Abstract
İnfertilite nedeniyle gelen 400 hastanın semen ve sperm özelliklerine ilişkin yaş, konsantrasyon, motilite, morfometrik analiz ve mast hücresi parametrelerinin değerlendirilmesi amaçlandı. 3-4 günlük cinsel perhizi takiben steril bir kaba alman ve likeifîkasyondan hemen sonra değerlendirilen semen örneklerinin, konsantrasyon ve motilite değerlendirilmesinde Makler sayım kamarası, morfolojik analiz için Testsimplets lamlar kullanıldı. Mast hücrelerini görmek amacıyla, hastaların 6 'şar adet semen yayması % 1 Toluidin Blue (pH:4) ile boyandı, herhangi birinde mast hücresi görülen vakalar mast (+) olarak kabul edildi. Morfolojik değerlendirme Kruger'in strict kriterlerine göre ve X 1000' de yapıldı. Çalışma kapsamına alınan 400 olgunun yaş ortalaması 29.4 ± 5.6 idi. 375 olgunun ortalama sperm konsantrasyonu 36.7 ± 30.8 x 106 /mi bulundu. Bunların % 33.2'si oligozoospermik, % 60.5'i normozoospermik sperm özelliğinde idi. Sperm görülmeyen (azoospermik) % 6.25 olguda yaş ortalaması 29.2 ± 6.3 idi ve bu olgularda mast hücresine de rastlanmadı. Mast hücresine rastlanan % 21.5 olguda ise yaş ortalaması 31.6 ± 6.7 bulundu. Olguların normal ilerleyici motilite oram % 58.9, astenozoospermik olgu oram ise % 41.1 bulundu. Hastaların % 45.6'sı fertil grupta, % 46.4'ü subfertil grupta, % 8.0'i ise infertil grubta yer aldı. Olguların tümünde ortalama anormal sperm oram 86.4 ± 7.4, normal morfolojili sperm oram ise 13.4 ± 7.4 bulundu.Sonuç olarak yaş ilerledikçe sperm hareketliliği azalmakta, normal morfolojiye sahip sperm oram yükseldikçe motilite ve konsantrasyonu da yükselmekte, sperm sayısı arttıkça ilerleyici motilitesi de artmaktadır. Mast gözlenen olguların gözlenmeyen olgulara göre yaşlan daha yüksek; konsantrasyonları, ilerleyici motilite ve normal morfolojili sperm oranlan daha düşüktür. The aim of this work was to make an evaluation of sperm properties according to, age, concentration, motility, morphometry and mast cell content. The subjects were patients whom semen analysis was performed because of infertility suspicious, total number of subject was 400. Semen samples were collected in sterile cups after a 3-4 day sexual abstinance period. After liquefaction concentration and motility was evaluated by a Makler chamber, morphometric analysis was done with Testsimplets slides and morphology was done according to Kruger's strict criteria under immersion oil magnification (XI 000). To evaluate mast cells 6 smear slides was prepared for each subject and stained with 1% Toluidin Blue (pH:4), if a mast cell was observed in on of the slides, the semen was concluded as (+).Mean age value of subjects was 29.4 ± 5.6. The mean concentration of 375 subject was 36.7 ± 30.8 x 106/ml. 33.2% was considered of the total subjects as oligozoospermic while 60.5% was normozoospermic. The mean age of azoospermic patients (21.5% of total patients), was 31.6 ± 6.7. Normal progression motility subjects occupied 58.9%, while asthenozoospermic subjects were 41.1%. Fertile percentage of subjects was 45.6%, subfertile 46.4% and infertile subjects occupied 8.0% in our work. According to strict criteria, abnormal sperm percentage was 86.4 ± 7.4 while normal percentage was 13.4 ± 7.4. 97As a result we conclude that as aging occurs sperm motility decreases, as normal morphologic sperm percentage increases the progressive motility percentage and concentration also increase, as sperm total number increases also progressive motility rate increases. The mean age of mast cell (+) patients was higher then mast cell (-) patients. But mast cell (+) semen samples contained lower sperm concentration and lower progressive motility rate and abnormal morphologic sperm rate was higher when compared with (-) samples.
