Obezite cerrahisinin serum ghrelin ve hepatosteatoz düzeylerine olan etkisi

Abstract
Obezite ve buna bağlı komplikasyonlar, endüstriyel toplumlarda hızla artış gösteren bir halk sağlığı sorunu olup dünya genelinde artmaya devam etmektedir. Obezite cerrahisi, morbid ve süper morbid obezite tedavisinde başarı sağlamadaki en etkili yöntemdir. Bu çalışmada obezite cerrahisinin serum ghrelin ve hepatosteatoz düzeylerine olan etkisini inceledik.Çalışmaya, Selçuk Üniversitesi Tıp Fakültesi Genel Cerrahi kliniğine başvuran 23 morbid obez (BKİ= 49,27±7,46 kg/m²) hasta alınmıştır. Bu hastalardan 16'sına laparoskopik sleeve gastrektomi ve 7'sine laparoskopik Roux-en-Y gastrik bypass operasyonları uygulanmıştır. Hastalardan, operasyon öncesinde ve operasyon sonrası 1., 3. ve 6. aylarda kan örnekleri toplanmıştır. Ghrelin düzeyleri, ELİSA yöntemi ile çalışılmış, steatoz varlığı ise uzman radyolog tarafından ultrasonografi ile tespit edilmiştir. BKİ, postoperatif 1, 3 ve 6. aylarda ameliyat öncesi ile karşılaştırıldığında anlamlı olarak azalmıştır (p < 0,05). Ghrelin düzeyinde ise preop dönemle, postop 1., 3. ve 6. aylar arasında anlamlı fark bulunmamıştır (p= 0,384). Hepatosteatoz varlığı, preop döneme kıyasla postop 1., 3. ve 6. aylarda anlamlı olarak azalmıştır (p < 0,05). Ghrelin düzeyleri ve hepatosteatoz arasında postop 1. ve 3. Aylarda negatif yönde zayıf ve anlamlı bir ilişki bulunmuştur (sırasıyla; r= -0,417 p= 0,48, r= -0,459 p= 0,028). Sonuç olarak obezite cerrahisi, bazı endokrin anormalliklerde düzelme sağlamış olsa da ghrelin seviyelerinde anlamlı bir farklılık ortaya çıkarmamıştır. Cerrahi sonrasında hepatosteatoz durumunda anlamlı azalmalar sağlanmıştır. Bu konuyla ilgili ghrelin ve hepatosteatozun birarada değerlendirildiği ileri çalışmalara ihtiyaç vardır.Anahtar Sözcükler: Ghrelin; obezite cerrahi; steatoz Obesity and related complications are a rapidly growing public health problem in industrial societies and continue to increase worldwide. Bariatric surgery is the most effective method to achieve success in morbid and super morbid obesity treatment. We investigated the effect of obesity surgery on serum ghrelin and hepatosteatosis levels in our study. 23 patients with morbid obesity (BMI = 49,27 ± 7,46 kg / m²) who were referred to Selcuk University Medical Faculty General Surgery Clinic were included in the study. Sixteen of these patients underwent laparoscopic sleeve gastrectomy and seven underwent laparoscopic Roux-en-Y gastric bypass operations. Blood samples were gathered from the patients before of the operation and at 1st, 3rd and 6th months after the operation. Ghrelin levels were studied by ELISA method and hepatosteatosis were performed by radiologist with ultrasonography. BMI was significantly decreased at postop 1., 3., and 6. months compared to preoperative values (p < 0,05). There was not found meaningful difference in ghrelin levels between preop and postop 1st, 3rd and 6th months (p = 0,384). The hepatosteatosis was significantly decreased at postop 1., 3. and 6. months compared to preop period (p < 0,05). There was a weak, significant and negative correlation between ghrelin levels and hepatosteatosis at postoperative 1. and 3. months.As a result, obesity surgery has improved some endocrine abnormalities, but did not show any significant difference in ghrelin levels. Significant reductions in hepatosteatosis were observed after obesity surgery. Further studies involving ghrelin and hepatosteatosis should be perform.Key Words: Ghrelin; obesity surgery; steatosis
