Abstract
ÖZET Amaç:Lokal ileri (evre IHA ve IIIB) küçük hücreli dışı akciğer kanserinde standart tedavi olan primer radyoterapi ile sağlanan genel sağkalım ve genel progresyonsuz sağkalım sonuçları kötüdür. Bu çalışmada neoadjuvan sisplatin-vinoralbin kemoterapisi sonrası uygulanan akselere radyoterapi {konkomitan boost) şemasının etkinliğinin araştırılması amaçlanmıştır. Yöntem: Temmuz 1999 ile Ekim 2001 yılları arasında yaşı 70 ve altında, Karnofsky performansı 60 ve üzerinde olan ve sistemik kemoterapi almasına engel herhangi bir medikal kontrendikasyonu olmayan 3 1 hasta çalışmaya dahil edilmiştir. Hastaların 29'u erkek, 2'si kadındır. Oniki hasta 60 yaş ve altında, 19 hasta 60 yaş üzerindedir. Patolojik tanılar Dünya Sağlık Örgütü'nün (DSÖ) önerdiği sınıflama esas alınarak değerlendirilmiştir. Yirmialtı hastada epidermoid karsinom saptanırken, 5 hastada adenokarsinom gözlenmiştir. Hastalara; sisplatin l.ve 22. günler, vinoralbin 1., 8., 22. ve 30. günler olacak şekilde iki siklus neoadjuvan kemoterapi uygulanmıştır. Sisplatin dozu 18 hastada 75 mg/m2, 13 hastada 100 mg/m2,dir. Vinoralbin tüm hastalara 30 mg/m2 olarak uygulanmıştır. Son kemoterapi küründen iki hafta sonra radyoterapi başlanmış, primer tümör ve bölgesel lenf bezlerinden oluşan geniş alana, günde 2 Gy ile 25 günde toplam 50 Gy verilmiştir. Bu tedavinin son sekiz gününde primer tümör bölgesine, geniş alan ışınlamasından en az altı saat sonra, 1.8 Gy ile 14.4 Gy konkomitan boost yapılmıştır. Toplam tümör dozu 64.4 Gy'dir. Bulgular: Çalışmaya alınan 31 hastanın 6'sı birinci kür kemoterapi sonrası gelişen grade 3 bulantı-kusma nedeni ile kendi istekleri ile çalışmadan ayrılmışlardır. Bir hasta ikinci kür kemoterapi sonrası gelişen akciğer enfeksiyonu nedeni ile kaybedilmiştir. Kemoterapi sonrası radyoterapi alan 24 hastanın bulguları değerlendirilmiştir. Kemoterapiye bağlı grade 3 toksisite olarak, 9 hastada lökopeni, 8 hastada bulantı-kusma gözlenmiştir. Radyoterapiye bağlı grade 3 akut toksisite olarak 4 hastada özefajit, 5 hastada pnömoni gözlenmiştir. Diğer erken dönem yan etkiler tolere edilebilir düzeydedir. Çalışmamızda grade 3-4 geç toksisite gözlenmemiştir. Medyan sağkalım 1 1 ay; radyoterapi sonrası elde edilen objektif yanıt oranı %75; bir yıllık genel sağkalım %38.4; bir yıllık lokal progresyonsuz sağkalım %42; bir yıllık uzak hastalıksız sağkalım %65'dir. IVSonuç: Neoadjuvan kemoterapi ve konkomitan boost akselere radyoterapi sonuçları konvansiyonel fraksiyone radyoterapi sonuçlarından daha iyidir. Ancak tek başına konkomitan boost tekniğinden daha iyi değildir ve gözlenen toksisite oranları konkomitan boost tekniğine oranla daha yüksek bulunmuştur. Bu bilgiler ışığında bu yöntemin tolere edilebilir bir yöntem olduğunu söylemek ve standart tedavi yöntemi olarak kabul etmek güçtür. Standart tedavi yöntemi olarak kabul edilebilmesi için çok sayıda hastayı kapsayan prospektif randomize çalışmalara ihtiyaç vardır. V ABSTRACT Purpose: The overall survival and progression free survival rates provided by standard primary radiotherapy for the locally advanced (stage MA and IIIB) non-small cell lung cancer are poor. In this study, we aimed to investigate the efficacy of the accelerated radiotherapy (concomitant boost) after neoadjuvant cisplatin-vinoralbine chemotherapy. Methods: Thirty-one patients who were <70 years old, who had a Karnofsky performance of >60 and had no medical contraindication for systemic chemotherapy were included to this study between July 1999 and October 2001. Of these patients, 29 were male and 2 female. Twelve patients were <60 years old and 19 patients >60 years old. The pathologic diagnoses were evaluated according to the classification recommended by WHO. Twenty-six patients were epidermoid carcinoma and 5 patients were adenocarcinoma. Patients received two cycles of neoadjuvant chemotherapy; Cisplatin on days 1 and 22, vinoralbine on days 1, 8, 22, and 30. In 18 patients the dosage of cisplatin was 75 mg/m2, and in 13 patients 100 mg/m2. Vinoralbine was given 30 mg/m2 to all patients. Two weeks after the last chemotherapy cycle, radiotherapy was started and given to a large-field including the primary tumor and regional lymph nodes as on daily of 2 Gy for completing 50 Gy in 25 days. In the last days of this therapy a concomitant boost of 1.8 Gy to 14.4 Gy was applied to primary tumor at least 6 hours, after the large-field irradiation. Total tumor dose was 64.4 Gy. Result: Six of the 3 1 patients discontinued the study, because of grade 3 nausea and vomiting after the first chemotherapy cycle, one patients had died because of pneumonia developed after the second chemotherapy cycle. The results of 24 patients who received radiotherapy after chemotherapy were assessed. As grade 3 toxicities seen due to chemotherapy were nausea and vomiting in 8 patients and leukopenia in 9 patients. As acute grade 3 toxicities seen due to radiotherapy, esophagitis was seen in 4 patients, pneumonia in 5 patients. The other early adverse effects were tolerable. In our study no late grade 3-4 toxicity was observed. As a result of median follow-up 8 months; the median survival was 11 months, the objective response rate to radiotherapy was 75%, one-year overall survival rate was 38.4%, one-year locally progression free survival rate was 42%, one-year distant disease free survival was 65%. VI ~sss»ssrConclusions: The neoadjuvant chemotherapy followed by accelerated radiotherapy has better results when compared with conventional fractionated radiotherapy. But it has similar results and higher toxicities when compared with the concomitant boost technique alone. Based on these results, it is difficult to say that this protocol is tolerable, and difficult to accept as a standard therapy. More randomized prospective studies that includes large series of patients are required to accept this protocol as a standard therapy.
