Abstract
ÖZET Amaç: Çalışmamızda kronik viral hepatitin (KVH) progresyon takibinde, sirozun tanı ve derecelendirilmesinde renkli dupleks Doppler ultrasonografinin (RDUS) değerini ortaya koymayı amaçladık. Gereç ve Yöntem: 99 olgu (36 KVH, 63 siroz) ve 30 sağlıklı olgu RDUS ile incelendi. Tüm olgularda karaciğer ve dalak boyutu, portal ve splenik ven çapı, portal ven akım hızı (PVH), debisi, akım yönü ve pulsatilite skoru, hepatik arter rezistans indeksi (HaRI) ve pulsatilite indeksi (HaPI), hepatik vasküler indeks (HVI), arteriyoportal indeks (API), hepatik vende ters akım komponentinin kaybı (TAKK) ve fokal hız artışı değerlendirildi. KVH'lı olgular histolojik aktivite indeksi kullanılarak derecelendirme ve evrelendirmeye göre, sirozlu olgular ise Child sınıflaması, kollateral ve asit varlığına göre alt gruplara ayrıldı. Elde edilen Doppler parametrelerinin bu alt gruplardaki istatistiksel farklılığı değerlendirildi. Bulgular: HaRI, HaPI, HVI ve API değerleri üç grup arasında anlamlı derecede farklıydı (p<0.001). PVH siroz grubunda diğer iki gruba göre azalmıştı (p<0.001). KVH'lı olgularda HaRI ve API inflamasyonun hafif ve şiddetli dereceleri arasında, PVH ve API fibrozisin birinci ve üçüncü evreleri arasında anlamlı farklılık göstermekteydi (p<0.05). Hepatik vendeki TAKK fibrozisin evresi ve karaciğer yağlanması ile ilişkiliydi. Sirotik olgularda ise sadece kollateral ve asit varlığı ile hepatik vendeki TAKK arasında ilişki bulundu. HaPI >1.30, HVI <15 cm/sn, API <29 cm/sn olarak alındığında sensitivite değerleri sırası ile %90, %81 ve %83 idi. Tüm parametrelerin spesifisitesi %100 idi. Sonuç: RDUS, KVH'daki nekroinflamasyon ve fibrozisin progresyon takibinde kullanışlıdır. Sirozda ise yüksek tanı değerine sahip iken klinik derecelendirmede yeri sınırlıdır. Anahtar Kelimeler: Siroz, Kronik hepatit, Portal ven, Hepatik ven, Hepatik arteriyel indeks IV ABSTRACT Purpose: The aim of this study is to assess the value of color duplex Doppler sonography (CDDS) in the follow up of progression in chronic viral hepatitis (CVH) and in the diagnosis and grading of cirrhosis. Materials and Methods: Ninety-nine patients (36 CVH, 63 cirrhosis) and 30 controls were examined by CDDS. The parameters evaluated by CDDS were the dimensions of liver and spleen, portal and splenic vein diamaters, pulsatility score, blood flow, direction and flow velocity (PV) in portal vein, hepatic arterial pulsatility index (HaPI) and resistance index (HaRI), liver vascular index (LVI), arterioportal index (API), loss of the reverse flow component (LRFC) and existence of focal flow acceleration in the hepatic veins. In CVH cases, differences between subgroups, which were divided according to grading and staging, were observed with regard to each Doppler parameter. In cirrhotic patients were observed for Child classification, collateral and refractory ascites formation. Results: There were significant differences among all these three groups, for the values of HaPI, HaRI, LVI and API (p<0.001). For the splenic size, portal and splenic vein diamaters and PV, there were significant differences between cirrhotic and other groups (pO.001). In CVH for the HaRI, API, there were significant differences between the subgroups having mild and severe inflammation. In the similar way, there were significant differences between the subgroups of fibrous stage I and III for the PV and API. LRFC was related to fibrous stage and fat deposition. However only presence of collateral vessels and ascites were found to be related with the LRFC in hepatic veins in cirrhotic cases. For the parameters of the cirrhosis cohort, HaPI exceeded 1.30, LVI less than 15 cm/sec, API less than 29 cm/sec had a sensitivity of 90%, 81% and 83%, respectively. The specificity of all of these parameters was 100%. Conclusion: We suggest that CDDS is useful in follow up of progression in necroinflammation and fibrosis in cases with CVH. Although CDDS has a high diagnostic accuracy in cirrhosis, it has a limited role in clinical grading. Key Words: Cirrhosis, Chronic hepatitis, Portal vein, Hepatic vein, Hepatic arterial index V
