Abstract
ÖZET Bu çalışmada, şizofreni tanısıyla kapalı psikoz servislerinde yatan bireylerin aileleriyle yapılan iletişim becerileri odaklı psiko-eğitim grup programına bağlı olarak değişen aile işlevleri araştırılmıştır. Bu amaç doğrultusunda yanıt aranan denenceler dört grupta toplanmıştır. Birinci ve ikinci grupta, deney ve kontrol grubundaki deneklerin Aile Değerlendirme Ölçeği (ADÖ) puanlan birbirleriyle karşılaştırmışken, üçüncü grupta deney grubundaki deneklerin uygulama öncesi ve uygulama sonrası ADÖ puanları, dördüncü grupta ise yine deney grubundaki deneklerin uygulama öncesi ve izleme testi ADÖ puanlan birbirleriyle karşılaştırılmıştır. Araştırmaya katılan denekler 1996 yılı başından itibaren dört aylık süre içinde A.Ü.T.F. Psikiyatri polikliniğine başvuran, katlarda yatarak tedavisi planlanan ve DSM-IV sınıflandırma sistemine göre şizofreni tanısı alan hastaların yakınlarından oluşmuştur. Toplam 23 hasta yakını çalışmaya alınmış, bunlardan on tanesi deney grubunu 13 tanesi de kontrol grubunu oluşturmuştur. Her iki gruba başlangıçta Aile Değerlendirme Ölçeği, aile fonksiyonlarını belirleme amacıyla uygulanmıştır. Bu aşamada deney ve kontrol grubunun Aile Değerlendirme Ölçeği alt ölçek puanlarına göre birbirlerine benzer oldukları parametrik olmayan istatistiksel yöntemlerden Mann-Whitney U testi ile saptanmıştır. Daha sonra deney grubundaki deneklerle araştırmacı tarafından geliştirilen iletişim becerileri odaklı psiko-eğitim grup programına başlanmış ve 12 oturumluk süreç sonunda deney ve kontrol gruplarına Aile Değerlendirme Ölçeği tekrar verilmiştir. Altı ay sonra aynı ölçek deney ve kontrol gruplarına (izleme testi) son kez tekrar verilmiştir. Araştırmanın birinci ve ikinci grup hipotezlerini oluşturan deney ve kontrol gruplarının birbirleriyle uygulama sonrası ve izleme testi puanları açısından karşılaştırılmasında Mann-Whitney Utesti kullanılmıştır. Üçüncü ve dördüncü grup hipotezleri oluşturan deney grubunun kendi içinde karşılaştırılmasında ise Wilcoxon Uyumlu-Çiftler İşaretli-Mertebeler testi kullanılmıştır. Bütün analizlerde hata payı p<.05 olarak alınmıştır. Araştırmadan elde edilen bulgular şu şekilde özetlenebilir. 1. Yapılan iletişim becerileri odaklı psiko-eğitim grup programının sonucunda Aile Değerlendirme Ölçeği uygulama sonrası puanları açısından deney ve kontrol grubu karşılaştırıldığında `problem çözme, iletişim, roller, davranış kontrolü ve genel fonksiyonlar` alt ölçeklerinde deney grubu lehine anlamlı farklar bulunmuştur. Gereken ilgiyi gösterme ve duygusal tepki verebilme alt ölçeklerinde ise her iki grup arasında anlamlı bir fark bulunamamıştır. 2.Yapılan iletişim becerileri odaklı psiko-eğitim grup programının sonucunda Aile Değerlendirme Ölçeği izleme-testi puanları açısından deney ve kontrol grubu karşılaştırıldığında `iletişim, davranış kontrolü ve genel fonksiyonlar` alt ölçeklerinde deney grubu lehine anlamlı farklar bulunmuştur. Gereken ilgiyi gösterme, roller, duygusal tepki verebilme ve gereken ilgiyi gösterme alt ölçeklerinde ise her iki grup arasında anlamlı bir fark bulunamamıştır. 3.Yapılan iletişim becerileri odaklı psiko-eğitim grup programının sonucunda deney grubundaki deneklerin Aile Değerlendirme Ölçeği uygulama öncesi-uygulama sonrası puanları karşılaştırıldığında `Problem çözme, iletişim, roller, gereken ilgiyi gösterme, davranış kontrolü ve genel fonksiyonlar` alt ölçeklerinde uygulama sonrası lehine önemli farklar bulunmuştur. Duygusal tepki verebilme alt ölçeğinde ise uygulama öncesi-uygulama sonrası puanları açısından anlamlı bir fark bulunamamıştır.m 4. Yapılan iletişim becerileri odaklı psiko-eğitim grup programının sonucunda deney grubundaki deneklerin Aile Değerlendirme Ölçeği uygulama öncesi-izleme testi puanları karşılaştırıldığında `Problem çözme, iletişim, duygusal tepki verebilme, gereken ilgiyi gösterme, davranış kontrolü ve genel fonksiyonlar` alt ölçeklerinde izleme-testi lehine önemli farklar bulunmuştur. Roller alt ölçeğinde ise anlamlı bir fark saptanmamıştır. IV SUMMARY In this study, the effects of communication skills training focused family psychoeducational group approach on family functioning was assessed in the families of schizophrenic patients. Four groups of hypothesis were tested. The first and second group of hypothesis were consisted of the comparison of control and experiment group with family assessment device (FAD). The third group of hypothesis were dealing with the comparison of the pre-test and post-test FAD scores and the fourth group of hypothesis with the pre-test and follow-up FAD scores within the experiment group itself. The subjects were selected among the relatives of the patients who have brought into and admitted to locked psychiatric units of Ankara University, Medical School, Department of Psychiatry during the period between January 1996 and April 1996. All subjects were the relatives of patients with the DSM-IV diagnosis of schizophrenia. A total of 23 relatives enrolled into the study; 1 0 were assigned to experiment group and 13 into control group. These two groups were subjected to a pre test with FAD for the assessment of the level of family functioning in advance of the experiment and no significant differences were found between the experiment and control groups with Mann-Whitney U test. After the communication skills training focused family psychoeducational group approach which is developed by the investigator was applied in twelve sessions, the FAD was re- administered to the experiment and control groups as a post-test. A folow-up test was given to the same groups after 6 months. The comparisons between control and experiment groups for the first and second group of hypothesis were done with Mann-Whithey U test with respect to scores of pre-test, post-test and follow-up tests. Where as to compare experiment group within itself for the third and fourth groupof hypothesis, the Wilcoxon Signed-Rank Test was used. The findings were evaluated at p<05 significance level. The results of this study can be summarized as follows: 1-At the end of the communication skills training focused family psychoeducational group sessions, there were a significant decrease at the post-test FAD subtests of `problem solving`, `communication`, `roles`, `behavior control` and `general functions` indicating better level of family functioning in the experiment group. No statistically significant differences were found in the `affective responsiveness`, and `affective involvement` FAD subtest scores. 2- At the follow up assessment, there were a significant decrease at the follow-up FAD subtests of `communication`, `behavior control` and `general functions` indicating better level of family functioning in the experiment group in comparison to the control group. No statistically significant differences were found in `problem solving`, `roles` `affective responsiveness` and `affective involvement` FAD subtest scores. 3- The comparison of pre-test and post-test FAD subtest scores of the experiment group, there were a significant decrease at the post-test FAD subtests of `problem solving`, `communication`, `roles`, `behavior control`, `affective involvement` and `general functions` indicating better level of family functioning after the communication skills training focused family psychoeducational group sessions in the experiment group. The difference between pre-test and post-test scores of `affective responsiveness` subtest was not significant. 4- The comparison of pre-test and follow-up test FAD subtest scores of the experiment group, there were a significant decrease at the follow- up test FAD subtests of `problem solving`, `communication`, `affectiveVI responsiveness`, `behavior control`, `affective involvement` and `general functions` indicating better level of family functioning even 6 months after the communication skills training focused family psychoeducational group sessions in the experiment group. Roles subtest scores were not statistically different between groups.
