Chlamydıa trachomatıs`in servikal örneklerde sitolojik ve immünofloresan teknikleri kullanılarak araştırılması

Abstract
Bu çalışmada, Chlamydia trachomatis varlığını saptamak amacıyla HacettepeÜniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı kliniğinebaşvuran 200 hastaya ait servikal örnek, sitolojik ve immünofloresan [DirektFloresan Antikor (DFA)] teknikleri kullanılarak incelenmiştir. İncelenen bu 200örneğin 7'si (%3.5) sitolojik teknik ile 37'si (%18.5) immünofloresan tekniği ile ve12'si (%6) hem sitolojik hem de immünofloresan teknikleri ile C. trachomatis pozitifolarak değerlendirilmiştir. Yapılan istatistiksel incelemeler sonucunda C.trachomatis tanısı açısından sitolojik ve immünofloresan teknikler arasında anlamlıbir ilişki bulunduğu görülmüştür (p<0.05).Sitolojik inceleme için alınan örnekler Papanicolaou boyama yöntemine göreboyanıp sitolojik bulgular yönünden incelenmiştir. Sitolojik teknik ile C. trachomatistanısı verilirken, servikal örneklerde metaplazik hücrelerin içinde Chlamydialinklüzyon cisimciklerinin bulunması esas alınmıştır. 200 servikal örneğin 36'sında(%18) metaplazik hücrelerin bulunduğu saptanmıştır. Metaplazik hücrelerinbulunduğu 36 (%18) yaymanın 15'inde (%41.6) bu hücrelerde Chlamydialinklüzyon cisimcikleri tespit edilmiş ve C. trachomatis varlığı ile metaplazikhücrelerin bulunması arasında istatistiksel açıdan anlamlı bir ilişki bulunmuştur(p<0.05). Chlamydial inklüzyon cisimcikleri saptanan yaymalarda bol miktardapolimorfonükleer lökositler (PMNL) görülmüş, ayrıca yaymalarda makrofaj veeozinofil lökositlerin de bulunduğu saptanmıştır. Yaymalarda Chlamydial inklüzyoncisimciklerinin yanı sıra Candida da görülmüştür.İmmünofloresan teknik için alınan servikal örnekler ise metanol ile fikse edilmiş veörneklere floresan izotiyosiyanat (FITC) işaretli Chlamydial monoklonal antikoruygulanmıştır. Örnekler floresan mikroskobunda incelenmiş ve toplu iğne başıbüyüklüğünde, düzgün yuvarlak şekilli ve elma yeşili renkte ışıma verenChlamydial elementer cisimciklerin (EC) görülmesi ile C. trachomatis pozitifliktanısı konulmuştur.i In this study, cervical smears taken from 200 patients who were applied toGynecology and Obstetrics Clinics were examined through cytological andimmunofluorescence [Direct fluorescence antibody (DFA)] techniques to detect theprecence of C. trachomatis. Of the 200 samples, 7 (3.5%) were found positive bycytological technique, 37 (18.5%) were positive by immunofluorescence techniqueand 12 (6%) were positive by both cytological and immunofluorescence technique.A statistically significant difference was detected between these techniques interms of diagnosing C. trachomatis (p<0.05).All the samples taken for cytological examination were stained by Papanicolaoumethod and investigated in respect of cytological findings. The cervical smearswere considered positive if Chlamydial inclusion bodies were present inmetaplastic cells. In 36 (18%) of 200 cervical smears the presence of metaplasticcells were detected of which 15 of them contained Chlamydial inclusion bodies.The difference between the presence of C. trachomatis and metaplastic cells werefound statistically significant (p<0.05). It was also seen abundant polymorphonuclear leucocytes (PMNL) in smears having Chlamydial inclusion bodies.Moreover, macrophages and eosinophil leucocytes were detected in thesesmears. In addition to Chlamydial inclusion bodies, Candida was also observed inthe smears.The cervical samples taken for immunofluorescence technique, were fixed bymethanol and fluorescein isothiocyanate (FITC) labelled Chlamydial monoclonalantibody was applied to these samples. The samples were examined byfluorescence microscobe and it was diagnosed as C. trachomatis positivities wereobserved as the presence of pin head-like, regular round shaped and bright applegreen coloured Chlamydial elementary bodies (EB).iiiIn conclusion, the patients who applied to Gynecology and Obstetrics Clinics withvarious gynecologic problems are also to be examined in terms of Chlamydiapositivity, in addition to other infectious agents. The cytologic andimmunofluorescence techniques should be used together in order to increase thechance of the diagnosis.
