Topolojik optimizasyon yöntemiyle tasarlanan yeni bir mandibular distraktörün teorik ve deneysel olarak değerlendirilmesi

Abstract
Mandibular Distraksiyon Osteogenezisi (MDO), mandibula (alt çene) gelişim yetersizliği tedavisinde sıklıkla uygulanan klinik bir yöntemdir. Ancak, mevcut MDO protokollerinde osteotomi (kemik kesisi) hattının eğiminin belirlenmesi konusunda standartlaşmış bir yaklaşım olmadığından tedavi sürecinde distraksiyon doğrultusunda sapmalar ve diş kapanış bozuklukları artmakta, bu durum kallus (yeni kemik dokusu) oluşumunu olumsuz etkileyecek ek operasyonları zorunlu kılmaktadır. Diğer yandan, çok sayıda vida yuvası içeren mandibular distraktörlerde, vida konfigürasyonunun tecrübeye dayalı olarak belirlenmesi, kallusun stabilizasyonunu azaltabilmektedir. Bu durumlar relaps (nüks) ihtimalini artırmakta ve remodeling (yeniden şekillenme) periyodunun uzamasına sebep olmaktadır.Bu tez çalışmasında, söz konusu problemler için çözüm içeren yeni bir MDO protokolü önerilmiştir. Bu protokolün aşamaları; Bilgisayarlı Tomografi (BT) verileri kullanılarak modellenen mandibulada Optimum Osteotomi Hattının (OOL) hastaya özel olarak belirlenmesi, OOL yaklaşımının operasyon ortamına aktarılmasını sağlayacak osteotomi kılavuzunun modellenerek üretilmesi, Optimum Vida Konfigürasyonunun (OSC) MATLAB-PYTHON-ANSYS programları arasında kurulan Parçacık Sürü Optimizasyonu (PSO) döngüsü ile elde edilmesi ve son olarak distraktör plak geometrisinin Topoloji Optimizasyonu yöntemiyle tasarlanması olarak belirlenmiştir. Önerilen MDO protokolünün test edilmesi amacıyla 3 farklı Sonlu Elemanlar (SE) modeli kurulmuştur. Bunlardan ilki Konvansiyonel Osteotomi Hattına (COL) ve Konvansiyonel Vida Konfigürasyonuna (CSC) göre oluşturulan model, ikincisi OOL ve CSC'ye göre oluşturulan model ve sonuncusu ise OOL ve OSC'ye göre oluşturulan modeldir. Deneysel aşamada ise, kurulan SE modellerine uygun numuneler üretilmiş ve SEA sınır şartlarını simüle eden sistem yardımıyla testler gerçekleştirilmiştir. SE analizi sonuçlarına göre 100 N yük altında; numuneler, sadece osteotomi hattı optimize edildiğinde %28,5 oranında, osteotomi hattı ile vida konfigürasyonu birlikte optimize edildiğinde ise %64,2 oranında daha az deplasman değeri göstermişlerdir. Aynı yükleme koşulundaki deneysel sonuçlarda ise, numuneler sırasıyla %62 ve %84,5 oranında daha az deplasman değeri göstermiştir. Sonuç olarak, hem nümerik hem de deneysel çalışmalarla geliştirilen kişiye özel MDO protokolü, kallus dokusunun çok daha stabil olmasını sağlamaktadır. Önerilen protokolün etkinliği, bir hastaya uygulanarak klinik gözlemlerle de doğrulanmıştır. Dolayısıyla bu protokolün klinikte kullanımının, iyileşme periyodunu kısaltarak operasyon başarısını artırması beklenmektedir. Mandibular Distraction Osteogenesis (MDO) is a common clinical procedure to correct mandibular retrognathia cases. However, since there is not a standard approach for determining the angulation of osteotomy line for current MDO operations, deviation of distraction direction and malocclusion increase. These may necessitate additional operations that adversely affect the callus (remodelled bone tissue) stabilization. On the other hand, for mandibular distractors having a large number of screw holes, determining the screw configuration based on surgeon's experience may reduce the stabilization of callus tissues. In these cases, the probability of relapse increases and remodelling period gets longer. In this thesis, a new MDO protocol has been proposed to solve these problems. This protocol includes; determining the optimal Osteotomy Line (OOL) for the mandible modeled using computed tomography (CT) data, modeling an osteotomy guide to aid in applying the OOL approach in real operations, determining the Optimum Screw Configuration (OSC) using Particle Swarm Optimization (PSO) loop linking between MATLAB-PYTHON-ANSYS programs and finally design of distractor plate geometry with Topology Optimization method.In order to test the proposed MDO protocol, 3 different Finite Element (FE) models were established. The first model was set up based on Conventional Osteotomy Line (COL) and Conventional Screw Configuration (CSC). The second and the third models were established according to OOL-CSC and OOL-OSC, respectively. In the experimental stage, the samples were prepared in accordance with the FE models and the tests were performed by simulating the FEA boundary conditions.According to FEA results under 100 N load; the displacement of the samples showed a 28.5% reduction as the osteotomy line was separately optimized, and 64.2% less displacement was determined when the osteotomy line and the screw configuration were optimized together. In the experimental results for the same loading condition, the samples showed 62% and 84.5% less displacement values, respectively. As a result, MDO protocol, which is developed by both numerical and experimental studies, makes the callus tissue much more stable. On the other hand, efficiency of the proposed protocol implemented in a patient was verified with clinical observations. Therefore, the clinical use of this protocol is expected to increase the success rate of the operation by shortening the recovery period.
