Abstract
Özet ÖZET Bu çalışmanın amacı, yüzeysel mesane kanserleri için TUR (transurethral resection) sonrası tedavi amaçlı alternatif bir yöntem geliştirmektir. Geleneksel tedavilerde farmakoterapötikler TUR sonrası periyodik olarak (6 haftadan 36 haftaya kadar, her hafta) intravesikal olarak mesane içine boşaltılır. Fakat bu tedavilerin çok büyük problemleri vardır (uygulama sırasında hastaya verilen rahatsızlık, uygun dozun verilmesindeki zorluk, kullanılmayan aktif ajanın kaybı gibi). Bu nedenle bu çalışmada alternatif bir metod geliştirilmiştir. Bunun için, in vivo çalışmalarda yerleştirme işlemini kolaylaştırması amacıyla, bir mukoadesif polimerden {kitosan) silindirik geometride farmakoterapötik ajanı (Mitomicin-C) serbestleştirici sistem hazırlandı. Kitosan taşıyıcılar çözücü buharlaştırma yöntemi ile çapraz bağlanarak hazırlanmıştır. Kitosan taşıyıcıların hazırlanmasında farklı molekül ağırlıklı kitosan polimeri, farklı miktalarda çapraz bağlayıcı (gluteraldehit) ve farmakoterapötiğin farklı miktarları kullanılmıştır. Böylece kitosan taşıyıcının mesane iç duvarına yapışması ve farmakoterapötik ajanın optimum serbestleşmesi sağlanmaya çalışılmıştır. Diğer taraftan kitosanın jel yapısından dolayı polimerin şişme davranışları sulu ortamda gravimetrik olarak incelenmiştir. Şişme oranları etkili parametrelere (çöktürme ortamındaki çapraz bağlayıcı konsantrasyonu, kitosanın molekül ağırlığı ve NaOJH konsantrasyonu gibi) bağlı olarak % 190- 210 aralığında değişmektedir. Çalışmanın son aşamasında, hazırlanan kitosan taşıyıcılar in vivo çalışması olarak Venezuela tavşanlarına yerleştirilmiştir. Daha sonra, bu taşıyıcıların mesane iç duvanna yapıştığından emin olmak için periyodik olarak ultrasonografi ile izlenmiştir. Alınan ultrasonogram fotoğraflardan kitosan silindirlerin mesane iç duvarına yapıştığı ve fotoğrafların alındığı süre zarfında o bölgede sabit kaldığı gözlenmiştir. Ahstrant ABSTRACT The aim of this study is to develop an alternative post operative (i.e. transurethral resection, TUR) pharmacotherapy for superficial bladder cancer. In conventional therapies the pharmacotherapeutics are infused intravesically into the bladder after Tur periodically (i.e., upto 6 to 36 weeks, each week). But these therapies are having very big problems (i.e., disturbancv to patients, adjustment of the suitable dosage, loss of active agents without using etc.). Therefore an alternative method was developed in this study. For this purpose the pharmacotherapeutic agent (i.e.. Mitomvcin-C) delivery system was prepared by using a mucoadhesive polymer (i.e., chitosan) in the form of cylindrical geometry to facilitate the insertion of the carrier for in vivo studies. The chitosan carriers were prepared bv crosslinking during the solvent evaporation tecnigue. In the preparation of the chitosan carriers the chitosan polymers with different molecular weights, different amounts of cross-linker (i.e., glutaraldehvde) and different amounts of pharmacotherapeutic agent were used to obtain desicecLattachment yo inner wall of bladder and optimum release rate of the agent. On the ather hand because of the oelous structure of the chitosan, the swelling behaviour of the polymer was evaluated bv gravimetric determinations in agueous media periodically. The swelling ratios were changed in the range of 90-210 % based on the effective parameters (i.e.. cross-linker concentration in precipitation medium, chitosan molecular weight and NaOH concentration). In the last part of this study; prepared chitosan carriers were inserted into the bladder of Venezuela rabbits as in vivo studies. Afterwards, those carriers were followed by ultrasonography periodically to be sure for the fixation of carriers to the inner wall of bladder.
