Ameliyathanede hasta güvenliği: Cerrahi ekibin güvenli cerrahi kontrol listesinin uygulanışına ilişkin tutumları

Abstract

`Güvenli Cerrahi Hayat Kurtarır` projesi kapsamında oluşturulan `Güvenli Cerrahi Kontrol Listesi` (GCKL) ile cerrahide güvenliğin sağlanması hedeflenmiştir. Güvenli Cerrahi Kontrol Listesi 2009'da `Güvenli Cerrahi Kontrol ListesiTR` (GCKLTR) adı ile Türkiye'de kullanılmaya başlandı. Çalışma cerrahi ekibin ameliyathanelerde GCKL'nin uygulanışına ilişkin tutumlarını saptamak amacıyla tanımlayıcı türde planlandı. Araştırma Trabzon il merkezinde bulunan tüm hastanelerin ameliyathanelerinde çalışan 303 cerrahi ekip üyesi ile gerçekleştirildi. Veriler araştırmacı tarafından geliştirilen 29 maddelik soru formu ile toplandı; sayı, yüzdelik ve ki-kare testleri ile değerlendirildi. Cerrahi ekip üyelerinin yaklaşık 3/4'ünün GCKLTR'nin hasta güvenliğine katkısı olduğuna inandıkları ancak yarısının GCKLTR'nin kurumlarında doğru uygulandığını düşündükleri saptandı. Katılımcıların çoğunluğu kendi kurumlarında GCKLTR uygulamasının gerektiği gibi olmadığını ve ameliyatın acil/planlı oluşu, büyüklüğü ve ekip üyelerinin bu uygulamaya ilişkin tutumlarına göre değişiklik gösterdiğini belirttiler. Listenin tüm maddelerinin yanıtlandığını belirten cerrahi ekip üyelerinin çoğunlukta olduğu saptandı. Güvenli Cerrahi Kontrol ListesiTR en çok uygulanan maddelerin; hastanın alerji öyküsünün sorgulanması; ameliyatta kullanılacak malzemelerin ameliyat öncesi hazırlanması ve sterilizasyonunun sağlanması ve ameliyat sonrası hastanın transfer edileceği bölümün teyit edilmesi olduğu belirlendi. `Anestezi Güvenlik Kontrol Listesi` nin uygulanması, ekip üyelerinin kendilerini tanıtmaları ve hastanın kimliğinin, gerçekleştirilen ameliyat ve ameliyat bölgesinin doğrulanmasının ise listenin en az uyum sağlanan maddeleri olduğu bulundu. With the `Safe Surgery Checklist` (SSC) established as part of `Safe Surgery Saves Lives` project; enabling safety in surgery was aimed. Safe Surgery Checklist was started to be used under the name of `Safe Surgery ChecklistTR` (SSCTR) in Turkey in 2009. The study was planned in a descriptive manner in order to determine the attitudes of surgery team members towards implementing safe surgery checklists in operating rooms. The study was conducted with 303 surgery team members working in the operating rooms of hospitals located in Trabzon city center. The data were gathered using a questionnaire form of 29 items developed by the researcher and evaluated by number tests, percentage tests and chi-square tests. It was found out that nearly three fourth of the surgery team members believed that SSCTR made contributions to the safety of patients, but only half of them thought that SSCTR was used properly in their institutions. Most of the participants pointed out that the implementation of SSCTR was not proper in their institutions and differed depending on the surgery being urgent or planned, the scale of the surgery and the attitudes of surgery team members towards the implementation. It was seen that the surgery team members who stated that all the items in the list were fully answered outnumbered the ones stating the contrary. The items that were implemented the most were noted to be questioning the patient's allergy history, preparation and sterilization of the kits to be used in the surgery and confirmation of the unit to which the patient is to be sent after the surgery. It was found that the implementation of `Anesthesia Safety Checklist`, presentation of the surgery team members and confirmation of the patient's identity, type and area of surgery were implemented the least. The results of the study will make contributions to the planning of the attempts made to increase the implementation of SSCTR.Key Words: Nursing, operating room, safe surgery, safe surgery checklist, surgery team, patient safety.
