Abstract
Hepatit B virüsü enfeksiyonu tüm dünyada olduğu gibi ülkemizde de önemli bir sağlık sorunudur, ciddi sosyal ve ekonomik kayıplar ile ilişkilidir. Yaşam kalitesi, belli yaşam koşullarında bireyi etkileyen rahatsızlıkların bedensel, sosyal ve ruhsal etkilerine günlük yaşamda verilen bireysel yanıt olarak tanımlanabilir. Kronik HBV enfeksiyonunun erişkinlerin yaşam kalitesini düşürdüğü gösterilmiştir, ancak kronik HBV enfeksiyonlu çocuklarda yaşam kalitesi konusunda çok az bilgi vardır. Çalışmamıza Aralık 2017-Aralık 2018 arasında Selçuk Üniversitesi Tıp Fakültesi Hastanesi Çocuk Gastroenteroloji, Hepatoloji ve Beslenme Polikliniği'nde izlenen 7-18 yaş arası HCC'si olmayan ve non-sirotik kronik hepatit B enfeksiyonu tanılı 32 çocuk ile Çocuk Sağlığı ve Hastalıkları Polikliniği'ne başvuran aynı yaş aralığındaki herhangi bir kronik hastalığı olmayan, hepatit B taraması yapılmış ve tarama sonucu negatif gelen 36 sağlıklı çocuk alındı. Hasta çocuklarda KINDL toplam ve özgüven alt ölçek puanları sağlıklı çocuklara göre anlamlı düzeyde düşüktü. Halsizlik/yorgunluk gibi yakınmalar hasta grupta daha yüksek oranda ve her iki cinsiyette benzer düzeyde idi. Hastalar arasında serum ALT düzeyi ≥55 U/L ve/veya HBeAg-pozitifliği olanlarda daha sık semptom görülmekte, HBV-DNA düzeyi ≥2000IU/mL olması durumunda yaşam kalitesinde bozulma (depresyon/anksiyete) riski artmakta idi. HBeAg pozitif ve negatif immün aktif dönemler ayrıca KINDL arkadaş ve okul alt ölçek puanlarında düşüş ile ilişkili idi. Hasta çocuklardaki yaşam kalitesi annenin tutumu ve eğitim düzeyinden etkilenmemekte idi. Kronik HBV enfeksiyonlu çocuklarda yaşam kalitesinin değerlendirilmesi ve hastalıkla ilişkili yaşam kalitesini etkileyen faktörlerin belirlenmesi, bu faktörlere yönelik önlem ve tedavi ile yaşam kalitesini iyileştirebilir. Hepatitis B virus infection is an important health problem in our country as all over the world and is associated with serious social and economic loss. Quality of life can be defined as the individual response in daily life to the physical, social and psychological effects of the disorders affecting the individual under certain life conditions. Chronic HBV infection has been shown to decrease the quality of life in adults, but little is known about the quality of life in children with chronic HBV infection. Between December 2017 and December 2018, 32 children without HCC and with non-cirrhotic chronic hepatitis B infection who were followed up in Pediatric Gastroenterology, Hepatology and Nutrition Outpatient Clinic of Selçuk University Faculty of Medicine and 36 healthy children, who had no chronic disease in the same age range, who had been screened for hepatitis B and had negative results were included in the study. KINDL total and self-confidence subscale scores were significantly lower in patients compared to healthy children. Complaints such as fatigue were higher in the patient group and were similar in both genders. Symptoms were more frequent among patients with serum ALT level ≥55 U / L and / or HBeAg-positivity, and the risk of impairment in quality of life (depression / anxiety) was increased if the HBV-DNA level was ≥2000IU / mL. HBeAg positive and negative immune active periods were also associated with a decrease in KINDL friends and school subscale scores. The quality of life was not affected by the mother's attitude and education level in patients. Evaluation of the quality of life in children with chronic HBV infection and identifying the factors affecting the quality of life associated with the disease may improve quality of life through prevention and treatment of these factors.
