Arteria tiroidea inferior bilateral ligasyonunun serum kalsiyum düzeyine olan etkisi

Abstract
38 ÖZET inferior tiroid arter bilateral ligasyonunun, serum total kalsiyum seviyesine ve semptomatik hipokalsemi üzerine olan etkisini araştırmak amacıyla Atatürk Üniversitesi Tıp Fakültesi Genel Cerrahi kliniğinde 1.3.1991-15.11.1991 tarihleri arasında yapılan 50 bilateral subtotal tiroidektomi olgusu prospektif olarak çalışmaya alınmıştır. Tiroid maligniteleri, tiroiditler ve nüks guatrlar çalışmaya dahil edilmemiştir. Hastalar iki gruba ayrılarak I. grupta 25 vakada, inferior tiroid arterleri bilateral bağlanmış, II. grupta 25 vakada ise inferior tiroid arterleri bağlanmamıştır. Hastalarda preoperatif, postoperatif 2. ve 4. gün serum total kalsiyum ve inorganik fosfor değerlerine bakılmış, semptomatik hipokalsemi yönünden izlenmişlerdir. Her iki grupta postoperatif dönemde serum total kalsiyum seviyelerinde istatistiksel olarak çok anlamlı düşüşler görülmüş olup (p<0.001), her iki grup karşılaştırıldığında serum total kalsiyum seviyelerindeki düşüşler anlamlı bu1unmamıştır(p>0.05). Toksik nodüler guatrlı 7 vaka ile tiroid aktivitesi normal olan 43 vaka karşılaştırıldığında postoperatif 2. gün serum total kalsiyum seviyelerindeki düşüş, hipertiroidili 7 vakada anlamlı bulunmuştur (p<0.01). Her iki grupta görülen Chvostek, Trousseau ve karpopedal spazm belirtileri arasında anlamlı bir fark görülmemiştir. Şiddetli tetaniye rastlanmamıştır. I. grupta bir (%4), II. grupta iki (%8) olguda geçici hipoparatiroidizm ortaya çıkmış olup istatistiksel olarak önemli bulunmamıştır. II. grupta geçici hipoparatiroidi görülen hastaların birisinde toksik nodüler guatr (%14), diğer ikisinde ise basit multinodüler guatr'ı mevcuttu. Verilen tıbbi tedaviye kısa zamanda39 olumlu cevap alınmıştır. Olgularımızda kalıcı hipoparatiroidizme rastlanmamıştır. Sonuç olarak arteria tiroidea inferior bilateral ligasyonunun subtotal tiroidektomi sonrası hipoparatiroidi gelişmesine direkt etkisi olmadığı görülmüştür. THE EFFECT OF BILATERAL LIGATION OF INFERIOR THYROID ARTERIES ON SERUM CALCIUM LEVELS SUMMARY In order to investigate the effect of bilateral ligation of inferior thyroid arteries on serum total calcium level and symptomatic hypocalcemia, 50 patients in Department of General Surgery (Atatürk University) underwent the bilateral subtotal thyroidectomy were prospectively included in this research between 03.01.1991 and 11.15.1991 patients with thyroid malignity, thyroiditis, and goitre recurrence were excluded. Patients were divided into two groups according to whether the ligation was performed : Grup I (with ligation, 25 patients); Group n (without ligation, 25 patients). The levels of serum calcium and phosphor were determined in preoperative, and postoperative (the 2nd and the 4th days) periods and the patients were observed in view of symtomatic hypocalcemia. Serum total calcium levels in postoperative period were found to be lower in both patient groups than those of preoperative period (p<0.001), though there was nodifference between two patient groups (p>0.05). Seven patients with toxic nodulary goitre were operated after medical treatment Decrease of the serum total calcium level in the postoperative 2nd day was found to be more in these 7 patients than that of the, other 43 patients with normal thyroid activity (p<0.01). There was no difference in term of sings such as chvostech, Trousseau, and carpopedal spasm of both patient groups. Severe tetany was not seen in any patient Temporary hypoparathyroidism was seen in 1(4%) patient of group I and in 2 (8%) patients of group II; this difference wast not significance of patients seen temporary hypoparathyroids, 1 had toxic nodulary goitre, and the others had simple multinodulary goitre. These patients have responsed positevly to medical treatment within a short time. Constant hypoparathyroidism was not seen in patients. In conclusion, we observed that there was not directly effect of the bilateral ligation of inferior thyroid arteries on the development of hypoparathyroidism after subtotal thyroidectomy.
