Abstract
NİFEDİPİNİN PREEKLAMPTİK ve NORMAL PLASENTA DAMARLARI ÜZERİNE ETKİSİNİN KARŞILAŞTIRILMASI Zeynep Çabuk ÖZ Bu çalışmada, nifedipinin preeklamptik ve normal plasenta damarları üzerine etkileri çalışılmıştır. Klinik uygulamalarda hipertansiyon ve preeklampsi tedavisinde kullanılan nifedipinin, maternal damarlar aracılığıyla bebeğe geçişinin araştırılması amacıyla plasental perfüzyon sistemi kullanılmıştır. Normal ve preeklamptik kadınların, normal doğum ya da elektif sezeryan sonrası alınan plasentaları, plasental perfüzyon sistemine yerleştirilmiş ve maternal taraftan verilen maddelerin fetal tarafa geçişine bakılmıştır. Fetal taraftan elde edilen perfüzatta bulunan nifedipin ve antipirinin varlığı HPLC analizi ile incelenmiştir. Nifedipinin normal ve preeklamptik plasenta damarları üzerine etkisinin araştırılması amacıyla damar içi basıncı ölçülmüştür. Yapılan deneyler sonucunda hem normal hem de preeklamptik plasentada maternal tarafta bulunan nifedipinin fetal tarafa perfüze olduğu gözlenmiştir. Nifedipinin perfüzyon oranı normal plasentalarda daha fazladır. Damar içi basınç deneyleri, nifedipinin fetal plasenta damar basıncını hem normal hem de preeklamptik plasentada aynı oranda düşürdüğünü göstermiştir. Placental perfusion system was used in order to investigate the transfer of nifedipine -which is used in hypertension and preeclampsia treatment in clinical applications- to fetus via maternal vessels. Placentas obtained from normal and preeclamptic women after vaginal or elective section deliveries, were placed into the placental perfusion system and transplacental transfer of the materials added to the maternal solution was estimated. Existence of nifedipine and antipyrine in the fetal perfusate was estimated by HPLC analysis. Intra vessel pressure was measured in order to determine the effect of nifedipine on normal and preeclamptic placental vessels. As a result, nifedipine in the maternal side was found to be perfused to the fetal side in both normal and preeclamptic placentas. The ratio of perfused nifedipine is higher in normal placentas. Intra vessel pressure experiments showed that nifedipine lowered the fetal placental vessel pressure in the same ratio in both normal and preeclamptic placentas.
