitme kaybı. 39 SUMMARY A Genetic Epidemiological Study in Students of the Schools for Hearing Impaired The prevalence of prelingual profound deafness has been reported to range from 0.4 to 1.1 in 1,000 births in the developed countries. The proportion of genetic deafness in all hearing loss etiologies is 50-60%. Autosomal resessive inheritance is responsible for about 60-80% of genetic hearing loss. Preliminary data of newborn hearing screening in Turkey have shown that the incidence of congenital deafness is 2 in 1,000 births. In order to evaluate the genetic epidemiology of deafness in Turkey, we analyzed data of 2,169 families from 31 schools for children with hearing loss. Classical single major locus segregation analysis was performed after families were grouped according to hearing status of the parents. Proportions of sporadic, resessive and dominant causes of deafness were later estimated in the Turkish families. Sporadic phenocopies and genetic causes were found in 23.2% and 76.8%, respectively. Autosomal resesive and autosomal dominant inheritance were responsible in 93.4% and 6.6% in all children with genetic hearing loss, respectively. Proportions of genetic etiology and autosomal recessive inheritance were higher in Turkish people with hearing loss when compared with those reported in western populations. Results of this study should be used in future studies regarding deafness and during counseling of families with hearing impairment.
