Meme kanseri kök hücre (benzeri) kültürlerde kanser ilaçlarının etkinliğinin moleküler mekanizmalarının araştırılması

Abstract
Tez araştırmasının amacı meme kanseri hastalarının tümör dokularından elde edilmiş olan meme kanseri primer kök hücre (benzeri) kültürlerde trastuzumab ile karboplatin ve paklitaksel kombinasyonlarının etkinliğini ayrıca HER2, sitokin ve matriks metalloproteinaz ifadelerini değerlendirmektir. Histopatolojik açıdan değerlendirildiğinde HER2 pozitif olarak sınıflandırılan hastalardan üretilen primer kültür popülasyonunda HER2 negatif hücre oranın belirlenmesi trastuzumab tedavisi sırasında görülen dirençliliği açıklamayabilmeyi sağlayacaktır. Primer kültürden saflaştırılmış olan CD44+/CD24-/ALDH+ hücreler kanser kök hücresi (benzeri) hücreler olarak adlandırılmıştır. Kültüre edilen hücrelerde (CD44+/CD24-/ALDH+ tip kültür meme kanseri hücreleri) antikanser ilaçlarının etkinliği, HER2 reseptörü ifadesi, sitokinlerinin ifadeleri ve metastatik hücre belirteçleri olan matriks metalloproteazların ifadeleri moleküler yöntemlerle (akım sitometri ve protein array) belirlenmiştir. Meme kanseri primer kültür popülasyonu % 34,6 HER2 pozitif, % 6,4 HER2 negatif meme kanseri kök hücresi benzeri popülasyon içermektedir. MMP-1, MMP-2, MMP-3, MMP-8, MMP-9, MMP-13, TIMP-1, TIMP-2 proteinleri HER2+ KKH'lerde primer kültüre göre 1.30 – 2.42 kat daha fazla ifade edilmektedir (p< 0.05). Sitokin ifadelerini incelediğimizde ise HER2 pozitif meme kanseri kök hücre benzeri kültürlerde primer kültüre göre önemli bir fark gözlenmemiştir. HER2 pozitif ve HER2 negatif kanser kök hücre (benzeri) kültürlerine ilaçların tek tek ve kombinasyonları uygulanarak, her kültür tipi için uygulanan ilacın IC50 konsantrasyonları ve ilaç etkileşimleri belirlenmiştir. Herseptin (Trastuzumab) HER2 pozitif kanser kök hücre (benzeri) hücrelere HER2 negatif alt kültüre olduğundan daha etkili olmuştur. Paklitaksel ve karboplatinde ise bu durum söz konusu değildir. Paklitaksel ve karboplatin kombinasyonu tüm alt kültürlerde sinerjik etki gösterirken, trastuzumab ise karboplatinle kombinasyonlarda alt kültürlerde sinerjik veya aditif etkileşim göstermiştir. Sonuçlar değerlendrildiğinde meme kanserinde HER2 hedefli tedavilerde tümörde bulunan HER2 negatif kanser kök hücrelerinin tedavinin ve tadaviye yanıtın seyrini değiştirebilecek önemli bir faktör olduğu anlaşılmaktadır. The aim of this study was to isolate breast cancer stem-like cells from HER2 negative and HER2 positive patients, by establishing their primary breast cancer stem cell cultures, The other main objective was to determine the effects of Trastuzumab, Paclitaxel and Carboplatin on sub cultures. Additionally investigaiton of HER2, cytokine and matrix metalloproteinase status of these cells was intended. The source of primary culture was histopathologically classified as HER2 positive. Determination of HER2-negative cell population ratio in primary culture will provide a description of trastuzumab resistance occurs during treatment. CD44+/CD24-/ALDH+ cells were isolated from primary breast cancer cultures which are called breast cancer stem cells. The cultured cells (CD44 + / CD24- / ALDH-type culture of breast cancer cells) were identified by using molecular methods (flow cytometry and protein arrays) to determine the effects of anticancer drugs, HER2 receptor expression, expression of cytokines and expression of matrix metalloprotease with metastatic cell markers. Breast cancer primary culture include 34.6% HER2 positive and 6.4% of HER2-negative cancer stem cell-like population. The HER2+ cancer stem cells were expressed 1.30 - 2.42 fold higher than primary cell cultures in MMP-1, MMP-2, MMP-3, MMP-8, MMP-9, MMP-13, TIMP-1, TIMP-2 proteins (p <0.05). When we examined the expression of cytokines, HER2 positive breast cancer stem cells did not significantly overexpress the cytokines when compared to primary cancer cells population. HER2 negative and HER2 positive breast cancer stem cells were treated with trastuzumab, paclitaxel, carboplatin and combination of these anticancer agents. The IC50 levels of the drugs were determined for all of these breast cancer stem cell subgroups. Trastuzumab (Herseptin) was more effective on HER2 positive cancer stem (like) cells than HER2 negative sub-culture. This situation was not same for paclitaxel and carboplatin. As Paclitaxel and carboplatin exhibited synergistic effects in all sub-cultures, Paclitaxel- Herceptin binary application showed antagonistic effects. Herceptin combinations with carboplatin demonstrated synergistic-additive interaction in the sub-culture.When the results were analyzed, it can be deduced that the HER2-negative breast cancer stem cells in tumor can change the response to the treatment in HER2-targeted therapies.
