Abstract
İNSAN SERVİKO-VAJİNAL YAYMALARINDA SİTOLOJİK VE MİKROBİYOLOJİKYÖNTEMLERLE ACTINOMYCES'İN ARAŞTIRILMASIDilek KayaÖZBu çalışmada 200 hastaya ait serviko-vajinal örnek hem sitolojik hem demikrobiyolojik olarak Actinomyces açısından incelenmiştir. İncelenen bu 200 örneğinaltısına (% 3) sitolojik, yedisine (% 3.5) mikrobiyolojik ve birine (% 0.5) de hemsitolojik hem de mikrobiyolojik olarak Actinomyces tanısı verilmiştir. Yapılanistatistiksel incelemelerde Actinomyces tanısı açısından bu iki yöntem arasındaanlamlı bir ilişki olmadığı bulunmuştur (p>0.05).Sitolojik inceleme için alınan serviko-vajinal örnekler Papanicolaou boyamayöntemine göre boyanıp sitolojik bulgular açısından değerlendirilmiştir. Actinomycessaptanmış yaymalarda, bazı Actinomyces'lerin yüzeyel epitel hücreleri ile bağlantıkurarak onlara tutunduğu, bu tutunma noktalarında epitel hücresinin zarındakalınlaşmalar meydana geldiği saptanmıştır. İncelediğimiz yaymalardaActinomyces'in epitel hücreleri yanı sıra eritrosit ve polimorfonükleer lökositlere detutunduğu gözlenmiştir. Ayrıca Actinomyces saptadığımız yaymalarda bol miktardaeozinofil lökosit görülmesi dikkatimizi çekmiştir. Bu bulgu istatistiksel açıdan daanlamlı bulunmuştur (p<0.05). Yaymalarda Actinomyces yanı sıra Trichomonasvaginalis, Candida ve Leptothrix gibi diğer bazı patojen mikroorganizmalar dagörülmüştür. Diğer bir bulgu olarak, Actinomyces varlığı ile Rahim İçi Araç (RİA)kullanımı arasında anlamlı bir ilişki olduğu tespit edilmiştir (p<0.05).Mikrobiyolojik inceleme için de anaerobik transport sistemi ile alınan serviko-vajinalörneklerin önce direkt yaymaları yapılıp bu yaymalar gram ile boyanarak mikroskobikolarak incelenmiştir. Daha sonra anaerop organizmaların üremesi için kullanılançeşitli besiyerlerine ekilen kültür örnekleri inkübe edilerek Actinomyces varlığıaçısından incelenmiştir. Amongthese 200 samples, cytologically six (3 %), microbiologically seven (3.5 %), bothcytologically and microbiologically one (0.5 %) of them were diagnosed as havingActinomyces. In statistical analyses, it was found that there was not a significantassociation between these methods in respect of diagnosing Actinomyces (p>0.05).Cervico-vaginal smears taken for cytological examination were stained byPapanicolaou method and investigated in terms of cytological findings. In smearswith Actinomyces, it was detected that some of Actinomyces adhere to superficialepithelial cells and membrane thicknesses on the epithelial cells were observed inthese attachment regions. In these smears, it was observed that Actinomyces adhereto polymorphonuclear leucocytes (PMNL) and erytrocytes besides epithelial cells.Moreover, it was detected abundant eosinophil leucocytes in smears withActinomyces which was considered statistically reasonable (p<0.05). In addition toActinomyces, other pathogen microorganisms such as Trichomonas vaginalis,Candida and Leptothrix were observed in the smears. The other finding is that therewas a statistically significant correlation between the presence of Actinomyces andthe usage of intrauterine device (IUD) (p<0.05).For microbiological examination, initially, cervico-vaginal samples taken by anaerobictransport system were spread out, stained by Gram method and examinedmicroscopically. Then samples were inoculated several media and incubatedanaerobically in respect of detecting subsistence of Actinomyces. Media supposedto be Actinomyces were examined with Crystal identification system and sevenpatients were diagnosed as Actinomyces-positive.
