Abstract
tu ÖZET Akut serebral iskemili hastalarda serum soluble IL-2 ve İL- 6 düzeyleri Bu çalışmada, akut cerebral iskemili hastalarda IL-2 ve IL-6'nın serum seviyeleri ölçüldü. Aynı çalışmada bu sitokin düzeyleri ile BBT'de enfarkt hacmi, BBT'de enfarkt yerleşimi, lökosit sayısı, prognoz ve Glasgow Outcome Skalası (GOSs) düzeyleri arasında ilişkinin mevcut olup olmadığı da araştırıldı. Bu amaçla çalışma 51 hasta ve 40 sağlıklı kontrolde yapıldı. IL-6 düzeyleri hastalarda (1 16,16±101,24 pg/ml) kontrol grubundan (14,41±8,79 pg/ml) daha yüksekti (p< 0,0001) ve IL-2 seviyelerinde hasta (0,9432+0,6811 u/ml) ve kontrol grubu (0,7728±0,4012 u/ml arasında gözlenen farklılık istatistiksel olarak önemli değildi (p>0,05). Sonuçların istatistiksel değerlendirmesi serum IL-6 düzeyleri ile BBT'de enfarkt hacmi ve IL-6 düzeyleri ile BBT'de kortikal yerleşim arasında önemli korelasyonların olduğunu gösterdi. Lökosit sayısı ile kortikal yerleşim ve lökosit sayısı ile enfarkt hacmi arasında önemli bir korelasyon mevcuttu. Serum IL-6 ve grup 2 arasında gözlenen korelasyon önemliydi. GOSs'u 1-3 olan hastaların hafif klinik defısitleri vardı ve prognozları iyiydi, GOSs'u 4-5 olan hastalarda ciddi ölçüde sakatlık ve kötüleşen prognoz gözlendi. IL-6 düzeyleri yüksek olan hastalarda da prognoz kötüydü ve serum IL-6 seviyeleri ile ACM enfarktı arasında istatistiki açıdan önemli bir korelasyon mevcuttu. Diğer korelasyon parametreleri istatistiksel önem göstermedi. Elde edilen sonuçlar, akut cerebral iskemili hastalarda IL-6 seviyelerinin yüksek, IL-2 düzeylerinin ise normal olduğunu ve enfarkt hacmi arttıkça IL-6 seviyelerinin de arttığını gösterdi. Akut cerebral iskeminin patogenezinde serum IL-6 seviyelerinin önemli olduğu sonucuna varıldı. Akut cerebral iskemide sitokinlerin rolünü ayrıntılı biçimde aydınlatmak için daha detaylı çalışmaların yapılması gerekmektedir. IV SUMMARY Serum IL-2 and IL-6 levels in patients with acute cerebral ischemia In present study, serum IL-2 and IL-6 levels were determined in patients with acute cerebral ischemia. It was also investigated whether relationships between the selected cytokine levels and the infarct volume in computerized tomography (CT), the infarct localisation in CT, white blood cell count, the prognosis, and GOSs values were present. For this purpose 51 patients and 40 healthy control were included in the study. The patients had higher levels of IL-6 (116,16±101,24 pg/ml) than control levels (14,41 ±8,79 pg/ml) (p< 0,0001), and the difference in IL-2 between the patients (0,9432±0,681 1 u/ml) and controls (0,7728±0,4012 u/ml) was not of statistical significance (p>0,05). The statistical evaluation of the results showed significant correlations between serum IL-6 levels and infarct size in CT and cortical localization in CT. A significant correlation was present between white blood cell count and cortical localisation and infarct size. The correlation between serum IL-6 and group 2 was significant. The patients having GOSs of 1-3 showed light clinical deficits and good prognosis, and those having GOSs of 4-5 severe sequele and poor prognosis. On the other hand, the patients having high IL-6 levels also had poor prognosis, and there was a statistically significant correlation between serum IL-6 levels and ACM infarct. The other correlation parameters showed no statistical significance. The results obtained showed that the patients with acute cerebral ischemia had high levels of IL-6 and normal levels of IL-2 and that the larger enfarct size and the higher levels of IL-6. It was concluded from the results that serum IL-6 levels are important in pathogenesis of acute cerebral ischemia. However, the further studies are needed to elucidate in detail the role of cytokines in acute cerebral ischemia.
