Abstract
IV ÖZET Bu çalışmada 400 hastadan alınan vajinal ve servikal örnekler Papanicolaou boyama yöntemi ile boyanıp, sitolojik olarak incelenmiştir. İncelenen bu 400 örneğin 20'sinde (%5)Trichomonas vaginalis 'in trofozoit formları saptanmıştır. T.vaginalis saptanmış yaymalarda parazit, oval, yuvarlak ve armut şekillerde ve de kamçısız olarak görülmüştür. Bazı T.vaginalis 'lerin epitel hücreleri ile bağlantı kurarak onlara tutunduğu ve parazit hücre zarının, tutunduğu epitel hücre zarının şekline uyacak biçimde değişikliğe uğradığı tespit edilmiştir. Özellikle epitel hücrelerine tutunan T.vaginalis 'lerin ameboid şekle dönüştüğü ve bir epitel hücresinin çevresinde birden fazla parazitin bulunduğu saptanmıştır. İncelemiş olduğumuz yaymalarda T.vaginalis'ler'in epitel hücrelerinin yanısıra eritrosit, Polimorfnükleer lökosit (PMNL) gibi hücrelere de tutunduğu gözlenmiştir. Parazitlerin bu yapılara tutunmaları esnasında da şeklinin tutunduğu yapıya göre değişikliğe uğradığı ve içeri doğru bir çöküntü oluşturduğu tespit edilmiştir. Ayrıca, T.vaginalis saptamış olduğumuz yaymalarda karyoreksiz, karyopiknozis, binükleasyon ve çekirdeğin benign sınırlarda irileşmesi gibi çekirdeğe ait değişikliklerin yanısıra perinükleer hale, pseudoeosinofıli ve boyanmada bozukluk gibi sitoplazmik değişiklikler de gözlenmiştir. İstatistiksel değerlendirmeler için Ki Kare ve Fisher Testleri kullanılmıştır. Bu testlerle yapılan değerlendirmelerde, servikal ve vajinal yaymalarda T.vaginalis bulunması ile epitel hücrelerinin çekirdek ve sitoplazmasına ait hücresel değişiklikler arasında ve klinik şikayetlerle T.vaginalis 'in görülmesi arasında anlamlı bir ilişki olduğu saptanmıştır. V ABSTRACT In this study cervical and vaginal samples taken from 400 patients were examined cytologically by using the Papanicolaou method. The trophozoite form of Trichomonas vaginalis was determined in 20 of the 400 smears. The parasites in the smears had a typical morphology of T. vaginalis, e.g. rounded, spherical and bulbous in shape, having no flagellum. It was observed that T. vaginalis were attached to epitelial cells through interdigitations, hence fitting their shapes to the topology of the membranes of the cells. It was found that the parasites attached to the epitelial cells took amoeboid forms and there were more than one T.vaginalis per cell. It was observed that in addition to epitelial cells, T.vaginalis were also attached to Polimorphonuclear leukocytes (PMNL) and erythrocytes. The parasites were found to change their shapes according to the attachment site, where they caused invaginations. Besides, smears with T.vaginalis contained cells exhibiting karyorhexis, karyopyknosis, binucleation, swellling to the normal size. Cells also showed cytoplasmic changes such as existence of perinuclear halo, pseudoeosinophilia, and abnormal staining patterns due to the interruption of the cell's dynamic shape. Statistical inference was made with Khi2 test and Fisher's exact test. Evaluations point to a significant correlation between the existence of T.vaginalis and the epitelial cell alterations and also between clinical symptomps and T.vaginalis existence.
