Samsun bakır işletmelerinde çalışan kişilerin plazma bakır ve çinko düzeyleri ile lipid parametrelerinin ilişkisi

Abstract
ÖZET Son yıllarda koroner aterosklerotik kalp hastalıklarında bilinen risk faktörlerine ek olarak, bakır ve çinko gibi eser elementlerin de etyolojide etkili olabileceği ileri sürülmektedir. Buradan hareketle, bakır ve çinkonun yoğun olabileceğini düşündüğümüz Karadeniz Bakır İşletmeleri Anonim Şirketi, Samsun İşletme Müdürlüğü'nde çalışan kişilerde bakır, çinko, lipid, lipoprotein ve apolipoprotein düzeyleri ve bu parametreler arasındaki ilişkinin belirlenmesi amaçlandı. Karadeniz Bakır İşletmeleri Anonim Şirketi Samsun işletme Müdürlüğü'nde çalışan ve lipid parametrelerini etkileyebilecek herhangi bir hastalığı olmayan erkek işçiler çalışmaya alındı. Burada çalışan kişiler, çalıştıkları bölümlerdeki bakır yoğunluğuna göre iki grupta toplandı. Grup-1'e yoğun olarak bakır ile temas edenler (n=76), grup-2'ye daha az yoğunlukta bakır ile temas edenler (n=23) alındı. Grup 3'de ise, burada çalışmayan yaş ve cinsiyet uyumu olan sağlıklı kişiler kontrol grubu (n=40) olarak seçildi. Grupların bakır, çinko ve lipid parametreleri düzeylerinin gruplar arası istatistiksel karşılaştırılması yapıldıktan sonra birbirleri ile olan korelasyonuna bakıldı. Daha sonra tüm gruplar hiperkolesterolemik (>200 mg/dL ve n=76) ve normokolesterolemik (<200 mg/dL ve n=63) olarak gruplandırılarak bakır ve çinko düzeyleri değerlendirildi. Elde edilen serum numunelerinden kolesterol, trigliserid ve HDL-kolesterol düzeyleri spektrofotometrik; apolipoprotein A-l, apolipoprotein B ve lipoprotein(a) nefelometrik; elde edilen plazma numunelerinden bakır ve çinko düzeyleri atomik absorpsiyon spektroskopi ile ölçüldü. LDL-kolesterol düzeyleri Friedewald formülü ile hesaplandı. Lipid parametreleri, bakır ve çinko düzeyleri: Total kolesterol; grup-1'de 197.13 ± 43.64, grup-2'de 217.22 ± 54.34, grup-3'de 174.05 ± 32.88 mg/dL (aritmetik ortalama ± SD), trigliserid;, grup-1'de 169.14 ± 108.09, grup-2'de 237.30 ±180.03, grup-3'de 129.75 ± 55.13 mg/dL, HDL-kolesterol; grup-1'de 28.54 ± 6.88, grup-2'de 27.54 ± 7.04, grup-3'de 37.05 ± 9.2 mg/dL, LDL-kolesterol; grup-1'de 134.08 ± 34.82, grup-2'de 147.65 ± 45.64, grup-3'de 112.43 ± 36.35 mg/dL, lipoprotein(a); grup-1'de 36.84 ± 12.5, grup-2'de 42.91 ± 10.6, grup-3'de 14.33 ± 8.7 mg/dL, apolipoprotein A-l; grup- 1'de 142.11 ± 18.25, grup-2'de 141.35 ± 21.60, grup-3'de 122.05 ± 18.95 mg/dL, apolipoprotein B; grup-1'de 131.01 ± 32.81, grup-2'de 137.57 ± 36.68, grup-3'de 134.55 ± 148.69 mg/dL, bakır; grup-1'de 110.13 ± 20.23, grup-2'de 108.91 ± 23.55, grup-3'de 106.5 ± 14.64 ug/dL, çinko; grup-1'de 99.93 ± 52.04, grup-2'de 118.48 ± 94.67, grup-3'de 86.15 ± 15.37ng/dL, serüloplazmin; grup-1'de 26.43 ± 4.99, grup- 2'de 25.16 ± 3.86, grup-3'de 28.35 ±15.30 mg/dL olarak saptandı. VIITotal kolesterol, trigliserid, LDL-kolesterol, apolipoprotein A-l, apolipoprotein B, lipoprotein(a) düzeyleri grup-1 ve grup-2'de grup-3'den istatistiksel olarak anlamlı ölçüde yüksek, HDL-kolesterol ise düşük saptandı (sırasıyla p<0.005, p<0.005 p<0.01, p<0.001, p<0.001, p<0.01, p<0.01). Grupların kendi içlerinde bakır ve çinko düzeyleri ile lipid parametreleri arasındaki korelasyon araştırıldığında; grup-3'de, istatistiksel olarak anlamlı bir korelasyon bulunamadığı halde (p>0.05), grup-1 'de; bakır ile total kolesterol, LDL-kolesterol, apolipoprotein B ve serüloplazmin arasında (sırasıyla p<0.05, p<0.01, p<0.05, p<0.001), grup-2'de; bakır ile total kolesterol, LDL-kolesterol, apolipoprotein B ve serüloplazmin arasında (sırasıyla p<0.001, p<0.01, p<0.001, p<0.001) istatistiksel olarak pozitif bir korelasyon saptandı. Grup-1 ve grup-2'de; serüloplazmin ile total kolesterol, LDL-kolesterol, apolipoprotein B arasında da istatistiksel olarak pozitif bir korelasyon saptandı (sırasıyla p<0.01, p<0.01, p<0.01). Grup-1 ve grup-2'de; çinko ile lipid parametreleri arasında istatistiksel olarak anlamlı bir korelasyon bulunamadı. Her iki grupta da aynı sonuçlar elde edildiği için, daha sonra iki grubu bir araya getirerek bakır işletmelerinde çalışanlar ve çalışmayanlar olarak gruplandırdıktan sonra, aralarındaki korelasyona bakıldığında; bakır işletmelerinde çalışanlarda, bakır ile total kolesterol, LDL-kolesterol, apolipoprotein B ve serüloplazmin arasında istatistiksel olarak pozitif bir korelasyon bulundu (sırasıyla p<0.001, p<0.001, pO.001, p<0.001). Aynı şekilde serüloplazmin ile total kolesterol, LDL-kolesterol, apolipoprotein B arasında da istatistiksel olarak pozitif bir korelasyon bulundu (sırasıyla p<0.01, p<0.01, p<0.01). Bakır işletmelerinde çalışmayanlarda ise istatistiksel olarak anlamlı bir korelasyon saptanamadı. Hiperkolesterolemik ve normokolesterolemik olarak yapılan gruplama ise; hiperkolesterolemik grupta bakır düzeyleri istatistiksel olarak anlamlı ölçüde yüksek bulunurken, serüloplazmin ve çinko düzeylerinde istatistiksel olarak anlamlı bir farklılık bulunamadı. Bulgularımıza göre, bakır ile temas halinde olan, bakır işletmelerinde çalışan kişilerde bakır ve çinko düzeyleri normaldir. Ancak plazma bakır ve çinko düzeyleri vücut bakır ve çinko durumunu tam olarak yansıtmayabilir. Bakır ile aterojenik olarak bilinen total kolesterol, LDL-kolesterol, apo B arasındaki ilişki koroner aterosklerotik kalp hastalıklarının insidansında artışa neden olabilir. Ancak şimdiye kadar yapılan araştırmalarda böyle bir ilişkinin mekanizması tam olarak anlaşılamamıştır. Bu nedenle bakır düzeyleri ile lipid parametreleri arasındaki ilişkiyi açıklamak için daha fazla insan çalışması yapılmasına ihtiyaç vardır. Anahtar sözcükler: Eser element, plazma bakır, plazma çinko, lipid, koroner hastalıklar VIII SUMMARY In recent years, in addition to the known risk factors of the heart diseases due to the coronary artery atherosclerosis, it has been suggested that the trace elements such as the copper and the zinc might play an effective role in the etiology. Because of that, in this study, it is aimed to measure the blood copper, zinc, lipid, lipoprotein, and apoprotein levels and to determine the possible interactions between these parameters among the blue collars whose copper and zinc levels would be supposed to be found in high levels. All of them were working in Samsun Exploiting Directory of the Black Sea Copper Exploiting Company. With this purpose, the male blue collars not having any of the disorders affecting the lipid parameters were included in this study. Subjects working in Samsun Exploiting Directory were divided into two groups according to the copper density of their working departments. Subjects having excessive contact with copper are assigned to group-1 (n=76), group-2 was composed of the subjects having lesser contact with copper (n=23), and group-3 was composed of the healthy people matching with the preceding two groups according to age and sexuality (n=40). Firstly copper, zinc and lipid parameters were measured in each of the groups and compared each other, then correlational analysis was carried out. Later the subjects were divided into two groups as hipercholesterolemic subjects (cholesterol level>200 mg/dl, n=76) and normocholesterolemic subjects (cholesterol level<200 mg/dl, n=63) and their copper and zinc levels are measured and evaluated. Total cholesterol, triglyceride and HDL- cholesterol levels are measured spectrophotometrically, apolipoprotein A1, apolipoprotein B, and lipoprotein(a) nephelometrically in serum samples, copper and zinc levels are measured by the atomic emission spectroscopic method. LDL- Cholesterol levels are calculated by using the Friedewald formula. Lipid parameters, zinc and copper levels: Total cholesterol level was 197.13 ± 43.64 mg/dl in group-1, 217.22 ± 54.34 mg/dl in group-2, 174.05 ± 32.88 mg/dl in group-3 (arithmetical mean ± standart deviation); triglyceride level was 169.14± 108.09 in group-1, 237.30 ± 180.03 mg/dl in group-2, and 129.75 ±55.13 mg/dl in group-3; HDL-cholesterol was 28.54 ± 6.88 mg/dl in group-1, 27.54 ± 7.04 in group-2, 37.05 ± 9.2mg/dl in group-3; LDL-cholesterol level was 134.08 ± 34.82 mg/dl in group-1, 147.65 ± 45.64 mg/dl in group-2, and 1 12.43 ± 36.35 mg/dl in group-3; lipoprotein (a) level was 36.84 ± 12.5 mg/dl in group-1, 42.91 ± 10.6 mg/dl in group-2, and 14.33 ± 8.7 mg/dl in group-3; apolipoprotein A-1 level was 142.11 ± 18.25 mg/dl in group-1, 141.35 ± 21.60 mg/dl in group-2, and 122.05 ± 18.95 mg/dl in group-3; apolipoprotein B level was IX131.01 ± 32.8 mg/dl in group- 1, 137.57 ± 36.68 mg/dl in group-2, and 134.55 ± 148.69 mg/dl in group-3; copper level was 110.13 ± 20.23 ng/dl in group-1, 108.91 ± 23.55 (ag/dl in group-2, and 106.5 ± 14.64 (ig/dl in group-3; zinc level was 99.93 ± 52.04 ag/dl in group-1, 118.48 ± 94.67 ng/dl in group-2, and 86.15 ± 15.37 jig/dl in group-3; and lastly ceruloplasmine level was 26.43 ± 4.99 mg/dl in group-1, 25.16 ± 3.86mg/dl in group-2, and 28.35 ± 15.30 mg/dl in group-3. With respect to total cholesterol, triglyserid, LDL-cholesterol, apolipoprotein A1, apolipoprein B, lipoprotein (a) statistically significant higher levels were found in group- 1 and group-2 than that of group-3 (p<0.005, p<0.005, p<0.01, p<0.001, p<0.001, p<0.01 respectively), but HDL-cholesterol level found to be lower (p<0.01). As for the correlations between the copper, zinc levels and the lipid parameters within each of the group, although a meaningful correlation could not be found in group-3 (p>0.05) a statistically significant possitive correlation was found between the copper and the total cholesterol, LDL-cholesterol, apolipoprotein B, and ceruloplasmine levels in group-1 (p<0.05, p<0.01, p<0.05, and p<0.001 respectively) and between the copper and the total cholesterol, LDL-cholesterol, apolipoprotein B, and ceruloplasmine levels in group-2 (p<0.001, p<0.01, p<0.001, p<0.001 respectively). Similarly, a statistically significant possitive correlation between the ceruloplasmine and the total cholesterol, LDL-cholesterol, apolipoprotein B level was found in group-1 and group-2 (p<0.01, p<0.01, p<0.01 respectively). In group-1 and group-2, any statistically significant correlation between the zinc and the total cholesterol, LDL-cholesterol, apolipoprotein B could not be found. Because the same results were obtained in both of the groups, we classified them into two new groups; the subjects working in the Copper Exploiting Directory and the subjects not working in the Copper Exploiting Directory. In this case a possitive statistically significant correlation was found between the copper and the total cholesterol, apolipoprotein B, ceruloplasmine levels in subjects working in the Copper Exploiting Directory (p<0.001, p<0.001, p<0.001, p<0.001 respectively). In the same way a statistically significant correlation was found between the ceruloplasmine and the total cholesterol, LDL-cholesterol, apolipoprotein B levels (p<0.01, p<0.01, p<0.01 respectively). Any statistically significant correlation was not found among the subjects not working in the Copper Exploitry. Lastly.when subjects were grouped as hypercholesterolemic and normocholesterolemic, altough the copper level was found in higher levels in the hypercholesterolemic group and this was statistically significant (p<0.001), any statistically significant difference was not found for the ceruloplasmine and the zinc levels (p>0,05, p>0.05).Finally, the copper and the zinc levels of any subject having contact with these elements and working in the copper exploiting company are in the normal range. However plasma copper and zinc levels may not reflect the total body copper and the zinc status exactly. The total cholesterol, LDL-cholesterol and apoprotein B are well- known atherogenic substrates; and the interaction between the copper and these substrates may cause a risk in the coronary atherosclerotic heart disease incidence. However, more human studies are required to classify the interaction between the copper levels and lipid parameters.
