Abstract
BAKTERİYEL VAJİNOZ'UN İNSAN SERVİKOVAJİNAL ÖRNEKLERİNDESİTOLOJİK VE MİKROBİYOLOJİK OLARAK ARAŞTIRILMASIAYŞEGÜL KÜÇÜKÖZÇalışmamızda, bakteriyel vajinozun (BV) araştırılması için 200 hastadan alınanservikovajinal örnekler sitolojik ve mikrobiyolojik yöntemle incelenmiştir. Sitolojikyöntemle 200 hastanın 21'inde (%10.5) ipucu hücresi ve BV-ilişkili serbest kok vekokobasillerin bulunduğu ancak laktobasil ve polimorfonükleer lökositlerin (PMNL)görülmediği veya çok az olduğu belirlenmiştir. Bu bulgulara sahip olan hastalar BVpozitif olarak değerlendirilmiştir. Ayrıca, mikrobiyolojik yöntemle bu 21 hastanın13'ünde Gardnerella vaginalis bulunduğu belirlenmiştir. BV pozitif olduğubelirlenen simirlerde, ipucu hücrelerinin zarında düzensizlikler ve sitoplazmikboşluklar olduğu gözlenmiştir. Ayrıca, eritrositlerin ipucu hücreleri ile olanetkileşimi ışık mikroskopik olarak saptanmıştır.Mikrobiyolojik yöntemde ise 200 hastanın 16'sından (%8) Gardnerella vaginalisizole edilmiştir. Bu 16 hastanın Gramla boyanan preparatında ipucu hücresi vegram negatif kokobasiller bulunduğu ancak laktobasil bulunmadığı belirlenmiş vebu hastalar BV pozitif olarak sınıflandırılmıştır. Ayrıca alınan örnekler %10'lukpotasyum hidroksit (KOH=Whiff) ile test edilmiş ve pH değerleri ölçülmüştür.Araştırmamızdaki istatistiksel analizler Kikare ve Fisher Kesinlik testine göreyapılmıştır. Yapılan istatistiksel değerlendirme sonucunda sitolojik vemikrobiyolojik yöntemle elde edilen sonuçlar arasında BV enfeksiyonu açısındananlamlı bir ilişki olduğu belirlenmiştir. Clue celland BV-associated coccobacilli and cocci were found but polymorphonuclearleukocyte (PMNL) and lactobacilli were not seen in 21 of 200 (10.5 %) patients bycytological method. These patients were considered as BV positive. Also,Gardnerella vaginalis was detected in 13 of these 21 patients by themicrobiological methods. In smears with BV, cytoplasmic cavitations and irregularcell membrane were observed on clue cells. Also, the interaction of erythrocyteswith the clue cells were detected by the light microscopic examination.By using the microbiological methods, Gardnerella vaginalis was isolated from 16of 200 (8%) women. Clue cell and gram negative coccobacilli were seenabundantly and vaginal lactobacilli were not observed in gram stainedpreparations of these 16 patients. These samples also tested for the release offishy odor with the addition of 10% KOH (potassium hydroxide) and the pH wasmeasured. In this study, the statistical analysis was performed by Chi-square andFisher?s exact test. In statistical analysis, there was a significant association forbacterial vaginosis between the microbiological and cytological methods.
