Abstract
ÖZET Yabancı, N., Okul çağı çocuklarda büyüme ve obezite durumunun saptanması, etkileyen etmenlerin değerlendirilmesi, Hacettepe Üniversitesi Sağlık Bilimleri Enstitüsü Beslenme ve Diyetetik Programı, Doktora Tezi, Ankara, 2004. Bu çalışma, Ankara ilinde okul çağı çocuklarda yürütülmüş kesitsel bir çalışmadır. Çalışmanın amaçlan; okul çocuklarında büyüme durumu ve obezite sıklığı ile büyüme ve obeziteye etki eden etmenleri saptamak, obez olan çocuklarda koroner kalp hastalığı risk etmenlerini belirlemektir. Birinci aşamada 7-14 yaş arası toplam 1042 erkek ve 1009 kız çocuğun antropometrik (vücut ağırlığı, boy uzunluğu, üst orta kol çevresi,bel ve kalça çevresi, triseps deri kıvrım kalınlığı-DKK) ölçümleri alınmıştır. Çocukların vücut ağırlıkları ile boy uzunluklarının yaşla birlikte doğrusal artış gösterdiği saptanmıştır. Çocukların toplam %2.6'sımn yaşa göre vücut ağırlığı, %8.9'unun yaşa göre boy uzunluğu 5. persentilin altındadır. Yaşa göre beden kitle indeksi (BKİ), >85.-<95. persentil (hafif obez) olan erkek ve kızların sıklığı sırasıyla %16.1 ve %12.3, >95. persentil (obez) olanların sıklığı ise %8.5 ve %5.3'tür. ikinci aşamada, birinci aşamadan gönüllü olarak seçilen 11-14 yaş arası 211 erkek, 258 kız çocuk üzerinde yürütülmüştür. Bu aşamada, diğer aşamaya ek olarak bazı antropometrik (biseps, subskapular ve suprailiak DKK, kulaç uzunluğu ve baldır çevresi) ölçümler yapılmış, çocukların besin tüketimi, fiziksel aktivite düzeyini ve bazı özelliklerini saptamak için soru kağıdı uygulanmıştır. Çocukların antropometrik ölçümleri ile anne-babalarının vücut ağırlığı, boy uzunluğu ve BKİ'leri arasında anlamlı ilişkiler saptanmıştır (p<0.01). Çocukların %94.7'si anne sütü almıştır. Çocukların anne sütü alma durumları, ek besine başlama zamanı ve doğum agırlıkları, şu andaki antropometrik ölçümleri ile ilişkili bulunmuştur (p<0.05). Çocuklarda enerjinin yağdan gelen yüzdesi, kızlarda erkeklere göre daha fazla olup, bu farklılık 11, 12 ve 13 yaşta istatistiksel olarak önemlidir (p<0.05). Erkekler, kızlara kıyasla daha fazla karbonhidrat tüketirken, saf karbonhidrat olan şekerin enerji yüzdesi kızlarda erkeklere göre daha fazladır. Erkeklerin %22.3'ü, kızların ise %14.7'si enerjiyi yetersiz almaktadır. Vitamin B6, folat ve vitamin B12 alımları erkek çocukların sırasıyla %5.7, %9.5 ve % 14. 2' sinde, kızların ise %4.3, %11.2 ve %18.2'sinde yetersiz düzeydedir. Kalsiyum erkeklerde %42.7, kızlarda %41.9, demir erkeklerde %3.3, kızlarda %7.4 oranında yetersiz alınmaktadır. Demir alımı kızlarda daha yetersizdir (%2=7.12, p<0.05). Hafta içinde erkeklerin %74.4' ü sedanter, %12.8' i hafif aktif, %10.0' u aktif, %2.8' i de çok aktif bulunmuştur. Çocuklar genelde sedanter olup, hafta sonları, hafta içine göre daha aktiftirler. Çalışmanın üçüncü aşamasına, obez olan ve olmayan 20 erkek ve 20 kız çocuk alınmıştır. Obez olan çocukların açlık kan şekeri, VLDL-kolesterol düzeyleri ve total kolesterol/HDL-kolesterol oranı obez olmayanlara göre yüksek bulunmuştur (p<0.05). Obez çocuklarda obez olmayanlara göre, kan homosistein düzeyi yüksek (p<0.001), folat (p<0.05) ve vitamin Bu (p<0.01) düzeyleri düşüktür. Obez çocukların %15.0'inde homosistein düzeyi 15.0 umol/mL'nin üzerindedir ve hiperhomosisteinemi olarak tanımlanabilir. Obez olan ve olmayan çocukların hiçbirinde serum folat ve vitamin B]2 düzeylerine göre yetersizlik saptanmazken, obez çocukların %25.0'inde, obez olmayanlarında %75.0' inde serum folat düzeyi 6.0 ng/mL ve üzerindedir (x2=10.0, p<0.005). Obez çocuklarda koroner kalp hastalığı riski yüksektir. Homosistein düzeyi ile diyetle alman folat (r=-0.35, p<0.05), vitamin Bö (r=-0.18) ve vitamin Bi2 (r=-0.17) arasında negatif ilişki saptanmıştır. Ailede şişman birey olması, çocuğun doğum ağırlığı, anne sütü alma durumu, fast food tüketmesi gibi etmenler obezite oluşumunda etkilidir. Çocuk ve adolesanlarda büyüme ve obezite durumunun değerlendirilmesi için ulusal bir yöntem belirlenmelidir. Okul çağı çocuklarda büyüme ve obezitenin izlenmesi, sağlıklı beslenmeye önem verilmesi, okul sağlığı programlarının geliştirilmesi gelecek için halk sağlığına yapılan bir yatırım olacaktır. Hacettepe University, Institute of Health Sciences PhD Thesis in Nutrition and Dietetics, Ankara, 2004. This cross-sectional study was conducted on school-age children in Ankara province. The aims of the study were to determine the growth and obesity of school-children, to evaluate the affecting factors, and to find out the risk factors of coronary heart disease in obese children. In the first stage body weight and height, mid-upper arm, waist and hip circumferences, triceps skinfold thickness of 1042 boys and 1009 girls were measured. A linear increase in body weight and height with age was determined. Body weight of 26% and height of 8.9% children in terms of age were below the 5 percentile. Prevalences of overweight (>85.-<95. percentiles) and obesity (>95. percentile) for boys and girls were 16.1 and 12.3% and 8.5 and 5.3%, respectively. For the second stage, volunteers of 211 boys and 258 girls, aged 11-14 years were selected from the first stage sample group. At this stage some anthropometric measurements (such as biseps, subscapular and suprailiac skinfold thicknesses, arm-span and calf circumference) were added in addition to the first stage and a questionnaire was applied to determine the food consumption and physical activity level and other characteristics of the children. Significant relations were determined between the anthropometric measurements of children and parents' body weights, heights and BMIs (p<0.01). A total of 94.7% of children were breastfed. Significant correlations were found between the status of breastfeeding, age beginning to complementary feeding, body weight at birth and the current anthropometric measurements of children (p<0.05). Percentage of energy from fats was higher in girls than boys. This difference was found to be statistically significant at 11, 12, and 13 years of age (p<0.05). Boys consumed more carbohydrates than girls while girls had more percentage of energy from sugar, which is simple carbohydrate, than boys. Out of total, 22.3% of the boys and 14.7% of the girls had inadequate intakes of energy. Inadequate intakes of vitamin B6, folate and vitamin Bi2 were determined in 5.7, 9.5 and 14.2% of boys and 4.3, 11.2 and 18.2% of girls, respectively. Calcium intakes were inadequate in 42.7% of boys and 41.9% of girls while iron was inadequate in 3.3% of boys and 7.4% of girls. Intake of iron was highly inadequate in girls (x2=7.12, p<0.05). Within the week; 74.4% of the boys were found to be sedentary, 12.8% slightly active, 10.0% active and 2.8% very active. In general, the children were sedentary and more active during the week-ends than within the week. Obese and non-obese 20 boys and 20 girls were included to the third stage of the study. Obese children had higher fasting blood glucose and VLDL-cholesterol levels and total cholesterol/HDL-cholesterol ratio than non-obese children (p<0.05). Obese children had higher blood homocysteine level than the non-obese (p<0.001), while they had lower levels of folate (p<0.05) and vitamin B!2 (p<0.01). Obese children had blood homocysteine level over 15.0 umol/mL and could be defined as hyperhomocysteinemia. While no insufficiency were determin'ed on none of the obese and non-obese children in terms of serum folate and vitamin B!2 levels; 25% of the obese children and 75.0% of non-obese children had serum folate levels of 6.0 ng/mL and over (x2=10.0, p<0.005). In obese children, the risk of coronary heart disease was high. Negative correlations were found between homocysteine level and dietary folate (r=-0.35, p<0.05), vitamin Bğ (r=-0.18) and vitamin B12 (r=-0.17) intakes. Obese individuals in the family, body weight of the child at birth, breastfeeding and fast-food consumption and such factors are effective in the development of obesity. A national technique should be determined in order to evaluate the growth and obesity in children and adolescents. Monitoring of growth and obesity at school age children, paying particular attention to healthy nutrition and developing school health programs will be an investment in public health for the future.
