5-fluorourasil ile birlikte kullanılan Granülosit makrofaj koloni stimüle edici faktör ve İnterlökin-2`nin deneysel kolon anastomozlarına etkisi

Abstract
31 Kolorektal kanserler batı ülkelerinde akciğer kanserlerinden sonra ikinci sıklıkta görülürler. Kolorektal cerrahi girişimlerden sonra oluşan anastomoz ayrılması ve kaçak, yüksek morbidite ve mortalite ile sonuçlanan ciddi bir komplikasyondur. Bugüne kadar lokal veya sistemik bîr çok faktörün anastomoz iyileşmesi üzerine farklı etkileri olduğu gösterilmiştir. Çalışmada tüm deneklere 5 cm'lik distal kolon rezeksiyonu ve daha sonra uc uca kolokolik anastomoz yapıldı. Bu işlemden sonra denekler her grupta 10'ar adet tavşan olmak üzere dört gruba ayrıldılar. Grup l'e(kontrol grubu) post operatif birinci günden 4. güne kadar 2 cc intraperitoneal serum fizyolojik, grup H'ye post operatif birinci günden 4. güne kadar 20 mg/kg intraperitoneal 5-Fu, grup IH'e post operatif birinci günden 4. güne kadar 20 mikrogram/kg intraperitoneal GM-CSF ve 20 mg/kg 5-Fu, grup IVe post operatif birinci günden 4. güne kadar 2.1 06 1 U/kg subkutan IL-2 ve 20 mg/kg intraperitoneal 5- Fu verildi. Post operatif 7.günde tüm denekler sakrifiye edildi. Tüm deneklerin anastomoz hattındaki patlama basınçları ölçüldü. Biyokimyasal olarak doku hidroksiproiin değerleri ve histopatolojik olarak yara iyileşme skorları belirlendi. Patlama basınçları, doku hidroksiproiin değerleri ve yara iyileşme skorları istatisiksel olarak karşılaştırıldığında, kontrol grubu ile çalışma grupları arasında istatistiksel olarak anlamlı farklar bulundu(sırasıyîa p<0.0001, p0.0001, p<0.001). Çalışma grupları kendi aralannda karşılaştırıldığında, her üç çalışma metoduna göre istatistiksel olarak anlamlı fark tespit ediiemedî(p>0.05). Sonuç olarak 5-Fu'nun yara iyileşmesini bozduğu, 5-Fu ile birlikte kullanılan GM-CSF ve IL-2'nin yara iyileşmesini arttırmadığı tespit edildi. Bu sonuçların, daha sonra yapılacak deneysel ve klinik çalışmalarla irdelenmesi gerekmektedir. 32 ABSTRACT In Western countries, cancer of the colon and rectum ranks second after cancer of the lung in incidence. Dehiscence and leak of anastomosis after colorectal surgical procedures continue to be a serious complication with a high morbidity and mortality rate. Up to date, many local and systemic factors have been reported to have different effects on the healing of anastomosis. In the study, aii of the subjects underwent colonic resection and end to end colocolic anastomosis. Then the subjects were divided in to 4 groups of 10. In group I (control group), 2 cc serum physiologic was injected intraperitoneally from first day to 4th day postoperatively. In group II, 20 mg/kg 5-Fu was injected intraperitoneally from first day to 4th day postoperatively. In group III, 20 microgram/kg GM-CSF and 5-Fu was injected intraperitoneally from first day to 4th day postoperatively. In group IV, 2.106 JU/kg IL-2 was injected subcutaneously and 20 mg/kg 5-Fu was injected intraperitoneally from first day to 4th day postoperatively. All of the subjects were sacrificied at the 7th day postoperatively. Burst pressure of the anastomoses were measured. Tissue hydroxyproline values were measured biochemically and wound healing scores were investigated histopathologicaiiy. In the comparison of burst pressures, tissue hydroxyproline values and wound healing scores, statistically significant differences were found between the control group and study groups( p<0.0001, p<0.0001, p<0.001, respectively ). In the comparison of those three values among the study groups, no significant difference was found( p>0.05). We conclude that 5-Fu distrubs wound healing and GM-CSF and IL-2 which have been used with 5-Fu do not enhance wound healing. Experimental and clinical studies are needed to support these data.
