Abstract
ÖZET İkinci G., Nikotinin Biyoadezif Dozaj Şekli Üzerinde Çalışmalar, Hacettepe Üniversitesi Sağlık Bilimleri Enstitüsü Farmasötik Teknoloji Prgramı Doktora Tezi, Ankara, 2003. Nikotin hidrojen tartaratın (NHT) sığır bukal mukozasından permeasyonu üç farklı pH'da (3.6, 7.2 ve 8.6) incelenmiştir. Nikotinin sığır bukal mukozasından geçişinin pH'nın artmasıyla arttığı tespit edilmiştir. pH artırıcı olarak seçilen magnezyum hidroksit, alüminyum hidroksit ve kalsiyum karbonatın nikotin permeasyonu üzerine etkisi incelenmiştir. Donör kışıma magnezyum hidroksit ilavesi ile NHT permeasyonunda önemli artış görülmüşken (p<0.05) diğer iki madde ile (alüminyum hidroksit ve kalsiyum karbonat) permeasyon üzerine önemli bir etki göstermemiştir. NHT' ın seçilen pH artırıcılara adsorpsiyonu % 10' dan az bulunmuştur. Bukal adezif tablet formülasyonları, farklı oranda Carbopol 974 (CP), hidroksipropil selüloz (HPC), hidroksipropil metil selüloz (HPMC), sodyum aljinat (NaAlg) ve kitosan (Protosan 212) kullanılarak hazırlanmıştır. Tabletlerden in vitro ilaç salımı ve biyoadezif özelliklerinin polimer çeşidi ve oranı ile değiştiği bulunmuştur. CP:Protasan 212'nin 20:80 oranında kullanıldığı bukal tabletin in vivo performansı 8 sağlıklı gönüllüde incelenerek, piyasada bulunan bir transdermal yama (Nicotinell® TTS 20) ile karşılaştırılmıştır. Transdermal yama ve bukal tablet için Cmax değerleri 12,4±7,1 ve 11,7±6,2 ng/mL; tmax değerleri 11,5±5,10 ve 2,88±0,83 saat; AUC değerleri 204,1±124,9 ve 59,3±20,5 ng/mL.saat bulunmuştur. Bukal tabletle hiçbir denekte mukozada iritasyon olmazken, transdermal yama uygulama ile deneklerden birinde deride şiddetli reaksiyon gelişmiştir. Formülasyonu geliştirilen bukal tablet ile, piyasada bulunan transdermal yama ile elde edilen plazma nikotin düzeyine daha kısa sürede ulaşıldığından, akut nikotin ihtiyacının giderilmesi için uygun bir sistem olduğu düşünülmektedir. Buccal permeation of nicotine hydrogen tartrate (NHT) was studied using bovine buccal mucosa at pH 3.6, 7.2 and 8.6. Permeation of NHT across bovine buccal mucosa was increased with the increasing pH. The effect of pH increasing additives (magnesium hydroxide, aluminium hydroxide and calcium carbonate) on the permeation of nicotine was also investigated. Addition of magnesium hydroxide into the donor side increased the permeation of NHT (p<0.05) whereas addition of aluminium hydroxide and calcium carbonate had no significant effect. Adsorption of NHT on the additives was found to be less than 10 %. Buccal adhesive NHT tablet formulations were developed using Carbopol 974 (CP), hydroxypropyl cellulose (HPC), hydroxypropyl methylcellulose (HPMC), sodium alginate (NaAlg) and chitosan (Protosan 212) at different ratios. In vitro drug release characteristic and bioadhesion of buccal tablets were found to be influenced by the changing type and ratio of polymers. In vivo performance of the buccal tablet containing CP:Protasan 212 at the ratio of 20:80 was evaluated in 8 healthy volunteers in comparison to a commercially available transdermal nicotine patch (Nicotinell® TTS 20). Cmax values for transdermal patch and buccal tablets were 12,4±7,1 and 1 1,7±6,2 ng/mL, and tmax values were 11,5±5,10 and 2,88±0,83 h, AUC values were 204,1±124,9 and 59,3±20,5 ng/mL.h, respectively. No buccal mucosal irritation was observed with the tablets whereas skin reaction occurred with the transdermal patch only in one volunteer. As the same plasma level was obtained with the developed buccal tablet formulation in a shorter period of time when compared to the transdermal patch, the buccal tablet seems to be a suitable system for acute nicotine replacement.
